2 A message to prospective students of doctoral program in medicine The heart of our mission at Kyoto University Graduate School of Medicine is to improve health and quality of life through deepening our understanding of human body and elucidating the underlying mechanisms of diseases. With the advancement of technology, the current medical research communities encourage interdisciplinary approach to research topics. Thus at our graduate program we are focused to prepare our young scientists not only to become a leader within the specialized field but also to enrich the training environments to develop diverse disciplinary understanding of medical sciences. Our The Graduate Courses of Integrated Research was established in 2006 by combining six medical research departments including clinical medicine, basic medicine, and social medicine. We are seeking positive and enthusiastic students who are willing to give their best on the medical research. I Major and Enrollment Capacity Medicine 141 (Including students from the special admission for applicants with working experience) To learn more about the research fields in our program, please refer to our website of Graduate School of Medicine: II Requirements Applicants must satisfy one of the following requirements below (or who will satisfy one of the following requirements by the end of March 2014). 1. Those who have graduated from a six-year program of Japanese university (limited to programs in medicine, dentistry, veterinary medicine, and pharmacy). 2. Those who have completed 18 years of formal school education outside of Japan. * 3. Those who have completed 18 years of formal school education provided by a foreign institute by taking distance/online programs while residing in Japan. *. 4.Those who have completed a undergraduate programs provided by a foreign institutes (limited to programs which is equivalent to the 18 years of formal school education in Japan) in Japan, at an institution which is designated by the Minister of Education, Culture, Sports, Science and Technology. 5. Those who have been designated by the Minister of Education, Culture, Sports, Science and Technology (Notification No. 39, Ministry of Education, 1955). 1) Those who have completed an undergraduate program in either medicine or dentistry at a university in accordance with the former University Decree (1918, Imperial Edict No. 388). 2) Those who have graduated from Defense Medical Universities in accordance with the Ministry of Defense Establishment Law (1954, Law No. 164). 3) Those who have completed a master's program or a professional graduate school program which stated in accordance with the School Education Act (1947, Law No. 26) Article 99, Item 2 / Those qualified to receive a master's degree, or those who have been enrolled for two years or more in a consistent five-year doctoral program which is not divided into two-year master's program and three-year doctoral program, have obtained more than 30 credits there and completed required research work (including persons who fall to Degree Regulations (1953, Ministry of Education Directive No. 9) Article 6, No. 1, prior to the revisions of the Degree Regulations by order of Ministry Directive (1974, Ministry of Education Directive No. 29) and who are recognized by the Graduate School of Medicine, Kyoto University as having demonstrated academic excellence which is equivalent or superior to that of those who have graduated from a veterinary medicine program at a graduate school or non-degree program, or who have graduated from a program at a graduate school or non-degree program in medicine, dentistry or pharmacy founded in order to cultivate practical clinical ability. 2

