Antioch Community High School 2012-13 Local Scholarship Application (Return applications to the Guidance Office by 3 p.m. on Thursday, February 21, 2013) Local scholarships are provided to ACHS seniors by local service organizations and memorial funds. Please place a by all those scholarships for which you are applying and for which you qualify. Applicants are judged based on this application and a transcript of grades, with varying emphasis on academics, leadership and financial need. ALL APPLICATIONS MUST BE COMPLETED ONLINE USING THE PDF FORMAT. Scholarship recipients will be recognized at Honors Night on Wednesday, May 8, 2013. Please sign if you give Antioch Community High School permission to release a copy of your transcript of grades, including rank-in-class and verification of your college entrance exam scores, to the selection committees of the scholarship programs to which you ve applied. By your signatures, you and your parent/guardian also affirm that the information in this application is accurate to the best of your knowledge. Maximize your chances by making your application complete, attractive and readable. Your application will not have your name on it; therefore, the first impression for selection committees will be how your application looks. 1. Access: You may access the scholarship application via email attachment or online at www.sequoits.com on the Guidance Department website. Type in the required information and submit a printed copy. Do not staple the pages. 2. Legibility: Students should proof-read and spell check their application.. 3. Anonymity: These applications are to be as anonymous as possible, so do not write names or give details intended to make your identity known. 4. Thoroughness: Answer every question completely. 5. Notification: If you are chosen as the recipient of one of these scholarships, you will be invited to Honors Night where you will be acknowledged. At that time you will also be given information on how you will receive the scholarship money. Good luck! 6. Recognition: If you accept a scholarship from a college or organization (other than those on this application), this would be another opportunity for recognition at Honors Night on May 8 th. Please notify Ms. Lindgren or Mrs. Peck in the Counseling & Guidance Office and complete an Honors Night Information Form by Monday, April 1 st. These forms will be available later in the spring. 7. Name Local Scholarship Opportunities PLEASE PLACE A BY THE SCHOLARSHIPS FOR WHICH YOU WISH TO BE CONSIDERED. 885 Civic Club Scholarship (2) $1,000 scholarships awarded to seniors in good standing who plan to pursue a post-secondary education at a college, university, technological or trade school. ACHS Student Council Scholarship $500 scholarships to (3) seniors with a minimum of two years service in Student Council. Students must be in good standing and be active leaders in Student Council. Scholarships may be used for a 2- or 4-year college, university or trade school. American Midwest Bank Scholarship $500 scholarship will be awarded to a graduating high school senior who has actively participated in community service on his/her own or through such clubs as Interact, Blood Drive, Teen Court, Peer Mediation, FCA, Student Council or NHS. Must be planning to enroll in a 2- or 4-year postsecondary institution in the fall of 2013. Must include a letter describing his/her involvement in the club or individual community service. Antioch American Legion Post #748, Endowment Fund Scholarship $1,000 award for a senior who has at least one ancestor or blood relative who is a war veteran or has had military service. This award must be applied toward advancement of the senior s educational opportunities and is based on financial need. You must include a description of your relationship to the veteran on a separate piece of paper. Antioch Chrysler Dodge Jeep $1,500 scholarship for student planning to attend a 2- or 4-year college, technical or trade school. Above-average GPA. Open to any field of study. Antioch Lion s Club Scholarships: Academic Scholarship $600 award to (2) seniors intending to enroll in a 4-year college or university. Applicants must reside in Antioch Township or be a children/grandchildren of an Antioch Lion s member. Minimum GPA of 3.0 required. Winners will be expected to help the Lion s Club at their chicken BBQ the first Sunday in August Andrew Wade Male Sports Person of the Year Scholarship $600 award to a male senior intending to enroll in a 2- or 4-year college, university or vocational school. Applicants must reside in Antioch Township or be children/grandchildren of an Antioch Lion s member, have participated in athletic activities over four years of high school, and carry a minimum GPA of 3.0. Winners will be expected to help the Lion s Club at their chicken BBQ the first Sunday in August. Female Sports Person of the Year Scholarship $600 award to female senior intending to enroll in a 2- or 4-year college, university or vocational school. Applicants must reside in Antioch Township or be children/grandchildren of an Antioch Lion s member, have participated in athletic activities over four years of high school, and carry a minimum GPA of 3.0. Winners will be expected to help the Lion s Club at their chicken BBQ the first Sunday in August. Vocational Scholarship $600 award to (2) seniors intending to enroll in a 2-year college or vocational school. Applicants must reside in Antioch Township or be children/grandchildren of an Antioch Lion s member, have participated in vocational activities over four years of high school, and carry a minimum GPA of 2.5. Winners will be expected to help the Lion s Club at their chicken BBQ the first Sunday in August. Ed Vos Fine Arts Scholarship $500 award will be given to a student who has been involved in and will continue his/her education in the fine arts.

