1 The Poplars Medical Centre Job Descripti Job Title: Hours of Work: Line Manager reporting to: Accountability: Advanced Nurse Practitier Negotiable N Clinical Partner The Partners JOB SUMMARY The post holder will be an experienced nurse who, acting within their professial boundaries and with the appropriate qualificati as an Advanced Nurse Practitier, will provide care for the presenting patient, from initial history taking, clinical assessment, diagnosis, treatment including prescribing and evaluati of care. The post holder will demstrate safe, clinical decisi-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demstrate critical thinking in the clinical decisi-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirement for advanced practice must be met. Key duties and respsibilities - Clinical practice 1. Assess, diagnose, plan, implement and evaluate treatment/interventis and care for patients presenting with an undifferentiated diagnosis 2. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly 3. Assess, diagnosis, plan, implement and evaluate interventis/treatments for patients with complex needs. 4. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a lg-term cditi (as appropriate) 5. Diagnose and manage both acute and chric cditis, integrating both drug- and n-drug-based treatment methods into a management plan 6. Prescribe and review medicati for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and natial and practice protocols, and within scope of practice 7. Undertake home visits for acute illness and chric disease management. 8. Work with patients in order to support compliance with and adherence to prescribed treatments Provide informati and advice prescribed or over-the-counter medicati medicati regimens, side-effects and interactis 9. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situatis, including initiati of effective emergency care 10. Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health

Take a lead role in planning and implementing changes within the area of care and respsibility 9. Work with practice management to ensure sufficient staff of appropriate ability, quality and skill-mix are available to meet current and future service delivery, that selecti and recruitment processes are effective and that equality of treatment of the team incorporates quality HR principles and processes 10. Ctribute to the development of local guidelines, protocols and standards 11. Maintain effective communicati with those respsible for the overall commissiing and procurement process 12. Maintain active involvement in the planning and processes of practice-based commissiing or similar initiatives Team working 1. Use a structured framework (e.g. root-cause analysis) to manage, review and identify learning from patient complaints, clinical incidents and near-miss events 14. Assess the impact of policy implementati care delivery 15. Mitor and develop the performance of the practice nursing team. 16. Understand and apply legal issues that support the identificati of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance. 17. Ensure the whole team have skills and knowledge regarding domestic violence, vulnerable adults, substance abuse and addictive behaviour. Provide guidance and support to ensure appropriate referral if required 18. Effectively manage own time, workload, and resources. 19. Health & Safety The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Practice Health & Safety Policy, to include: 1. Using persal security systems within the workplace according to Practice guidelines 2. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks by cforming to health, safety and security legislati, policies, procedures and guidelines 3. Making effective use of training to update knowledge and skills 4. Using appropriate infecti ctrol procedures, maintaining work areas in a tidy and safe way and free from hazards within the practice according to local and natial guidelines 5. Reporting potential risks identified. 6. Assist in maintaining a safe working envirment for self, patients, carers and the multidisciplinary team. 7. Alert other members of the practice team to issues of quality and risk identified within in work with patients. 8. Ensure appropriate supervisi of safe storage, rotati and disposal of vaccines and drugs and checking all medical equipment. 9. Oversee the mitoring, stock ctrol and documentati of ctrolled drug usage according to legal requirements where appropriate 10.
