1.1 Project Summary 1.0 General Information McCabe Engineering & Contracting (McCabe), on behalf of the City of Mansfield (Owner), has prepared the following bid specifications for asbestos abatement at the Mansfield Foundry located at 500 North Main Street in Mansfield, Ohio (Site). The company that is successfully awarded the contract by the Owner will be responsible for implementing these asbestos abatement bid specifications and will be known throughout the remainder of the document (i.e. specifications) as the Contractor. The Owner is actively seeking proposals for a Contractor that will abate the asbestos containing materials (ACM) identified within this document at the Site. The work includes, but is not limited to, removal and proper disposal of ACM at the Site. The Contractor will be responsible for all work that is required to complete the work to the satisfaction of the Owner. The Contractor is to follow regulations referred to in Section 1.3 and receive approval from the Owner for any deviation from these specifications. The Contractor will be responsible for furnishing all labor, equipment, personnel protective equipment, air testing, air monitoring, and materials required for conducting the work described in these specifications. Arranging for electricity, water, or other utilities will be the responsibility of the Contractor, and costs for such should be reflected in bids submitted to the Owner. The Contractor will be responsible for securing the work areas at the Site and posting all notices. Posting notices shall also be performed in a manner compliant with all applicable federal, state, and local regulations. Please refer to Section 2 for details on the asbestos abatement. Please refer to Section 3 for details on inspections, air monitoring, and completion. Please refer to Section 4 for bid information. 1.2 Site Information and History The Site is approximately 18.5 acres in size that ceased operations in 2002 and consists of 22 buildings with the majority of the buildings sharing a common wall. The City of Mansfield purchased the Site in 2003 from the Mansfield Foundry Corporation/Citation Corporation. All the buildings at the Site are currently not in use except for Building 61, which is being used for storage. Please see Figure 1 for the Site Location Map. In November 2003, McCabe conducted an ACM Survey that concluded the presence of ACM at the Site. The following building materials were identified during the ACM Survey as containing asbestos: black mastic, floor tile, and pipe insulation, ceiling panels, and siding. The exact location and quantity of the ACM is discussed in Section 2.3. Refer to Exhibit 1 for ACM Survey. Please note that some of the building numbers referenced in the ACM Survey where the ACM is located differs from the building numbers noted in Section 2.3. updated after the ACM Survey was completed. Please refer to Section 2.3 for the correct building numbers and ACM locations. 1.3 Regulations The Contractor will be responsible for adhering to all federal, state, and local laws and regulations. Below is a list of applicable regulations and standards, which include, but not limited to, the following: 1. U.S. National Archives and Records Administration (NARA) a. 40 CFR 61- Subpart M; National Emission Standard for Asbestos 2. U.S. Department of Labor, Occupational Safety & Health Administration (OSHA) a. 29 CFR part 1910, subpart D: Occupational Exposure to Asbestos; Final Rule 3. U.S. Department of Labor, OSHA a. 29 CFR part 1926 1101: Occupational Exposure to Asbestos; Construction Industry Standard 4. U.S. E.P.A. a. 40 CFR part 61, subpart A: Regulation for Asbestos 5. OAC 3745-20: Asbestos Emission Control 1.4 Supervision The Contractor shall provide a Supervisor that is qualified, certified, and capable of overseeing the asbestos abatement activities. The Supervisor must be capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures. The Supervisor shall also be at the Site whenever work activities are being performed. The Contractor shall be responsible for all precautions as may be necessary to fully protect his work both during its execution and until its final acceptance or the Contractor shall be held responsible for all damages incurred. The Contractor shall be responsible to the Owner for the acts and omissions of his employees, subcontractors and their agents and employees, and other persons performing any of the work under a contract with the Contractor. The Owner or Owner s representatives shall have access to the Site at all times and the Contractor shall be available to assist, if necessary, the Owner or Owner s representative during any visits. 