Antioch Woman s Club $500 scholarships will be awarded to (2) seniors planning to earn a bachelor s degree from the College of Lake County and/or a 4-year college or university. Captain Benjamin Donald Jansky Memorial Scholarship $1,000 scholarship to a student planning to enroll in a college ROTC program and/or elementary/secondary education program. Captain Jansky, a 1994 graduate of ACHS, was killed in Iraq in 2005. Chicago Kickers Andrew Wade Memorial Scholarship One or two scholarships totaling $1,000 to student(s) planning to attend a 2-year or 4-year college, technical or trade school. Selection will be based on academics, school activities, community involvement, and athletic achievement. Applicants must have participated in a community or school soccer program. Community Scholarship Antioch Junior Woman s Club will present a $600 scholarship to a female student in or planning to attend a 2- or 4-year college or university. Strong consideration given to level of volunteerism, activities and community service involvement. Additional consideration given to financial need. GPA must be a minimum of 3.0. District 117 Education Association ACHS Scholarship $1,000 scholarship awarded to a senior planning to become a teacher. District 117 Viking Football and Cheerleading Scholarship $1,000 scholarship offered in each category to a District 117 student who participated in either the Viking Football or Cheerleading Program for at least two years in grammar and/or middle school. Applicant must have a minimum GPA of 3.0 and plan to enroll in a 2- or 4-year college, trade or technical school. In addition to completing the local scholarship application, please answer in 250 words or less, How did your experience with the Viking Football or Cheerleading Program help prepare you for college? Indicate if you were a football player or a cheerleader, and attach the typed essay to the application. Dolly Spiering/Florence Brown Scholarship (4) $1,000 scholarships awarded by the Antioch Senior Center to two female and two male college-bound seniors making a difference. Applicants will be judged on their community service through the high school, their GPA and on financial need. Donna Costoff Memorial Scholarship $500 award to a student planning a health career. Harold Redding Memorial Masonic Scholarship $1,000 award will be given to a senior planning to attend an Illinois school. Hispanic Leadership Scholarship $250 scholarship to a Hispanic female pursuing a career in the science or technology field. Applicants must be enrolling in a 4-year college/university for the 2013-14 school year and have a minimum 3.5 GPA. Jim Corrigan Memorial Scholarship $2,000 award to student who plans to pursue study in the humanities or education. Applicants must provide evidence of college enrollment, maintain a 2.5 GPA on a 4.0 scale, and indicate financial need. John J. Kakacek Memorial Athletic Scholarship $500 award will be given to an athlete who will enroll in a 2- or 4-year school, has been involved in school activities and who has shown leadership. John M. Cunnea Business Scholarship $2,000 scholarship to a student planning on studying business at a 2- or 4-year school. Applicants are judged based on their application and a transcript of grades, with varying emphasis on academics, leadership and financial need. Kyrie A. Brown Memorial Scholarship This award is in memory of Kyrie A. Brown, class of 2010, and has been established by her grandparents. A $1,000 scholarship will be awarded to a student who has overcome challenges in order to turn his/her life around. This scholarship is available to students who plan to enter a 2- or 4- year college or university, technical school, or trade school. As part of this application, please include a paragraph detailing the challenges you have faced and how you overcame them.

Louise Vos Memorial Scholarship - $1,000 scholarship to (2) special education students planning on continuing their education at a 2- or 4-year institution, including trade and technical schools. Applicants are judged based on application, academics, leadership and financial need. Mary Carney Memorial Scholarship $500 award to a student planning a health career, preferably in nursing. Northern Illinois Conservation Scholarship A $1,000 scholarship to a student interested in an environmentally based degree. Student should plan to enroll in a 4-year college. Applicants will need to write a 500-word essay stating how their degree will benefit the environment. Roger Alm Memorial Scholarship $300 to a graduating senior planning to attend a post-secondary school. GPA is not a consideration. Sarah Menoni Memorial Scholarship This award is in memory of Sarah Menoni, Spanish teacher at Antioch Community High School from 2006-2010. A $500 scholarship will be awarded to a senior planning to become a teacher. Applicants must have a minimum GPA of 3.0, and additional consideration will be given to financial need. State Bank of the Lakes CLC Scholarship $2,500 to a graduating ACHS senior for the purpose of supporting a post-secondary education at the College of Lake County. The $2,500 award is to be used for tuition and book expenses and will be paid in increments of $1,250 for each year of attendance, not to exceed two years. Students must maintain a 2.0 grade point average during this two-year period. Grades and proof of enrollment will be provided to the bank, prior to issuing payment to the school. Suraya Brook Memorial Scholarship $200 to senior who has studied Spanish in high school and who will continue to study Spanish in college. Terry Korom Memorial Scholarship Antioch Junior Woman s Club will present a $600 scholarship to a female student in or planning to attend a 2- or 4-year college or university. Requires student to have held a leadership position in an academic organization. Additional consideration given to financial need. GPA must be a minimum of 3.0. William Seemann Memorial Scholarship $500 award will be given to a senior who has been active in athletics and who is planning to pursue a career in science. Please consider me for any other scholarships which become available at a later date. PLEASE NOTE: The Antioch Rotary Club has opted to utilize a separate application this year.
