We are pleased to present the 2011 Santa Clara County Systems of Care HIV/AIDS Resource Directory. The directory was created to connect you or your patients/patients to needed services. Our hope is that this Directory will promote linkages between people in need and service providers, and assist in navigating our local Systems of Care for those living with or affected by HIV/AIDS. The information in this Resource Directory is accurate as of August 2011. If you would like to submit corrected services and/or other information, please e-mail hivprevention@phd.sccgov.org. The information in the Resource Directory is intended to make you aware of the HIV/AIDS services and providers in Santa Clara County, and is not an endorsement of any particular agency/organization or service. When looking for services, please remember to contact the service provider(s) to find out if there are any eligibility requirements and/or if they are currently able to take new patients/clients.