1.5 Permits, Fees, and Notices The Contractor shall secure and pay for all permits, governmental fees including Ohio EPA removal fees, licenses, and inspections necessary for the proper execution of the contract. Permits, fees, and notices include, but are not limited to, asbestos abatement notifications. The Contractor shall give all notices and comply with all laws, ordinances, rules, regulations, and lawful orders of any public authority bearing on the performance of the work. If the Contractor performs any work knowing it to be contrary to such laws, ordinances, rules, and regulations, and without such notice to the Owner Owner s representative, he shall assume full responsibility therefore and shall bear all cost attributable thereto. 1.6 General Work Practices The Contractor, at all times, shall keep the premises free from accumulation of waste materials or rubbish caused by his operations. At the completion of the work, he shall remove all his waste materials and rubbish from and about the Site as well as all his tools and equipment. The Contractor will prevent the spread of dust to neighboring sites and properties and avoid the creation of a nuisance or hazard in the surrounding area. The Contractor shall not perform any work that is not included specifically in this document without submitting a change order in writing describing the additional work and cost to the Owner or Owner s representative. The Contractor will not perform any work described in a change order without receiving prior written approval from the Owner or Owner s representative. The Contractor will dispose of all ACM waste removed during the project at an approved licensed disposal facility that accepts friable and nonfriable ACM waste. Prior to disposal of any ACM waste, the Contractor will notify the Owner or Owner s representative of the licensed disposal facility and receive approval from the Owner or Owner s representative. 1.7 Schedule The Contractor shall submit a schedule of all services to be performed with the bid. The schedule shall include work durations for all services and subtasks. Services and subtasks may be performed concurrently. Additionally, the schedule will include a detailed description of methods and equipment to be used for each operation and of the sequence of operations. The schedule shall be related to the entire project to the extent required by the specifications and shall provide for expeditious and practicable execution of the work. The Contractor shall maintain at the Site, for the Owner or Owner s representative, one record copy of specifications, change orders, and other modifications, in good order and marked currently to record all changes made during work activities. The Contractor shall not be relieved of responsibility for any deviation from the requirements of the specifications unless the Contractor has specifically informed the Owner or Owner s representative in writing of such deviation at the time of submission and the Owner or Owner s representative has given written approval to the specific deviation. The Contractor shall not be relieved from responsibility for errors or omissions. 1.8 Mandatory Site Visit & Question/Answer Period A mandatory pre-bid Site walk will be held on March 25, 2014 between 9:00 am and 12:00 pm. Bidders not attending the pre-bid Site walk will be excluded from evaluation. Bidders will be responsible for confirming estimated quantities provided in these specifications prior to submitting bids. Bidders, if applicable, will be responsible for submitting any questions to the Owner in writing by March 28, 2014. The Owner will respond to all questions by all potential bidders by April 1, 2014. All Bidders are responsible for reviewing these specifications along with all Exhibits thoroughly and submitting any questions regarding these specifications or work activities described below to the Owner during the question/answer period discussed above. The Contractor that is awarded the contract will be expected to understand the work scope described in these specifications completely. 1.9 Owner s Right to Stop the Work If the Contractor fails to correct defective work as required or persistently fails to carry out the work in accordance with the contract documents, the Owner or Owner s representative, may order the Contractor to stop the work, or any portion thereof, until the cause for such order has been eliminated. 1.10 Owner s Right to Carry out the Work If the Contractor defaults or neglects to carry out the work in accordance with the contract documents and fails, within seven days after receipt of notice from the Owner or Owner s representative, to commence and continue correction of such default or neglect with diligence and promptness, the Owner may, after seven days following receipt by the Contractor of an additional written notice and without prejudice to any other remedy he may have, make good such deficiencies. In such case, an appropriate change order shall be issued, deducting from the payments then or thereafter due the Contractor the cost of correcting such deficiencies, including compensation for additional services made necessary by such default, neglect, or failure. If the payments then or thereafter due the Contractor are not sufficient to cover such amount, the Contractor shall pay the difference to the Owner. 