Please take a few minutes to fill out the following information. It was approved by Dean Jason Lemon on August 31, This policy statement incorporates and references applicable University of San Diego policies and adds policies designed to implement and supplement those governing standards and guidelines. This policy statement supersedes all policies in place prior to its approval. This statement of policy is supported by a set of procedures designed to assure compliance with these policies. 3 Adriana Serrano Manager, Business Programs Instructors USD faculty and experienced and qualified practitioners approved based on the guidelines and regulations identified through Professional and Continuing Educations Policy. Course Format and Schedule Programs are offered online or in class and are offered at flexible and convenient times that allow busy professionals to attend. Hours vary depending on the program but are usually scheduled from 6 to 9:10pm or 8 to 5pm. Courses are typically 12.5 to 25 hours in duration. Classes are not held on New Year s Day, Martin Luther King s Birthday (observed), Memorial Day, Independence Day, Labor Day, Thanksgiving Day and the day following and Christmas Day through New Year s. Enrollment Policy Students may enroll in the different programs by mail, in person or online. All students must create a student account in the following URL and must pay the certificate fee for the program they are enrolling in. Records Student Record of Certification of eligibility for Veterans Benefits, enrollments and transcripts will be maintained in the Professional and Continuing Education Department. Attendance a. Students must meet a minimum of 70% to satisfactorily complete the course. Should the attendance requirement not be met, the program office will have a discussion with the student to address the matter and students may be reinstated after submitting a letter to our office petitioning his/her intent to continue in the program, with the new prescribed academic plan. The plan has to be approved by the Program Director, prior to student making up attendance hours. Grading b. Students are evaluated in all credit-bearing courses that carry a grade. 3
4 c. Grade points are assigned as follows: A, superior; B, very good; C, average; D, inferior; F, failure. Grade points are assigned to the above grades as follows: A = 4.0; A- = 3.7; B+ = 3.3; B = 3.00; B- = 2.7; C+ = 2.3; C = 2.00; C- = 1.7; D+ = 1.3; D = 1.00; D- = 0.7; F = d. The Grade Point Average (GPA) for awarding a certificate "With Distinction" must be at least 3.5 (out of 4) or higher, and shall be based only on credit courses taken from the University of San Diego Division of Professional and Continuing Education. Substituted courses from institutions outside USD shall not be used in this calculation. e. All courses taken must be completed with a letter grade of C or higher. The final GPA should be 2.5 (out of 4) or higher, based only on credit courses taken from the USD Division of Professional and Continuing Education. f. The program office will monitor your progress and grade point average and certify your enrollment in the Veteran Administration. The VA will be notified if you fail to meet the minimum requirements. g. Students will be contacted after approximately 33% of their program if the overall GPA is below a 2.5 and will be dropped from a program when student does not meet the minimum requirements stated. The program office will have a discussion with the student to address the matter and will provide notification in writing. Students may be reinstated after submitting a letter to our office petitioning his/her intent to continue in the program. Refund Policies Students who need to drop a course may receive a refund, if the request is received according to the policies listed below by course Instructional Method. Classroom Courses: To receive a refund, students need to drop the course through the MyContinuingEducation@USD student account portal prior to the first day or start date of the course. The refund of the course fee will be returned less a processing fee of 10% or $50, whichever is greater, of the course tuition. There are no refunds once the course begins. Online Fixed-Date Courses: To receive a refund, students need to drop the course through the MyContinuingEducation@USD student account portal before midnight on the third day of the course. The refund of the course fee will be returned less a processing fee of 10% or $50, whichever is greater, of the course tuition. No refunds will be issued after this deadline. 4
5 Independent Study Courses: To receive a refund, students need to drop the course through the MyContinuingEducation@USD student account portal within three (3) days of registration. The refund of the course fee will be returned less a processing fee of 10% or $50, whichever is greater, of the course tuition. Refund requests will not be processed after this 3-day period. A full refund is processed without penalty, if PCE cancels the course. Exception: If a student leaves the tuition funds in their account to apply to a future enrollment prior to the above deadlines, the drop fee is waived. Students that are deployed or put in active duty may receive a full refund after submitting a formal petition to do such. Drop Policy A course section Drop Request is accepted anytime, however, please read the Refund Policies for more information regarding