Amtryke Therapeutic Tricycle Evaluation Site Program Guide The Evaluation Site program was created to provide children and/or adults with disabilities with places to be evaluated for an Amtryke by licensed therapists. What is an Evaluation Site? An Evaluation Site is a therapy facility (hospital, clinic, school, etc.) that agrees to receive Amtrykes and accessories donated by a local AMBUCS chapter in return for using the tryke(s) during therapy and to perform Amtryke Evaluations. Many Evaluation Sites agree to evaluate riders who are not clients. Why are Evaluation Sites important to chapters? Establishing and maintaining a relationship with a local therapists/pediatric therapy center is the most important aspect of a successful chapter Amtryke project. This is where your chapter finds riders who need Amtrykes and where you obtain evaluations from therapists. Some chapters will have therapist-members and may not need an Evaluation Site partnership. This program encourages chapters to provide established Evaluation Sites with the newest generation of Amtrykes and accessories. Previously established Evaluation Sites need this equipment to use with their clients and to more accurately evaluate children on the Amtryke. It is important that Evaluation Sites receive the newest and best equipment, as well as be educated on the full range of accessories. If your chapter has sponsored an Evaluation Site, please visit with them regularly, get updated contact info for the Resource Center, show them the newest Amtryke catalog and find out what equipment would best meet their needs. When you order the appropriate equipment for your Evaluation Site partner, you will receive a deep discount. Remember that neither you nor the Eval Site may give away the equipment purchased at this discounted rate until the tryke becomes obsolete. Please Note: Your chapter will get scorecard credit for Evaluation Site tryke purchases in exactly the same way as normal chapter tryke purchases.

Amtryke Evaluation Site Frequently Asked Questions There is a facility in our chapter area that we would like to sponsor as an Evaluation Site. What do we do? Your chapter needs to fill out the Amtryke Evaluation Site Sponsorship Form. The facility must submit a completed Amtryke Evaluation Site Application as well. The AMBUCS Resource Center will coordinate with you and the facility to determine what AmTrykes and accessories will be ordered. Remember you receive scorecards per $600 Amtryke/accessory purchase for the facility, just like any other purchase. Amtrykes and accessories purchased at Evaluation Site prices MUST be used as therapy and evaluation bikes, not for Amtryke requests. You/the facility may only give it away when the tryke becomes obsolete. My chapter wants to purchase updated equipment for one of our Evaluation sites. How do we do this? Just contact customer support at the AMBUCS Resource Center at 1-800- 838-1845 x116 and identify your chapter s established Evaluation Site that you want to provide new equipment for before you place the order. Remember Evaluation Site trykes purchased by your chapter DO count on the scorecard, just like any other. Amtrykes and accessories purchased at Evaluation Site prices MUST be used as therapy and evaluation bikes, not for Amtryke requests. You/the facility may only give it away when the tryke becomes obsolete. Can my chapter sponsor an Evaluation Site outside of our area? No. Only sponsor an evaluation site to help you identify and evaluate riders. Where do you ship equipment that we donate for an Evaluation Site? In most cases the AMBUCS Resource Center prefers to ship equipment to the facility directly. If you d like equipment shipped to your chapter for a special presentation to the facility or to assemble if for them, tell the Resource Center before you place the order.

Amtryke Evaluation Site Program Frequently Asked Questions - Facility What is an Evaluation Sites? An Evaluation Site is a pediatric therapy facility (hospital, clinic, school etc.) that receives Amtryke therapeutic tricycles and accessories through a donation by an AMBUCS chapter. The Evaluation Site agrees to use the equipment with their clients in therapy as well as identify and evaluate riders for Amtrykes of their own. Many Evaluation Sites, based on the policy of the facility, agree to evaluate other children in addition to their clients or students who are in need of an Amtryke assessment. Many do so for free as a marketing tool to introduce new clients and families to the facility! The Evaluation program encourages prescriptive intervention by therapy professionals. Responsibilities of an Amtryke Evaluation Sites: In becoming an Amtryke Evaluation Site, the following must occur in return for the free Amtryke equipment: 1. The Evaluation agrees to use the equipment with their clients in therapy if practical. 2. The Evaluation Site agrees to recommend appropriate clients to receive their own Amtrykes through the local chapter. 3. The facility agrees to use the three Amtryke required documents for each Amtryke evaluation. 4. The Evaluation Site allows National AMBUCS/AmTryke, LLC to include its name and contact information on its website (www.ambucs.org). The following are requested, but not required, of the Evaluation Site: 1. The Evaluation agrees to perform Amtryke evaluations for children who are not clients or students of the facility. 2. The facility allows other therapists access to the Amtryke equipment for the purpose of evaluating other children. Benefits of becoming an Amtryke Evaluation Site: There are many benefits to your facility once becoming an Evaluation Site! Some of these benefits include: 1. Free Amtrykes for your clients or students through the efforts of a local AMBUCS chapter. 2. As an Evaluation Site you are able to purchase additional equipment at a discount! 3. Many facilities use its Evaluation Site status as a marketing tool to introduce new families to its facilities. How to Become an Evaluation Site: Reach out to your local chapter and ask if they are looking for an Evaluation Site partner. Fill out the Evaluation Site application found on page 4 and return it to your sponsoring chapter. You will then work with the chapter to identify your equipment needs. Once this needs assessment is completed, the chapter will turn in the paperwork to the AMBUCS Resource Center and place the equipment order. Then your facility is added to the Evaluation Site list on our website.

What is AMBUCS and how does the Amtryke program work? National AMBUCS is a 501c3 charitable organization dedicated to creating mobility and independence for people with disabilities. AMBUCS is composed of almost 5,000 members in 145 chapters throughout the country. These members work in a variety of ways to achieve the mission, one of which is the Amtryke program. AMBUCS chapters fundraise to purchase Amtryke therapeutic tricycles for children in their local communities and on the Amtryke Wish List. An important part of this process is the organization s relationship with therapists and facilities like you! It is through your help that we identify children who need Amtrykes and distinguish which Amtryke model and adaptations are appropriate for each child. How do I get an Amtryke for a client or student? You do not need to be an Evaluation Site to get an Amtryke for a child. The first step is to conduct an Amtryke evaluation using the assessment form. Ask the child s parent or guardian to fill out the request and waiver form. If there is a chapter in your local area, submit these forms to them. If you don t have a chapter in your local area, fax these forms to the AMBUCS Resource Center at 336-852-6830 or mail them to PO Box 5127 High Point, NC 27262. The child s name is then placed on the Amtryke Wish List until a sponsor is found to fund the tryke. What is the Amtryke Wish List? If the child lives in an area without an AMBUCS chapter, you can recommend a child for the National Amtryke Wish List. For more information and the required documents, visit www.ambucs.org/riders/wish-list/ or by contacting the AMBUCS Resource Center. When will the Amtryke equipment be sent to our facility? When your sponsoring chapter raises the funds to purchase the equipment. Once my facility becomes an Evaluation Site, can I submit as many Amtryke requests as I want? Be sure to speak with a member of your sponsoring chapter about their fundraising capacity. AMBUCS asks that you use discretion when submitting requests. If you overwhelm your chapter it will take longer for each child to receive a tryke. Is there anything I can do to help our children get an Amtryke more quickly? The fastest way to get Amtrykes for children at your facility is to start your own chapter of AMBUCS! It is easy to establish and only takes five interested people! Once you become a chapter you will be able to fundraise and offer donors a tax-deduction for their contributions. AMBUCS would be happy to help guide you through starting your own chapter!