1.11 Review of Contract Documents/Specifications The Contractor shall carefully study and compare the contract documents and shall at once report to the Owner any error, inconsistency, or omission he may discover. liable to the Owner for any damage resulting from any such errors, inconsistencies, or omissions in the contract documents. The Contractor shall perform no portion of the work, at any time, without contract documents for such portion of the work. 1.12 Daily Logs The Contractor will be responsible for completing daily work and activity logs that include the following: 1. The name of each person, and description of the type of respiratory protection worn by each person entering containment or work area; 2. Descriptions of meetings or discussions regarding the job, special or unusual events, records of daily containment inspections, records of waste removal from containment, the chart from the recording manometer, and air monitoring results; and 3. A copy of this daily work log, signed by the Contractor must be submitted to the Owner or Owner s representative no later than the end of the following working day. 1.13 Submittals The Contractor shall submit the following to the Owner prior to commencing work: 1. The resumes of the person(s) who are employed by the Contractor and are licensed as asbestos abatement contractors in the State of Ohio where work will take place, and all accredited Supervisors that will be used in the asbestos abatement project. 2. A copy of the training certificates for all personnel to be employed on the asbestos abatement project. 3. A copy of Contractor's Respiratory Protection Program that includes: a. A statement of Respiratory Protection Training; b. Current copies of qualitative and/or quantitative respirator fit test results for all employees to be used on the asbestos abatement project. Respirator fit tests must be performed using respirator brands worn by Contractor's employees; and c. A copy of the current medical certificates for all employees to be used on the asbestos abatement project. The certificates must have the name of the medical center, name of attending physician, and signature of attending physician, and some indication that the individual is physically capable of performing asbestos abatement work. 4. 2.1 General Preparation 2.0 Asbestos Abatement Proper preparation of the work area prior to asbestos abatement is crucial to the success of asbestos abatement projects. The following will be performed, but not limited to, by the Contractor in preparation of the work area: 1. Critical Barriers: All asbestos abatement work involving friable ACM and nonfriable ACM shall require the installation of critical barriers at all penetrations to the work area. 2. Pre-cleaning: Debris from work areas may need to be removed prior to ACM abatement activities can commence. Debris will not need to be disposed of by the Contractor, only relocated to access the ACM. 3. Polyethylene Sheeting: In general, all fixed objects and architectural surfaces in the work area must be protected from contamination during asbestos removal. 4. Pressure Differential: All work areas must be placed under a pressure differential of at least minus 0.02 inches of water column, with respect to outside areas, prior to disturbance of ACM. In instances where mastic materials are the only type of ACM disturbed in the work area, a pressure differential within the work area will not be necessary. Instead, the intake of a HEPA-filtered exhaust unit or HEPA-filtered vacuum may be placed close to the area where asbestos is being disturbed. 5. There is no power at the Site; therefore, the Contractor will need to provide temporary power and lighting and ensure safe installation of temporary power sources and equipment per applicable electric code requirements. There is no water at the Site; therefore, the Contractor will need to bring water to the Site. 2.2 Preparation for Friable or Nonfriable ACM The following will be performed, but not limited to, by the Contractor in preparation of abating friable or nonfriable ACM: 1. Post warning signs and barrier tape in and around work area as required by all applicable regulatory agencies. 2. Seal off all openings, including but not limited to: corridors, doorways, elevators, skylights, ducts, grills, diffusers, and any other penetrations of work areas 3. Doorways and corridors that will not be used for passage during work must be sealed with critical barriers. These seals must be left in place until final air testing is complete and the results received and approved. Allowances must be made for emergency exits. 