3 4) Those who have two or more subsequent years of a research experience at a university or research institution after graduated from a university (except for a veterinary medicine program at a graduate school or specialty school, or programs at a graduate school or non-degree program in medicine, dentistry or pharmacy founded in order to cultivate practical clinical ability), or after completed 16 years of education at a foreign school, and those who also have been recognized by their research achievement to have academic ability equivalent or superior to that of those who have graduated from a university in medical program, dentistry or pharmacy program founded in order to cultivate practical clinical ability (requires eligibility screening; see item V) [Applicable only to members of the special admission for applicants with working experience] 6. Those who have been enrolled for four years or longer in a six-year program (limited to programs in medicine, dentistry, veterinary medicine and pharmacy) of a university in Japan (including those the Minister of Education, Culture, Sports, Science and Technology approved as having equivalent education, in accordance with Article of the School Education Law), and who are recognized by the Graduate School of Medicine, Kyoto University as completed required academic credits with an academic excellence. (requires eligibility screening; see item V) 7. Those who demonstrated academic performance equivalent or superior to Requirement II-1. on the individual entrance examination given by the Graduate School of Medicine, Kyoto University and who will be at least 24 years of the age by March 31, (requires eligibility screening; see item V) *Those who have graduated or are expected to graduate from a university outside of Japan and who have completed or are expected to complete a program at a graduate school outside of Japan, or those who have obtained or are expected to obtain a master's degree outside of Japan must submit graduation certificates and Questionnaire for Foreign Applicants which is enclosed to the guideline booklet for preliminary eligibility screening to Academic Affairs/Student Support Office (Manager of Graduate School Academic Affairs) by 5:00 pm on Friday, August 23, 2013, before starting the application process. Note-1: Some research fields require students to have clinical experience; please contact and confirm to the research field which you are interested in. (List of Research fields) Note-2: Regarding the special admission for applicants with working experience. Graduate school of Medicine is implementing the special admission for applicants with working experience which allows students to be able to continue their research work in the program with maintaining their employment. The special admission for applicants with working experience is limited to those who are in employment and who do not have a medical license. Those who are students in universities or graduate schools cannot apply to the special admission for applicants with working experience. The examination items (number of the questions) will be the same as regular admission applicants. III Application procedures 1. Application period Between Tuesday, September 17, 9:00am to Thursday, September 19, 17:00pm, (Documents must arrive during the above period, by post or in person) Office hours: 9:00~12:00 noon, 1:00 pm ~ 5:00 pm. If sending mail, documents must arrive by 5:00 pm on September 19, However, application documents postmarked of September 17, 2013 and that were sent by registered express mail from within Japan will also be accepted even if they arrive after the deadline. 3

4 2.Submitting application documents Send by registered express mail using the envelope enclosed to the guideline booklet (Japanese only). [Regarding your desired research field] 1) Before submitting your application, please contact the professor in the research field of your first choice from the list on our website to obtain prior approval of applying. 2) You may also select a second choice of the research field. 3) At the secondary selection, you will be considered in your second choice of research field, however you must enroll to your second choice research field if you pass the secondary selection. 4) Before filling out the second choice of the research field, please contact the professor listed on the website to obtain prior approval of applying. 5) Please follow the instructions as shown in the Desired Research Field section in the Application Form for Admission. 2 Official Academic Transcript 3 Graduation Certificate (or Certificate of Expected Graduation) * 4 Exam Admission Ticket /Photograph Card These certificates must be prepared and sealed (or singed ) by the dean (department head) of the university (undergraduate) which you are currently attending or graduated from, and both should be written in English or Japanese. (original copy) Applicants who have graduated from the Faculty of Medicine, Kyoto University need not submit this item. Those who are currently attending or completed graduate school of master's program must submit an academic transcript and graduate certificate of master's program. (original copy) Applicants who are currently attending or completed the master's programs or professional degree programs of the Graduate School of Medicine, Kyoto University do not need to submit these items. Two photographs (3.5cm x 4.5cm) to be pasted in place on the exam admission ticket / photograph card enclosed to the guideline booklet. 4

