1 A Guide to Inpatient Rehabilitation Our Facility St. Joseph s Health Centre is a fully accredited healthcare organization specializing in Rehabilitation, Long-Term Care, and Complex Continuing Care. We are located at 100 Westmount Road in Guelph, Ontario. The rehabilitation unit is located on the 1 st floor of the Health Centre. The phone number is extension Our Programs o Short Term Rehabilitation Program Short Term Rehabilitation is a 12 bed inpatient program with a minimum stay 4 days and a maximum stay of 14 days. o Long Term Rehabilitation Program Long Term Rehabilitation is a 15 bed inpatient program with a minimum stay of 14 days and a maximum stay of 40 days. The Admission Process You have been informed that you would benefit from an inpatient rehabilitation program at St. Joseph s Health Centre. If you meet the criteria for one of our programs preparations will be made for your admission to the rehabilitation unit. If there is no bed available your name will be placed on a waiting list for admission. Some of the criteria include: Medical stability Ability to participate and learn Consent and willingness to participate Accommodation The Health Centre offers private and semi private accommodation. If you have insurance coverage for private or semi private accommodation you

2 will be asked to sign for your accommodation so that our Business office can bill your insurance company. Visiting Visitors are welcome at any time. We ask that visitors be respectful and consider your scheduled therapy times. The main entrance on Westmount Road is unlocked from 6 am to 8:30 pm daily. Visitors who are not feeling well should not visit until they have been well for 48 hours. Consider a phone call instead. Your Rehabilitation Team A team of health care workers will work with you to help you meet your rehabilitation goals. The rehabilitation team includes: You and your family members or significant support persons Nurse Practitioner Community Care Access Centre Case Manager Dietitian Doctors Horticultural Therapist Nurses Occupational Therapists Pharmacist Physiotherapists Recreational Therapist Rehabilitation Assistants Social Workers Speech Language Pathologists Spiritual and Religious Caregivers Expectations Daily participation in all aspects of the rehab program is the most important way for you to meet your rehabilitation goals. If for any reason you are unable to participate in the rehab program you may be discharged home or returned to the facility you came from prior to rehabilitation. Sometimes people complete the rehab program, but are not ready for discharge. In this case you may return to the facility you were referred from for further care planning.

3 Discharge Planning Discharge planning begins prior to being admitted to the rehabilitation program. Your discharge plan will be continually reevaluated to ensure that you are discharged to the most appropriate setting to meet your needs. Not everyone goes home following their rehab stay. There may be a difference between what a person wishes to achieve and what they are able to achieve in terms of safe independence. Some people may go to long term care homes, retirement homes, continuing complex care units, assisted living or supportive care. Once your discharge date has been set you will need to make arrangements to leave the unit by 10:00 am on that day. Things to Bring To help protect your personal belongings please make sure that they are marked with your name prior to coming. We are not responsible for any lost items. We recommend leaving valuables and money at home. Please bring the following items when you come to rehab: Advanced Health Care Directives If you have a Living Will/Advanced Health Care Directives, or Power(s) of Attorney please bring copies of these documents with you when you are admitted. If you have questions about these documents, we will assist you in obtaining necessary information. Your health care team will discuss Advanced Health Care directives with you on admission. Shoes It is very important that you bring 1 pair of flat, easy to put on, walking shoes with gripping soles (example running shoes with rubber soles) and 1 pair of slippers. Clothing You will be dressing the way you do at home. Bring a few changes of comfortable clothes (e.g. track pants, slacks, sweatshirts, cardigans, t-shirts, blouses, shirts, nightgowns/pajamas and housecoat). Include a light jacket or coat to fit the season. Toiletries Usual toiletry items such as deodorant, make-up, mouth care products, hair brush, comb, mirror, shampoo, electric razor, hand

4 cream, nail care kit. If you use incontinence products please bring them with you Other personal items Pictures, calendar, your own warm blanket, comforter or pillow etc. Assistive Devices If you use dentures, glasses, hearing aids, walker, cane, wheelchair, ostomy supplies and/or blood sugar monitor please bring them with you. Electrical Appliances Please do not bring any electrical appliance other than electric razor until you have discussed it with your health care team. The electrician must certify all electrical equipment (such as hair dryers, radios etc.) before it can be used. Humidifiers, small room heaters, heating pads and microwaveable bean bags for heating pads are not allowed for safety reasons. Medications Your medications will be supplied. Please bring your medications on admission so the team can review them with you. Laundry Laundry is your responsibility. Family members may launder your clothes for you or you may use a washer and dryer located close to the rehab unit. Laundry detergent is not supplied. Family Involvement We believe that involvement of family and significant support persons is critical to the successful outcome of your rehabilitation program. Family members or significant support persons are encouraged to participate in therapeutic activities and therapy sessions. We encourage open communication between family members, significant support persons and staff. We will ask for your feedback as you complete your rehabilitation program. With your permission, meetings with family and significant support persons are organized as required. Smoking St. Joseph s Health Centre is a smoke-free facility. Patients who smoke must smoke outside, 9 meters (30 feet) away from any entrance way to the

5 building. We encourage you to take this opportunity to quit smoking; please speak with the team or your doctor and we can assist you in the process. Allergies In order to create a safe environment for those with allergies, we discourage the use of perfumes by our staff, patients, residents, clients, and visitors. We also ask that no latex balloons be brought into the building. Please inform nursing staff of any allergies you may have. Transportation Your family members/ significant support persons are encouraged to take you for your appointments if you are able to travel in a car. You will be responsible to organize and pay for transportation required for appointments arranged by you or your family. Discharge transportation and costs are your responsibility also. Transportation that is required for medical tests ordered by your rehabilitation team will be organized and paid for by the Health Centre. Welcome!