4. If appropriate, cover surfaces in the proposed work area, which do not require asbestos removal, with polyethylene sheeting in the following manner: a. Cover walls or erect temporary walls with 2 layers of (4 mil minimum thickness) polyethylene sheeting sealed with tape. This sheeting must be secured by staples and tension nails as necessary to maintain the integrity of containment throughout removal and testing process. The two layers of polyethylene sheeting must be placed so the upper layer can be removed without damaging the integrity of the lower layer. b. Cover floors with 2 layers of (6 mil minimum thickness) polyethylene sheeting sealed with tape, when appropriate. The floor covering must extend at least 12" up the wall to contain leakage. c. Cover ceilings with 2 layers of (4 mil minimum thickness) polyethylene sheeting, sealed with tape, when appropriate. This sheeting must be secured in a manner that shall maintain the integrity of containment throughout removal and testing process. d. For work areas that do not have an adequately flat surface, or have extensive mechanical and/or electrical fixtures attached to the ceiling. The Contractor shall perform initial cleaning of the exposed surfaces at the ceiling prior to removal, and perform wet-cleaning and HEPA-vacuuming during final cleanup. The ceiling area shall be encapsulated prior to final air testing. 2.3 ACM Types, Quantities & Locations The following is a summary of the ACM types, quantities, and locations described in the ACM survey that requires removal by the Contractor: 1. Black mastic under brown floor tile, approximately 450 square feet in Building 61; 2. Black mastic under brown floor tile, approximately 1,440 square feet in Building 39A; 3. 2 Pipe insulation, approximately 40 linear feet in Building 33A; 4. 4 Pipe insulation, approximately 100 linear feet in Building 33; 5. 9 x 9 Green floor tile, approximately 240 square feet in Building 38; 6. Black mastic under brown 12 x 12 floor tile, approximately 1,600 square feet in Building 38; 7. Transite ceiling, approximately 500 square feet in Building 33B1; 8. Pipe insulation, approximately 30 linear feet in Building 33B1; and 9. Transite siding, approximately 14,560 square feet on Building 58, 35A, 33A, 33B, and 33B1. a. Transite siding is located along the exterior and interior walls depending on the building. Please refer to Figure 2 for a Location of Asbestos Map. The condition of the ACM referenced above is likely to be in worse condition since the ACM Survey due to weather and vandalism. Therefore, ACM may no longer be in its original position and now located on the floor beneath its original position. 2.4 ACM Removal Process 2.4.1 Asbestos Removal, Friable Materials The Contractor will perform, but is not limited to, the following procedures when removing friable ACM: 1. Prepare Site per Section 2.1 and 2.2. 2. Spray ACM with amended water using spray equipment capable of providing a mist application to reduce the release of fibers. Saturate friable ACM sufficiently to wet the substrate without causing excessive wetting, dripping, or delamination of the material. 3. Spray the asbestos ACM repeatedly during removal process to maintain wet condition and minimize asbestos fiber dispersion. The spraying must not be used as a technique to remove or dislodge ACM. 4. Remove saturated ACM in small sections. transport. Material must be placed in containers in a prompt manner consistent with federal, state, and local regulations. 5. Waste Load-out Procedure a. Seal bags or containers. Clean external surfaces of bags or containers thoroughly by wet cleaning in the work area. b. Move bags or containers to washroom, wet-clean each container thoroughly, and move to clean room area pending removal to uncontaminated areas. The material must be placed in a clean bag or container as it exits the washroom and enters clean room area. c. Ensure that containers are removed from clean room areas by workers who have entered from uncontaminated areas, dressed in clean coveralls. Ensure that workers do not enter from uncontaminated areas into washroom or work area. d. When disposal bags are used, the bagged material must be placed within a second bag in equipment decontamination unit. The second, outer bag must be labeled with all applicable warnings and D.O.T. labeling. 6. Secondary Removal a. After completion of gross removal work, all surfaces from which asbestos has been removed must be wet-brushed with a wire brush and/or wet-cleaned by an equivalent method to remove all visible material. During this work the surfaces being cleaned must be kept wet. 2.4.2 Asbestos Removal, Nonfriable Materials The Contractor will perform, but is not limited to, the following procedures when removing nonfriable ACM: 1. Prepare site as per section 2.1 and 2.2. 2. Wet nonfriable ACM with amended water and remove with appropriate equipment. 3. Spray the ACM repeatedly during removal process to maintain wet condition and minimize asbestos fiber dispersion. The spraying must not be used as a technique to remove or dislodge ACM. 4. Remove material in small sections. As it is removed place the material in sealable 6 mil polyethylene bags and place in appropriately labeled container for transport. Material must be placed in containers in a prompt manner consistent with federal, state, and local regulations. 5. Waste Load-out Procedure a. Refer to Section 2.4.1.5. 6. Secondary Removal a. Refer to Section 2.4.1.6. 