Directions for Completion of the ASC Academic Coach (Tutor) Application Forms Complete all forms included in this application packet as explained below. If you need assistance, please contact any of our Academic Skills Center staff. We will be glad to help you. Page 3: Read, complete and sign Academic Coach Training Agreement. Page 4-5: Complete all sections. Currently, ASC work hours are 8:00 a.m. to 7:30 p.m., Monday through Thursday; 11:00 a.m. 4:00 p.m., Friday; Saturday and Sundays closed. Times may vary depending upon the location of employment and funding. Pages 6-7: Read the ASC Tutor Contract thoroughly. Your signature indicates that you are in agreement with the conditions of employment and understand that you will be held responsible for meeting these requirements. Page 9: Bring completed application packet including TB Test results, a valid picture ID, and your original Social Security card to your appointment. (International and Work Study applicants may have additional requirements.) Page 10: Please complete. Some of this information may be used for ASC promotional materials. Page 11: Office Use Only Transcript: Please provide us with an unofficial transcript obtained through RegE or from the college that you attended. Submission of Application Packet: Bring the completed application, and any other related paperwork to the Academic Skills Center (ASC) office, I4-203. Remember to provide a copy of your most recent academic transcript from Mesa or last college or university you attended. Academic Coach (Tutor) Training Agreement I,, understand that I am obligated to complete ED 100 (Admissions enrollment) with a passing grade, either before or concurrent to my first semester of tutoring. If I fail to pass and/or complete ED 100 the first time, I will be able to retake it once more NOT CONCURRENTLY. If I do not complete/pass the tutor training course the second time, I will not be eligible to tutor for the Academic Skills Center at Mesa College. ED 100 is a one-unit, 8-week, hybrid class that has mandatory face-to-face meetings. This course prepares college-level students for tutoring adult/college students. Student trainees learn about tutoring methods as well as how to use appropriate written and mediated instructional materials. The course includes supervised tutoring practice." ASC Tutor Contract Name Job Requirements I understand that to be an Academic Coach, I must maintain an overall G.P.A. of 3.0 or Above. I will abide by all SDCCD and Mesa policies and procedures. I will abide by all ASC and discipline-specific procedures and guidelines. I will abide by the Academic Coaches' Obligations/Responsibilities as stated in the ASC AC Manual. Employment Training I understand that to be eligible to work as an Academic Coach and be rehired for subsequent semesters I must complete the one-unit, EDUC 100 Tutor Training: Level I; if not, I will be dismissed from the ASC. I will obtain a course textbook from the ASC. I know that this course has on-campus meetings, and if I have an unavoidable conflict, I will contact the course instructor well before the class begins. I understand that I must complete the Level I course in which I enroll. I agree to arrive on time, to be prepared, and to participate in all training activities. I will contact my course instructor and Carol Sampaga, I4-203, well before the course begins about any concerns I have regarding employment training. I understand that I may need to complete on-going, discipline-specific tutor training and, if so, that I will follow the requirements established by my discipline supervisor. Job Responsibilities and Professionalism I understand that it is my responsibility to accurately complete and turn in all required paperwork and timecards in a timely manner. Failure to do so may delay my paycheck. I will arrive on time for work-related assignments, classroom visitations, and scheduled work hours. I will take my breaks, whenever possible, when the center is least busy and will not exceed the maximum time allotted: work less than two hours = bathroom breaks; 2.5 to 3.5 hours - 5" - 10 paid break; 4 to 5.5 hours = 15" paid break and 30 unpaid lunch; 6.0 to 7.5 hours = two 10" paid break and 30 unpaid lunch; 8 hours - two 15 paid breaks and 30 unpaid lunch. I will accurately record and not exceed my budgeted number of work hours except with written permission from my supervisor in collaboration with my faculty coordinator. I must sign my timesheets before the end of each pay period. I will report any problems arising from the day-to-day operations within the assigned work area to the appropriate personnel. I will take responsibility to assure that daily operations of the center proceed smoothly. I understand that my employment with the Academic Skills Center will be terminated if I fail to keep my appointments, if I am not present during my assigned times, if I do not notify my coordinator and supervisor of absences, or if I am excessively late/absent. I will perform other reasonable duties assigned by the ASC staff or designee. I am expected to maintain confidentiality with all records. Hire Process Should you be selected to work for the Academic Skills Center, you will be hired as a Non-Academic Non-Classified Employee (NANCE) through Human Resources (HR) or as a Work Study Student Worker through Financial Aid or the Veterans Office. Drop off your completed application (including unofficial transcripts) to Carol Sampaga, Academic Skills Center Supervisor, Room I4-203. Schedule an appointment with Carol (619 388 2966 or csampaga@sdccd.edu) to review the employment packet and verify your training schedule. The employment packet consists of several forms to complete, including a test to ensure you are free of Tuberculosis. If you do not have medical insurance, Student Health Services offers the test to current students. There are also community Health Centers around the city that offer the exam but at a higher cost. The fees are not reimbursable by the Academic Skills Center. After verification that you do not have active Tuberculosis, follow up with Carol to submit the employment packet. Bring a valid picture ID, your original Social Security Card, and the days and times you are available to work to the follow-up appointment. Several items to note: International Students, VA Work Study, and Financial Aid Work Study will have additional requirements. Processing by Human Resources typically takes six to eight weeks from the time completed packets are submitted. Employees cannot start work until HR has provided clearance. Carol will inform you of your start date. Delays in processing will occur for incomplete forms/packets, holidays, and school closures. If you are not a full-time student, there are limits with the number of days and hours which you can work in a fiscal year. Academic Coaches are hired on a semester-to-semester basis. Assignments are temporary in nature with no guarantee of continued employment. Rehires are based on satisfactory evaluations.
