1 Post-High School Student Health Care Career Scholarship 1 - $750 scholarship awarded by Mayo Health System in Fairmont to posthigh school student in the medical center s service area, with plans to pursue a health care career. Scholarship money is mailed to the school following the first successful year of a health related study at an accredited college. Scholarship monies can be awarded prior to study on an individual basis. Eligibility requirements o High school graduate prior to 2013 o 18 years of age or older o Currently enrolled in an accredited health care program or agree to enter an accredited program within one year of receiving notification of the award. Erma H. Rosen Nursing Scholarship Scholarship(s) awarded to nursing student(s) who have completed two full years and are entering the third year of an accredited nursing program. Scholarship will fund 25% of the tuition for one academic year. The scholarship recipient(s) will be reimbursed following the completion of each semester for one academic year. Eligibility requirements o High school graduate o Completed two full years and entering the third year of a nursing program o Special consideration given to students attending Presentation College, Fairmont Campus, followed by students attending other colleges who do a clinical rotation at Mayo Clinic Health System in Fairmont. AWARDING OF SCHOLARSHIPS The Mayo Clinic Health System in Fairmont Scholarship Committee reviews all scholarship applications and selects a recipient and alternate. Scholarship recipients and alternates will be notified by May 15, Should there be no qualified applicants for an award; the committees may opt to forego selection until the following year. PROCEDURE TO CLAIM SCHOLARSHIP Scholarship recipients must enter college within one year of being awarded the scholarship. Failure to do so will disqualify the recipient. After the completion of the year of health care study, the scholarship recipient must provide a copy of the grade transcript to the Mayo Clinic Health System in Fairmont Department of Development to process the scholarship award. A reminder will not be sent.