2.4.3 Asbestos Removal, Mastic The Contractor will perform, but is not limited to, the following procedures when removing mastic: 1. Apply solvent to remaining mastic and allow an appropriate period of time for the solvent to dissolve the mastic. 2. Remove mastic floor tile material in small sections. for transport. Material must be placed in containers in a prompt manner consistent with federal, state, and local regulations. 3. Refer to Section 2.4.1.5. 2.4.4 Asbestos Removal, Exterior Transite Siding The Contractor will perform, but is not limited to, the following procedures when removing transite siding: 1. Provide suitable tools for removal of transite siding. 2. For transite siding, spray panels with amended water using spray equipment capable of providing a mist application to reduce the release of fibers. Saturate the transite siding sufficiently to wet the material without causing excess dripping. 3. Remove wet transite siding in small sections. As the transite siding is removed wrap the material in 6 mil polyethylene sheeting and place in appropriately labeled containers lined with 6 mil polyethylene sheeting for transport. 4. Transite siding must be removed carefully and in complete sections. Breakage of the panels must be minimized, and must not be used as a method of removal without written approval from the Owner or Owner s representative. 2.5 Preparation for Glove Bag Procedure 1. Post warning signs and barrier tape in and around work area as required by all applicable regulatory agencies. 2. Seal vents within work area with, at least, tape and polyethylene sheeting during the work. 3. Cover areas beneath and adjacent to the proposed work using 6 mil polyethylene sheeting, as appropriate. Cover scaffolding with at least one layer of 6 mil polyethylene sheeting, when appropriate. 4. Prepare curtained doorways at entrances to and exits from work area. 2.5.1 Asbestos Removal with Glove Bag The Contractor will perform, but is not limited to, the following procedures when removing ACM with a glove bag: 1. Install glove bag according to manufacturer s recommendations. 2. Cut covering on insulation along the top seam to allow wetting of the insulation, and cut cover all around section to be removed. 3. Remove ACM in small sections. Lower the insulation carefully in the bottom of the glove bag. Do not drop material. One glove bag must be used for each section of ACM to be removed. Sliding or re-use of a single glove bag is strictly prohibited. Use appropriate size bag for the dimensions of the material to be removed to ensure economy of materials. 4. Prior to removal of the glove bag, ensure that all surfaces from which asbestos has been removed are clean of all visible material, and that the upper portion of the bag is clean of all visible waste. Spray all surfaces and tools in the glove bag with amended water. Wipe all sections of pipe with rag or appropriate material. Wipe upper section of bag as well. 5. Use appropriate encapsulant on all surfaces inside the bag. 6. Place tools inside sleeves of glove bag and isolate from interior of glove bag. Collapse bag using HEPA-filtered vacuum. from upper portion of bag. Seal bag with duct tape or locking ties. Vacuum the unsealed upper portion. Keep HEPA-filtered vacuum connected until the glove bag is removed. Cut the glove bag along the top and sides, and then remove from pipe. Cut off isolated sleeves containing any tools or supplies from bag and place in bucket of water. Clean the tools in equipment room of decontamination unit. 7. Disposal of glove bag, material, and waste water (Refer to Section 2.4.1.5.) 3.1 Inspections Prior to and During Work 3.0 Inspections, Management, & Monitoring Contractor shall make all work areas available to inspection at all times. Each work area will be inspected by the Owner or Owner s representative accompanied by Contractor: 1. Immediately after initial cleaning has been completed and prior to the application of polyethylene sheeting to exposed surfaces. 2. Immediately prior to the commencement of removal of ACM (after preparation of work area is complete). 3. After removal is complete but prior to the application of any encapsulant to the exposed substrates, and pre-encapsulation air testing. 4. Regular inspections of the HEPA-filtered ventilation system will be performed by the Owner or Owner s representative to ensure filters are not excessively loaded with particulate debris, and are properly seated in HEPA-filtered exhaust units. 