any tuition fee refund that may apply. Dropped courses after the refund period will receive a grade of "W" on transcripts. Students can request a drop through the MyContinuingEducation@USD student account portal. (For HCIN courses please refer to USD Main Campus policies). Note: Students who do not submit a drop and have not submitted assignments to complete the course by the time of grading will be graded as NG (no grade). Transfer Policy within USD Courses Students requesting a transfer must comply with the policies listed below by course Instructional Format. No transfers will be issued after the deadline. The Transfer Request must be sent from MyContinuingEducation@USD student account portal and be received as follows: Classroom Courses: Prior to the first day or start date of the course. Online Fixed-Date Courses: Prior to midnight on the third day of the course. Online Self-Paced and Independent Study Courses: Within three days of registration. Credit Transfer 5
6 Students must complete the Transfer of Credit petition form upon admission into their program for credit to be evaluated. Transcripts and other documentation will need to accompany the Transfer for Credit petition form, and form must be addressed to Program Director. Substitutes Substitutions and waivers refer only to credit courses. Substituting one course for another within a certificate program is a straightforward replacement with the units of the substituted course counted toward the overall required hours for the certificate. When a course is waived a student must complete other courses at USD Professional and Continuing Education in order to make up the total program units required. Waiving courses does not decrease the total units required to complete a Certificate Program. No more than 1/5 of the semester units required for the certificate program may be substituted or waived. A substituted course must be from a regionally accredited academic institution. Each substituted course from another regionally accredited academic institution must be completed with "B" grade or higher. The responsible program director must approve the academic content of each substituted course or the written reference material or each waived course. The responsible program director should forward transcripts or written reference materials documenting substituted or waived courses to the Division of Professional and Continuing Education for entry on the student academic record. After entry, the Division of Professional and Continuing Education will retain these materials for a period of five years. The dean may approve exceptions to these substitution and waiver requirements. Additional USD Policies Additional USD Policies PCE adheres to USD campus policies. 8 Checklist Eligible VA students may utilize their educational benefits for approved certificate programs only. Students applying for VA Education Benefits must provide the following documents: Chapter 30 (Montgomery GI Bill) USD VA Education Benefits Bulletin Form A certified copy of your DD214 Member Copy 4 (or NOBE if still on active duty) Submit a copy of VA form # , or VA Form # if you previously used VA Education Benefits An official transcript from each post-secondary institution you previously attended. Chapter 35 (Dependents) USD VA Education Benefits Bulletin Form Submit VA Form # , or VA Form # if you previously used VA Education Benefits An official transcript from each post-secondary institution you previously attended. VA EDUCATIONAL BENEFITS WILL BE PROCESSED BY USD PCE WHEN All course fees are paid. Students must pay their course fees in full (except Chapter 33). Checks are to be made payable to University of San Diego. All standard enrollment deadlines and late fees apply. All proper forms, as listed above, have been submitted to USD PCE VA Representative. 8
9 SUBSEQUENT ENROLLMENTS Contact our VA Representative each semester after you have enrolled & paid for all of your classes. Chapter 33 students must contact our VA Representative each semester with their enrollment request listing all courses and section ID numbers they wish to enroll in for that quarter. STUDENT RESPONSIBILITIES VA funded students are responsible for notifying the VA Representative immediately of any action affecting their enrollment status (such as course drops and/or non-passing grades). Failure to do so may result in termination of benefits. Students must verify their enrollment with VA each month to receive payment for that month. Your enrollment can be verified on the last calendar day of the month by using Web Automated Verification of Enrollment (WAVE) at or by calling the VA toll free at 1 (888) Satisfactory progress toward certificate completion must be maintained. Students must follow the course requirements listed in this Bulletin. Follow the USD Standards of Student Conduct and Academic Integrity. Students will at all times, when on campus or in any virtual classroom (Online Learning), conduct themselves in an orderly and considerate manner. Violation of these standards is just cause for dismissal. TUITION ASSISTANCE Students who qualify for VA Tuition Assistance must submit the authorization contract from their Base Education Office in person at our USD Campus at the time of enrollment along with your completed USD Bulletin Enrollment form. Your payment (or student share) not covered by the government is due at the time of enrollment. 9
