general price list The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. You may keep this price list. This list is copyrighted, subject to change, and may not be reproduced in whole or part without our express permission. Prices are effective as of 10-15-2014 and subject to change without notice. Professional Services A. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $2495 Our charge includes but is not limited to staff, facilities and vehicles to respond to initial requests for service 24 hours every day, consultation with the survivors, coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased, sheltering of the deceased, procuring death certificates and permits, consultation and assistance with procuring various death benefits, accounting and billing, regulatory compliance, legal and audit fees, postage, printing and supplies, telephones, insurance, salaries and benefits for our staff, various county, state and federal taxes and fees. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains. B. EMBALMING $655 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Colorado law does require that the deceased be embalmed or refrigerated if the final disposition is not achieved within 24 hours following death. Add $130 for additional time and materials when an autopsy has been performed A coroner s protective pouch is $95. C. Transportation All transportation charges contained within this list apply to an area described as the counties of Denver, Arapahoe, Jefferson, Adams, and Douglas, or within a radius of 20 miles to our nearest facility. If we exceed this service area, in addition to the basic vehicle charge that covers 40 of the total miles traveled from our facility, we will make an additional charge of $2.00 per mile for a staffed service vehicle such as a van and $2.25 per mile for a staffed professional vehicle such as a limousine or funeral coach. If an overnight stay is required due to a round-trip exceeding 400 miles or, due to no fault of our own, our driver and vehicle cannot safely return the same day, there will be an additional charge of $250 for overtime, hotel and meal expenses, in addition to the basic mileage charges. For practical reasons and to comply with Colorado law requiring embalming or refrigeration after 24 hours from the time of death, we are able to strategically place synthetic cooling packs. While much less effective than embalming and invoking more risk of evidence of physical deterioration, this can allow for the deceased to be present for *viewing and/or services in our facility where we are best able to manage any physical issues. Usually, this will add a casket/container and other preparation of the deceased. Merchandise CASKETS $1,295 to $14,500 A complete list of caskets will be provided at the funeral home. Cremation Service Options It will be necessary to select or provide a container for any viewing and/or the cremation process as well as a container for the cremated remains. We offer a wide variety of rental and combustible containers for cremation. We also offer a wide variety of distinctive and value-priced urns. Traditional Cremation $7060 FULL SERVICE WITH CEREMONIAL CASKET, RECEPTION CENTER PACKAGE, CREMATION TO FOLLOW Includes: Basic services of funeral director and staff, transfer to mortuary from place of death, embalming, other preparation, dressing and casketing, viewing (one day), funeral ceremony, utility vehicle for death certificates and permits, ceremonial casket and alternative cremation container, Memorial Package consisting of 100 folders/prayer cards, memorial book and acknowledgments and the crematory fee. Add urn and any use of coach if service is held at church or away from mortuary. If reception center is not used this package will be reduced by $400. If selecting a casket other than stated above, deduct $995. Memorial Service/Commemorative Gathering $4635 CREMATION WITHOUT VIEWING FOLLOWED BY A MEMORIAL CEREMONY AND RECEPTION CENTER PACKAGE Includes Basic services of funeral director and staff, transfer to mortuary, memorial ceremony and Memorial Package consisting of 100 folders/prayer cards, memorial book and acknowledgments, utility vehicle for death certificates and permits and crematory fee. Add a combustible container, and the urn selected. You may also add pre-cremation identification (see below). If there is an escorted procession, add a lead vehicle (limousine). If reception center is not used this package will be reduced by $400. Non-Ceremonial Cremation/Memorial Reception $3795 NON-CEREMONIAL CREMATION WITH MEMORIAL RECEPTION This package includes: Basic services of funeral director and staff, transfer to mortuary, utility vehicle for death certificate and permits, crematory fee and a reception at one of our seven reception centers (see Reception Center and Catering Menu for complete options). Add a combustible container, selected urn and any printed materials. This package enables you to receive and host your family and friends in one of our warm and welcoming reception centers, honoring the basic human need to express and receive support from others during some of life s most difficult times. This assumes there is no formal ceremony requiring a microphone or video while using the reception center, though we understand that there may be informal toasts and family-managed public expressions of memories and sympathy. For a small additional fee, we are pleased to create a memorial video. If you would like any special music, our funeral director can play CD s provided by you, or we have a library of music, ipod, MP3 player or can download requested songs. Non-Ceremonial Cremation $2345 to $6940 CREMATION WITHOUT VIEWING OR CEREMONY This discounted package includes: Basic services of funeral director and staff, transfer to mortuary, utility vehicle for death certificate and permits and crematory fee. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made from materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are unlined, leak resistant and made of heavy cardboard ($95). NON-CEREMONIAL CREMATION WITH UNLINED ALTERNATIVE CONTAINER ($95) $2440 NON-CEREMONIAL CREMATION WITH CREMATION CASKET SELECTED BY PURCHASER $2740 to $6940 NON-CEREMONIAL CREMATION WITH CONTAINER PROVIDED BY FAMILY $2345 ADD any scheduled delivery of cremated remains to the cemetery or other facility*: (director & vehicle) $265 ADD any unscheduled delivery*: (apprentice & vehicle) $70 *If we are requested to assist in this manner, this charge will be added to any cremation plan you select. We waive this fee if we are involved in a funeral or memorial service. The Direct Cremation Options include sheltering/refrigeration for up to 4 days (other cremationists charge extra for each day of refrigeration). We also provide a windowed room that allows you to be present for the start of the cremation process. There is a charge of $90 to reserve the room and cover the additional time of our staff. ~5~