3.2 Final Visual Inspections If Owner or Owner s representative finds visible accumulations of asbestos debris in work area after the completion work, the Contractor shall repeat wet-cleaning until work area is in compliance, at Contractor's expense. When an inspection by the Owner or Owner s representative in the presence of Contractor determines that the area is free of accumulations of dust and visible asbestos debris and the final air clearance has been met, decontamination unit shall be removed, the area thoroughly wetcleaned, and materials from equipment room and shower room disposed of as contaminated waste. A final visual inspection will be made after all Contractors materials have been removed from work area and all removal, encapsulation, disposal, and related work is completed. Work area must be well lighted for inspection by Owner or Owner s representative. Insufficient lighting may result in delay of the final visual inspection. All polyethylene sheeting must be removed from work area, with the exception of critical barriers, and decontamination unit. HEPA-filtered exhaust units must remain operational, and pressure differential maintained until final clearance by Transmission Electron Microscopy (TEM) or phase contrast microscopy (PCM) is obtained. 3.3 Air Monitoring 3.3.1 General The Contractor is responsible for all air sampling required by federal, state, and local agencies. accredited laboratory using NIOSH method 7400 using PCM. In some cases, PCM may be used for final air testing. Final air samples will be analyzed by a laboratory accredited by NVLAP for TEM, using the AHERA Mandatory Transmission Electron Microscopy Method in Appendix A of 40 CFR 763, subpart E. 3.3.2 Background Air Testing Background air testing will be carried out by the Contractor prior to initiation of work in order to establish background levels of contamination. If air monitoring, during work by the Contractor, shows an increase in airborne fiber concentrations outside containment system, work shall cease until the source of the contamination is found and remedied to the Owner s or Owner s representative satisfaction. Any areas that have been contaminated as a result of Contractor's work shall be cleaned by Contractor at the expensive of the Contractor. Background air samples will be analyzed by PCM. TEM analysis of questionable samples will be made available at the expense of Contractor should he request it. 3.3.3 Personal Air Sampling Personal air monitoring will be performed by the Contractor and consist of: 1. An 8 hour time weighted average (TWA) for samples collected on 25% of the work force during each 8 hour shift for the duration of the project. 2. Continuous personal monitoring to be conducted during preparation, removal, and final cleanup, unless Type C pressure demand respiratory protection is used. 3. Excursion limit or short term exposure limit (STEL) sampling shall be performed during all phases of the asbestos abatement project to establish the STEL for each job function. 3.3.4 Conditions for Final Air Testing Final air testing shall take place when removal is complete, the polyethylene sheeting not necessary to the integrity of containment removed, and a visual inspection of work area shows that work area is clean and dry. 3.3.5 Air Clearance Criteria All clearance samples must pass the criteria set forth by applicable standards. 3.3.6 Final Air Testing After work area has met the 0.01 fibers/cc standard for the pre-encapsulation test, final air testing will be conducted and analyzed by TEM, when the amount of ACM removed in work area is greater than 160 square feet or 260 linear feet. Final air testing will consist of five TEM samples inside work area and five TEM samples outside work area. The sampling procedures and guidelines in EPA 40 CFR 763 part III will be followed. When the amount of ACM removed in work area is less than 160 square feet or 260 linear feet, the results of the pre-encapsulation PCM air test will be considered as the criteria for Contractor compliance. 3.3.7 Final Air Testing: Exterior Areas Final air testing will not be required for exterior, open work areas. Instead, a thorough and meticulous inspection will be performed by the Contractor with the Owner or Owner s representative present to determine Contractor compliance. 3.3.8 Final Air Testing: Glove Bag Procedure Each work area in which glove bag removal has occurred will be visually inspected by the Owner or Owner s representative prior to final air testing. Aggressive sampling procedures will not be used unless work areas are fully contained by critical barriers. Each work area will be tested and analyzed by the PCM method, using static sampling procedures. A TEM final air test of the general areas of glove bag removal may be performed at Owner s or Owner s representative discretion upon failure of a PCM final. 3.3.9 Failure of Final Air Tests When the results of the final air test show values of airborne asbestos in excess of the final air standard, the Contractor must re-clean work area. The final air testing procedure shall then be repeated at Contractor's expense. 