Individual Number Card Individual Number Card Sample (Front) (Back) An IC card with your photo. It can be used as an official personal identification. Valid for 10 years (5 years for persons under the age of 20) or for the approved duration of stay for foreign nationals The IC chip contains an electronic certificate. You can use this for electronic filings, such as e-tax, etc. The IC chip does not contain a record of private personal information, such as income or health condition, etc. Your first card is issued free of charge. For Business Owners Starting in October 2015, each business corporation will be designated a 13-digit Business Corporation Number and sent notice of this number to the address in the register. The Business Corporation Number can be used freely by anyone. Starting from January 2016, business owners will need to use the Individual Number of their employees when doing social security procedures or preparing certificates of withholding tax and income. Business owners will need to prepare for the start of the system because they will need to handle the security of specified personal information properly as well as use, provide and restrict custody of Individual Numbers that contain important personal information. -P.4-

What Will Change? Starting from January 2016 (sequentially) 1. You will need to write your Individual Number in social security applications and tax declarations. Pension, medical insurance, nursing care insurance, child benefits, procedures for child/childcare assistance, procedures for persons with disabilities, welfare benefits, vaccinations, etc. *Starting from January 2017 for tax declarations 2. You will present your Individual Number Card or your Notification Card and driver s license or health insurance card for number confirmation and identity verification.