The Cremation Gardens at Rocky Mountain Memorial Parks Located adjacent to our Parker Road and County Line facilities, Rocky Mountain Memorial Parks have been referred to as an oasis of beauty and serenity, perfect for Coloradoans who prefer cremation. The environment is reminiscent of our Rocky Mountains. Large areas are set aside for the cultivation of flowering bushes, majestic tress and flowers native to our state, including the largest collection of columbines in our state. These are not-for-profit cemeteries, perpetually endowed and built with the best materials possible to endure the tests of time and nature. Scattering Ceremony at Mt. Lindo $575 This includes meeting our director on the day and time of your choice on private land above the lighted cross west of Denver. Spectacular views and vistas are seen from all points and afford a memorable opportunity to say goodbye. If you prefer that we scatter the cremated remains in this area unwitnessed and at our convenience, the charge is only $290. Add this fee to any cremation option. You may also select a memorial or inscription at either of our Rocky Mountain Memorial Parks. We can package, transport and mail cremated remains by U.S. Postal Service Express Mail (other carriers will not accept cremated remains) anywhere in the United States for $95 (add cost of urn). Express Mail is typically an overnight delivery in the continental U.S. Because we schedule our crematory days in advance, and due in part to the time involved in meeting the legal requirements of the death certificate and disposition permit, we ask for 3-4 days following death to complete the cremation process. Let us know if this creates difficulty we ll try our best to accommodate you. FINAL IDENTIFICATION OF THE DECEASED Cremation is an irreversible process. Where there is human involvement, there will always be the potential for human error. This can apply to hospital, coroner and mortuary staffs. To be safe and sure, for your peace of mind and ours, we highly recommend a positive identification of the deceased before cremation. In our memorial service and direct cremation option there is no embalming and only a limited time that the deceased may be transferred out of refrigeration and identified (refrigeration or embalming are required by state law 24 hours after death CRS 12-54-111). So that the deceased remains in an essentially refrigerated state, we respectfully request that the identification be for no more than 20 minutes from the time we agree to begin. In the interest of full disclosure while minimizing movement of the deceased, the identification occurs in the container that will shelter the deceased prior to cremation and will be later consumed in the cremation process. Identification viewings range from $0-$300. Receiving A Deceased Person From Another Mortuary $1585 MINIMUM DELIVERY TO CEMETERY This discounted package includes basic services of funeral director and staff (discounted to reflect already-secured death certificate/disposition permit and reduced involvement of other personnel), transfer to mortuary from airport or other point in the metro area, and the use of funeral coach or van to a local cemetery. This package presumes no ceremony and no other use of the funeral home and that we may deliver to the cemetery at the time of our choosing. Add to the fee of $1585 any additional services selected, such as graveside ceremony, other preparation dressing/ casketing, viewing/visitation and funeral service at chapel or church. If the forwarding arrangements to Denver are made directly by the family with a forwarding director, eliminating the need for our staff to meet and coordinate any details with the family, this discounted package will be further reduced by $175. Forwarding A Deceased Person To Another Mortuary $1965 MINIMUM TRANSFER This discounted package includes basic services of funeral director and staff, transfer to mortuary from place of death, embalming, use of funeral coach or van to local point of embarkation (airline or other shipping charges not included) and utility vehicle. Other items that may apply and could be added to the billing are a combination shipping container ($160) or air tray ($140). If the forwarding arrangements are made directly by the family with a receiving funeral director, eliminating the need for our staff to meet and coordinate any details with the family, this discounted package will be further reduced by $175. STANDARD TRANSFER VIEWING ONLY, NO FUNERAL SERVICE $2955 This discounted package includes basic services of funeral director and staff, transfer to mortuary from place of death, embalming, other preparation dressing/casketing, one day viewing/visitation, use of funeral coach or van to local point of embarkation (airline or other shipping charges not included) and utility vehicle. An air tray may be necessary ($140). We appreciate the survivor s choice to view when we provide other preparation dressing/casketing, so as to verify that our work in this regard is satisfactory and to avoid any additional charges by the receiving funeral director, eliminating the need for our staff to meet and coordinate any details with the family, this discounted package will be further reduced by $175. ~6~

Immediate Burial $2540 to $3835 Immediate burial is the process of arranging for burial of the unembalmed body as soon as possible. This service is preferred by members of the Jewish, especially Orthodox and Conservative, and Muslim faiths. We offer separate, specially configured facilities for ritual washing as dictated by the religious faiths we serve. Our staff is also fully knowledgeable in serving these faiths. Difficulties in filing a death certificate and securing the necessary legal permit immediately and/or a cemetery s inability to open a grave on short notice may require that the body be embalmed ($655) or refrigerated ($95 per day) so as to comply with State of Colorado health regulations requiring a sanitary disposition of the deceased within 24 hours (CRS 12-54-11). We include 4 days of refrigeration at no extra charge. The casket must stay closed at all times in order to comply with company policy and state law that the deceased be in a refrigerated state. The Immediate Burial options presume that we deliver the deceased to the cemetery for burial at the time of our choosing and that there is no viewing or commemorative gathering involved. A commemorative service, which increases staff time for planning and assistance, can be added see add items I. (graveside ceremony only $610) or H. (memorial ceremony only $660). Adding other service options, such as commemorative services at both the chapel/church and grave, requires full non-discounted itemization.