3.4 Completion Upon completion of the asbestos abatement project, the Contractor must submit the following to the Owner or Owner s representative: a. All disposal receipts for asbestos-containing materials acquired during the asbestos abatement project; b. A copy of all daily logs, sign-in sheets, and daily reports; c. The original daily chart recordings from manometers used during the asbestos abatement project; d. A list of the names and signatures of all persons involved in the asbestos removal. Include state of accreditation and accreditation number for each person; e. A certification of compliance statement that all asbestos-containing materials related to these specifications have been removed and disposed of in accordance with all applicable federal, state, and local regulations for asbestos abatement; f. All analytical and air monitoring results; and g. All submittals to the Owner or Owner s representative are expected to be neat, complete, accurate, and readable. After final inspections and final air testing are complete and the results known, the Owner or Owner s representative will advise Contractor of any additional requirements. When a work area fails either the inspection or the final air testing, the area must be re-cleaned, re-inspected and re-tested. The sequence of re-cleaning and re-testing shall continue until the area passes the inspection and the final air test. 4.0 Bid Information This project is being funded under a State of Ohio Clean Ohio Assistance Fund grant, and prevailing wage is not applicable to the distribution of funds under the City s Agreement with the State. The anticipated start date for the project is April 28, 2014 with a project completion date of May 30, 2014. One original and one copy of the Contractor s sealed bid will be due to the City of Mansfield on April 4, 2014. A bid bond in the amount of 10% of the total estimated project cost will be required of the Contractor. The Contractor shall provide copies of their current certificate of insurance and current worker s compensation certificate with bids. Bids must be completed on the bid form provided in Exhibit 2 and become the property of the Owner once submitted, along with all other supporting documents required of the bidders. The Owner reserves the right to accept/reject any and all bids. All bids are to be submitted by 12:00 pm on April 4, 2014, in which all the bids will be opened and a Contractor will be selected. Introduction McCabe Engineering & Contracting was contracted by the City of Mansfield to perform a predemolition/renovation asbestos survey of the Former Mansfield Foundry Property located at 500 North Main Street, Mansfield, Richland County, Ohio. Please see Figure 1 for site location. The survey was conducted in order to identify and quantify the potential Asbestos Containing Materials (ACM) at the facility. Mr. Jim Smart conducted an initial reconnaissance of the site in August 2003. Samples were obtained from readily observable suspected ACM and transported to EA Group, Mentor, Ohio for analysis. The results of the preliminary sampling effort have been included in this report. Mr. Jim Smart (Certified Asbestos Hazard Evaluation Specialist, # 32566) and Edward M. McCabe of McCabe conducted the asbestos survey on September 10, 2003. The survey was conducted using non-destructive methods of investigation. Bulk samples were taken of all accessible suspect-building materials and sent to Environmental Hazard Services, Richmond, Virginia, for analysis. A total of fifty-three (53) samples were taken during the course of the main site inspection and initial site reconnaissance. Of the samples analyzed, nine (9) samples were identified as containing asbestos above the regulatory limit of 1%. Two samples, B-61-1 Mastic and 02-1 Mastic, contained 2% Chrysotile and were subsequently point counted to more accurately identify the percentage of asbestos in each. The point count results returned asbestos levels at 1.00% for B61-1 Mastic and 7.25% for 02-1 Mastic. Samples containing more than 1% ACM are identified as: B61-1 Mastic Black adhesive mastic (under and adhered to floor tile) contained 2% Chrysotile, approximately 450 square feet present in Building 61. Due to the asbestos content being close to the regulatory limit of 1%, a point count was performed on this sample to verify the asbestos content of the sample. Point count results returned asbestos levels of exactly 1%, verifying the material to be ACM. B39-A2 Mastic Black adhesive mastic (under and adhered to floor tile) contained 5% Chrysotile, approximately 1,440 square feet is present in Building 39. Siding 1 (also sample #5 from preliminary reconnaissance) Exterior transite siding contained 20% Chrysotile, approximately 14,560 square feet of transite siding is present on the exterior of the facility. The transite starts at the northwest corner of Building 38 between 10 and 12 feet off the ground and extends south then east around the main structure and then to the north encompassing Buildings 35A, 33A, 33B, and 65. B33-1 Pipe insulation contained 35% Chrysotile, approximately 100 linear feet of 4-inch piping is present in Building 33. B33-2 Pipe insulation contained 30% Chrysotile, approximately 40 linear feet of 2-inch piping is present in Building 33. 02-1 Black adhesive mastic (under and adhered to floor tile) contained 2% Chrysotile, approximately 1,600 square feet is present in a second floor office located in the central portion of the main foundry structure. Due to the asbestos content being close to the regulatory limit of 1%, a point count was performed on this sample to verify the asbestos content of the sample. Point count results returned asbestos levels of 7%, well above the regulatory limit of 1%. 1

