Telecommunications Installation Materials & Products SCOPE OF WORK I. Order Placement Responsibilities A. The authorized agent to purchase will provide the vendor s inside sales account support representative, via phone, Email, written, Fax or online order process, with purchase order requests containing appropriate information to complete any/all communications products procurement. Order(s) will include (1) a List of materials by part number and quantities required ie; Systimax 2071 White, Comcode 7000214208, Box s 1000 ea, 20 Box s =20,000 feet (2) Specific Purchase order number which must be associated with all invoicing of procured items,(3) anticipated arrival Date, (4) anticipated delivery date to, and/or pick up date by purchasing entity. II. Order Shipping A. All orders must be shipped and delivered within the specified time period required in the Phase I Product Description Sheet. B. Procured products may be picked up at the distributor s warehouse by purchasing entity or the State s authorized contractor with prior approval and scheduling. C. Orders of 5 identical items or less, per day, labeled on the Phase I Product Description Sheet as 1-5 Stock-, will be on the shelf and available for immediate pickup. Depleted stock will be replenished within 24 hours. D. Orders for items labeled on the Phase I Product Description Sheet as stock to 24 must be available upon order or within 24 hours of order. E. Orders for items labeled on the Phase I Product Description Sheet as 24-48
must be available upon order within 48 hours. F. Orders for items labeled on the Phase I Product Description Sheet as 48 72 must be available upon order within 72 hours. G. Orders for items labeled on the Phase I Product Description Sheet as 5 10 days must be available upon order within 10 days. H. All items not labeled on the Phase I Product Description Sheet by the Division of Purchasing must be filled in by the bidder in the column labeled Stock Rqurd with the availability time of the listed item. Product Required A. All specifications related to the products specified in this ITB are the sole ownership of the various manufacturer s patents or rights. B. This ITB requires the Bidder s respond with Part Numbers for each of the specified and listed products in the Phase I Product Description Sheet. No substitutions or equal products will be acceptable. Any substitutions to the specific product and part number specified in the Phase I Product Description Sheet document will be considered noncompliant and will receive no further evaluation or consideration in award. PRICE REDUCTIONS It is understood and agreed that in the event of a reduction and/or allowable discounts in price, the State of Utah will immediately and automatically be given the full benefit of such price decreases and/or discounts. The State will Benchmark these contracts with all available sources to verify that the State is receiving competitive discount pricing as compared to the benchmark sources.
PRICING All pricing must be based upon the manufacturer s most current available price list for communications products specified. Bidders are required to provide complete pricing with individual pricing as specified. Bidders are required to provide a percent off list discount as well. The percent off list discount provided will apply to all items not specifically called out on the Price Sheet. The individual pricing will be used to determine the compliant low bidder. Bidders who do not complete each item of the individual pricing will be considered noncompliant and will receive no further consideration of award. The successful bidder(s) will be required to supply a copy of the price list to all state agencies and political subdivisions upon request. V. Account Representative A. The successful bidder awarded a contract resulting from the award of this ITB will be required to maintain an Outside and Inside sales account representative assigned to the State of Utah account. The inside sales account representative will be the primary point of contact for all sales associated with the contract.
