Often, deciding what is best for a loved one is the most difficult decision that families face. Many attempt to care for their loved one only to find that the task is much more demanding than they anticipated. At Woodland Village Rehabilitation & Healthcare Center and Woodland Haus Assisted Living we understand the problems families face and our goal is to make this transition in their lives easier. You will find an environment of care exists at our facility and is practiced daily by our dedicated staff as they work to assist each resident maintain their dignity and independence as much as possible. We offer planned nursing care that is based on each resident s individual needs and abilities with the goal of achieving an optimal quality of life. Woodland Village Rehabilitation & Healthcare Center is a 149 bed Medicare/Medicaid Certified nursing facility providing skilled therapy and nursing services for both sub-acute and long term needs of the seniors in our community. Physical Therapy is a treatment method addressing the treatment, healing and prevention of injuries and disabilities. The goal is to restore normal movement and function while attempting to resolve problems such as pain, weakness, loss of motion, or lack of coordination. Our therapy staff understands the many challenges involved in the rehabilitation process, and with their significant experience in rehabilitation, they are able to overcome many of the barriers and produce positive results. Our therapists place special emphasis on working with the families in conjunction with the resident to ensure the plan of care developed for the resident is individualized and designed to offer the resident the best opportunity for a successful rehabilitation. Occupational Therapists work with individuals who have physical, developmental, emotional, or social deficits that affect their ability to participate in daily activities. The therapist helps the patient relearn daily living skills such as the ability to get dressed, walk, or perform routine functions. Our occupational therapy program maximizes the patient s ability to participate in the self-care, work related, leisure, and social activities of daily living. Patients who have difficulty dressing, bathing, and feeding themselves, have limitations in mobility, or who experience memory, attention, or problemsolving deficits that restrict independence and impair safety, will benefit from occupational therapy.
Speech Therapy Many residents who have suffered from strokes, brain injury, Alzheimer s or other neurological disorders often lose the ability to speak and may have difficulty understanding spoken and written material. Through careful evaluation, planning, and treatment programs, speech-language therapists work at improving a resident s communication skills. Therapy can stimulate and improve the resident s speaking, reading, writing, comprehension and cognitive ability. Our speech therapy program restores or improves the patient s communication abilities, speech/ language deficits, hearing impairments, and/or swallowing abilities. Ultrasound - Used to decrease pain, increase circulation to enhance tissue healing, increase range of motion and to increase cell membrane permeability. Shortwave Diathermy - Used for pain reduction, edema reduction, increased circulation to improve tissue healing, decrease muscle spasms and increase range of motion in the joint capsule, muscle fascia and scar tissue. Electrotherapy (Estim) - Use of alternating or direct current waveforms applied through electrodes to the body for pain control, decrease muscle spasms and increase range of motion.

Spacious private rooms are personalized by each resident s preference. Residents are encouraged and free to decorate and furnish their rooms with their own personal belongings. We encourage personal items such as pictures and other belongings that will make for more familiar surroundings and the transition to a new environment less difficult. All items brought from home are added to an Inventory Record and approved by the Maintenance Department. Woodland Village Rehabilitation & Healthcare Center has a large, oversized Activity and Common Area that is utilized for Activities, Special Events, Performances, Birthday Parties, Thanksgiving and Christmas Events, Bingo, socializing, watching TV, drinking coffee, playing Wii, games, reading and visiting with other residents or family.
If the time has come to choose a home for you or your loved one... let us help. Whether it s a short rehab stay or to be your new home, there are things to consider. Tour the facility and meet the Administrator, Admission Staff and the Director of Nursing. Ask questions and contact us with any concerns or suggestions that would help your loved one or our facility. Visitation is recommended between the hours of 7:00 am to 8:30 pm, but is allowed at any time. Admission Process - Bring insurance papers/cards, Medicare/Medicaid cards. List of contacts, in case of emergency. The sponsor coordinates with the facility any appointments for doctor or procedure visits outside the facility and provides for transportation. Should a need arise to assist in a special circumstance, please contact the Social Service Department in efforts for us to be of assistance. List of current health problems and medications. Do not bring medications from home, the pharmacy will provide meds on Admission. Upon discharge remaining meds will be sent home with resident. Meds are not allowed in a resident s room without prior approval and without being Care Planned. Items your loved one will need are seasonal and rehab clothing, undergarments, shoes and non-skid socks. Eyeglasses, dentures and/or hearing devices. Vinyl or leather recliner, space and safety/inspection permitting. Pictures and small items to personalize surroundings. Items not allowed are scissors, fingernail clippers, nail polish/remover, air fresheners, candles, matches, lighters, heating pads, extension cords, coffee pots and electric blankets. Curtain, spreads or blankets will be inspected and fire retardant spray applied prior to use. All electrical equipment will be inspected prior to use. We are a smoke-free facility, however residents are allowed supervised smoking in a secure outdoor area. Cigarettes are maintained by the Charge Nurse. We provide 24 hour licensed nurse coverage. Please allow our skilled and professionally trained staff the opportunity to assist you and your loved one during this transition and anytime throughout your stay with answers and solutions to your questions.