02-2 Green 9 x9 vinyl floor tile contained 5% Chrysotile, approximately 240 square feet is present in the second floor office of the main foundry structure. B65-1 Transite switchgear ceiling panels contained 40% Amosite, approximately 500 square feet is present in the sub-basement of building 68. The material is in very poor condition. #8 Pipe insulation contained 85% Chrysotile, approximately 30 linear feet was observed in Building 65 during an initial site walk conducted in August 2003. All sampling locations are exhibited on the site plan provided as Figure 2. The laboratory data from the subsequent analysis of all samples is provided as Table 1. The hard copy laboratory analytical reports are included as Attachment 1. Site photographs are provided in Attachment 2. Recommendations The sample methodology presumed a homogeneous material. That is to say it was presumed for this sampling event and subsequent report that all similar ceiling tile, siding, and vinyl floor tile (VFT) was the same. Four samples of VFT and mastic returned results above 1% rendering the material ACM. Two of the samples, B61-1 Mastic and 02-1 Mastic, were point counted to verify the asbestos content of each due to the original analytical results returning near 1%. Point count results retuned at 1% and 7% respectively, verifying both to be considered ACM. VFT and mastics, which adhere to their substrates, are listed as Category 1 non-friable materials. Category 1 non-friable materials do not need to be removed prior to demolition if the work area is kept adequately wet during the demolition activities. This would include the four samples of the floor tile and mastic that were identified as ACM. The sample of the transite siding, Siding-1, returned results of 25% Chrysotile, rendering the material ACM. Transite is considered Category 2 non-friable asbestos and would have to be removed prior to demolition activities or activities that may result in the material becoming friable. The piping insulation, samples B33-1, B33-2, and #8 (located in Building 65), present within the facility is considered friable asbestos. The material is listed in poor condition and no longer in use, therefore, should be scheduled for removal as time and funds are available. As renovations and/or demolition occur within the buildings, the probability of those materials to become more damaged is likely. Further, the switchgear ceiling tile located in the sub-basement of Building 65, sample B65-1, contained 40% Amosite and is also considered friable and listed in poor condition. The material should be scheduled for removal with the piping insulation. Now that ACM has been identified in the facility, if the buildings remain and re-development occurs and the asbestos is not removed, according to 29 CFR 1926.1101, a building owner has certain responsibilities to the employees that would perform work related to asbestos containing materials (Class III and IV work defined in Paragraph B, Definitions: and Paragraph K Section 9, Information and Training) and those employees which work adjacent to asbestos containing materials. As outlined in the above referenced standard, certain job tasks, which occur in the identified facility, need to be performed by individuals that have received a certain level of competent training (labeling of materials defined in Paragraph K, Communications of Hazards, Sections 7 and 8). 2

Further, the building owner/employer needs to identify/label the location of ACM (friable and non-friable) in the facilities in the form of a written or personal communication as well as actual labeling of accessible ACM and areas where it is likely that ACM will be encountered. Should redevelopment occur, a comprehensive Operations and Maintenance (O&M) program would need to be initiated pending the specific areas and work that would be occurring in the facility. The components of an O&M program are outlined in the Guidance for Controlling Asbestos Containing Materials in Buildings (EPA Purple Book). This document is published by the USEPA, Office of Pesticides and Toxic Substances (EPA 560/5-85-025, June 1985). Demolition of the on site structures would necessitate the removal of all of the identified friable asbestos (i.e. pipe wrap and transite ceiling tile) as well as the transite siding, which is considered Category 2 Non-friable requiring its removal due to the material becoming friable during demolition. The floor tiling and mastic samples, considered Category 1 non-friable asbestos, would not have to be removed prior to demolition activities. Limitations The conclusions and recommendations presented herein are based on the level of effort and investigative techniques referenced in the report.
