To ensure that the skills and capacity of the whole practice nursing team are deployed to best effect to meet the needs of the practices patients. To provide clinical care for practice patients. Diabetes, COPD, IUD and Implanon fitting experience preferred but clinical caseload according to current skills and future ambitions, within the context of the whole nursing team. To implement new clinical services after agreement by the practice. To act as the practice lead for better working with other nursing services providing care for our patients. Leadership/Management Responsibilities: General Attend practice/primary care meetings updating the partners and Practice Manager with relevant information. Have regular meetings with the nursing team to understand any clinical or organisation problems, providing clinical input allowing the PBM to oversee any processes necessary. Training: Work with the Practice Business Manager to ensure, through the appraisal process, that all staff have a training plan and keep themselves professionally updated. Ensure that all staff have refresher training in core work activities such as health and safety, manual handling, resuscitation etc. Identify training needs of Practice Nurse Team and actively promote life-long learning, and continuous improvement Absence: Support the Practice Business Manager where necessary so that all staff who have been absent have a return to work interview and are supported appropriately when they return to work. Clinical Practice: Practice independently in accordance with UKCC code of conduct and with reference to Practice Guidelines, appropriately referring to GPs. The post holder will work as part of the clinical team; their role will depend on training and experience. Wound Care and Management: Assessing routine wounds including trauma Assessing [and prescribing] to support good wound management in complex situations (e.g. use of Doppler technique) Prevention and Health Promotion: Working with patients to develop a management plan where health problems or potential health problems are identified Identifying health trends and public health issues and working with the team and others to combat these and promote increased physical activity, healthy eating, smoking cessation, safe alcohol drinking and sexual health Risk Assessment: Recognising issues and gathering sufficient information to refer (eg drugs; domestic violence; child protection; vulnerable adults; social problems) Health Screening: Ensure the Practice delivers on its role as part of the National Screening program. Travel Health: Provision of specialist and evidence based support to the team Immunisation (Adult and Child): Working with public health initiatives ensuring the work of the team is up to date. Leading the team in achieving the immunisation guidelines and targets Minor Surgical and Nursing Procedures: Leading the development of the nurses role in the delivery of the service. Mental Health; Learning Difficulties; Physical Disability Ensuring that appropriate systems are in place for effective patient care and monitoring. Implement new clinical services and continually improve quality of existing services Implement new clinical services after agreement by the practice. Practice lead on the development of the nursing team (as the service needs change and evolve). Attend meetings and disseminating to the practice. Advance clinical nursing practice in the wider nursing team through the development of shared, evidence based practice guidelines. Lead the development and monitoring of key quality markers for clinical practice Plan, develop, implement and evaluate new health promotion and chronic disease management services. Actively participate in educational programmes for whole Primary Health Care Team, including medical students and GP Registrar, in relation to areas of clinical and practice nursing expertise. Provide professional advice to the partners on practice developments, recommending changes to working practices in line with evidence based knowledge. Record keeping: Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