5 * 5 Self-addressed Stamped Envelope for the return of the exam admission ticket * 6 Evidence of Payment for Application Fees (Pasted on the designated form) Please put your name and address in Japan on the envelope enclosed to the guideline booklet and attach a 350-yen stamp. (for Japanese domestic express mail) *Please make sure the letters are not exceeding the size of the address sticker. Application fee: 30,000 yen [Japanese government sponsored international students and those expected to complete master's or professional degree programs at Kyoto University need not pay application fee] For applicants from the affected regions under the Disaster Relief Act whose head of household has been adversely affected by the March 2011 Great East Japan Earthquake (Tohoku earthquake and tsunami), the application fee may be waived. Please ensure to submit a copy of Risai-Shomeisho (Disaster Victim Certificate). For more information, please contact Academic Affairs/Student Support office at the Graduate School of Medicine by September 6, Payment period: From Wednesday, September 11 to Thursday, September 19, 2013 [Payment method] 1) Enter the applicant s name in the appropriate sections (three sections) on the Application Fee Payment Request Form provided by the Graduate School of Medicine (enclosed to the guideline booklet) Take the form to a Japanese bank (payment through post office or Japan Post Bank is not available), and make your payment. *Please note that payment via ATM or the Internet is not accepted. 2) No transfer fee is charged if payment is made at the head office or a branch office of Sumitomo Mitsui Banking Corporation. If payment is made at any other bank, the cost of transfer shall be borne by the applicant. 3) After making payment, make sure that the bank s receipt seal is stamped on the Evidence of Payment for Application Fees and the Application Fee (and Transfer Fee) Receipt returned from the bank. Paste the Evidence of Payment for Application Fees (left portion) on the Form for affixing evidence of payment for application fees (Applications will not be accepted if the evidence does not bear the bank s receipt seal.) 4) Application examination fees will not be returned once the application form has been processed. * 7 Address label Please fill out your postal address (in Japan) to be used for receiving the examination decisions and enrollment procedures. * 8 Letter of permission for taking admission exam. Only for the special admission for applicants with working experience. Please submit the official form 8 enclosed to the guideline booklet with the signature / the seal of the head of the institute/company that you are employed. 5

The location of the examination room will be posted on the bulletin board of the Graduate school of Medicine at 8:00 am on the day of the examination. 2. Regarding taking the Foreign language examination 1) The foreign language examination shall be English only. (Except for international (exchange) students.) 2) International (exchange) students are required to take the first question in English, and they are also required to choose second question either in English (English questions to answer either in English or Japanese) or in Japanese (Japanese questions to answer either in Japanese). The choice of language at second question must be declared when you are submitting the application. No changes will be accepted at the time of the examination. 3) Dictionaries may be used. (However, these shall be limited to general linguistic-use dictionaries. Neither specialized medical dictionaries nor electronic devices possessing dictionary functions shall be allowed.) 6

V Eligibility Screening (Applicants falling under II Requirements (page1) 5-(4), 6 and 7) 1. Application deadline Applicants falling under requirements 5-(4), 6 or 7 of above are required to submit the following documents for preliminary eligibility screening to the Academic Affairs/Student Support Office (Manager of Graduate School Academic Affairs) by 5:00 pm on Friday, August 23, When mailing the documents, please write Application for Eligibility Screening to the Doctoral Program of the Graduate School of Medicine, 2014 in red on the front of the envelope, and all documents must be sent by registered express mail. Application documents postmarked on or before August 21, 2013(if mailed from within Japan), and that were sent by registered express mail will be accepted even if they arrive after the deadline. 2. Certificate of final Graduation (completion) certificate and academic transcripts (Original copy) academic records Research Activity history Please describe your research history at your affiliated institute/company. Screening Decisions Screening decisions will be mailed to Applicants by special registered mail in the middle of September, VI Admission Decisions At 11:00 am on Friday, November 29, 2013, the list of examinee numbers of successful applicants are to be posted on the bulletin board of the Graduate school of Medicine, and acceptance letters are to be sent to successful applicants. The list of successful examinees' numbers will be posted on the website of the Graduate School of Medicine. ( (Telephone inquiries about the selection decisions shall not be accepted.) Examination voucher are to be kept on file until Enrollment Procedures are complete. VII Enrollment Procedures 1. Admission fee : 282,000 yen (subject to change) 2. Tuition First semester : 267,900 yen (annual tuition: 535,800 yen) *The amounts indicated above are tentative and subject to change. *If the above amounts are revised at or after your enrollment, the new amounts will be applied at the time of payment. 3.You will be informed regarding the payment schedule of admission fee, items to submit at the beginning of the school year, information of Japan Student Services Organization scholarships and exemption of tuition, and others by postal mail at the beginning of March

Applicants with physical disabilities who require special arrangements for taking examinations should contact Academic Affairs/Student Support Office, before submitting their application. 3.
