Minimum Admission Requirements Please note that for admission consideration, applicants MUST meet the minimum overall B (72 percent) in their undergraduate degree with at least a B+ (77 percent) in the final half or 20 single-term/semester graded courses. Applicants must have obtained a Baccalaureate in Nursing from a program accredited by CASN or approved by the appropriate provincial body. PLEASE NOTE FOR ADMISSION TO THE PHCNP CERTIFICATE ONLY (OPTION 1), STRONG PREFERENCE WILL BEGIVEN TO APPLICANTS WITH A MASTER S DEGREE IN NURSING OR A RELATED FIELD. IF YOU DO NOT POSSESS A MASTER OF NURSING DEGREE OR EQUIVALENT AND ARE INTERESTED IN THE PRIMARY HEALTH CARE NURSE PRACTITIONER CERTIFICATE, YOU ARE STRONGLY ENCOURAGED TO CONSIDER TAKING THE PHCNP PROGRAM IN CONJUNCTION WITH THE MASTER OF NURSING (OPTION II). In addition, admission to the Primary Health Care Nurse Practitioner Certificate requires the following: 1. Applicants must have the equivalent of two years of full-time nursing practice (3640 hours) within the past five years by September 1, 2010. 2. Other documents, such as official transcripts, written profiles, and references are required. Please see the Required Supporting Admission document for detailed descriptions. 3. Current RN Registration by the College of Nurse or eligibility for registration in Ontario. Application Fee A non-refundable cheque or money order for $100 Canadian, payable to Ryerson University must be submitted with the application form. Applications received without the required fee will not be processed. Application: First Consideration February 1, 2010 Applicants who submit all required documentation by February 1, 2010 will be given first consideration for admission. Please note that the cycle for September 2010 admission closes on June 30, 2010. 1
Transcripts Include in your application package, two officially certified and officially sealed undergraduate and, where applicable, graduate transcripts, from each post-secondary institution attended. Official transcripts are transcripts received by the Graduate Admissions Office in a sealed/stamped envelope directly from the issuing institution. Do not open the envelope(s). Transcripts received in an unsealed envelope cannot be considered official. Photocopies/student copies of transcripts will not be accepted. Arrange for transcripts with final marks for courses in progress to be sent directly from the institution to the above address. If your transcripts/documentation are in a language other than English, you must also submit notarized English translations. Translations do not replace original documentation. Both must be submitted. An official translation must be received in a signed/sealed envelope directly from an accredited translation service. If your transcripts are from institutions outside North America, you may be required to submit the institution s grading scheme, accreditation statement and profile (date established, programs offered and relevant course descriptions). Ryerson University Transcripts Applicants are not required to submit official transcripts of study undertaken at Ryerson University if they were registered for the first time after September 1984. TWO (2) copies of the application form must be submitted. If you were previously assigned a Ryerson Student Number or Reference ID, please provide the student number. Name State your first, middle and last name (legal family name). If they appear differently on your official documents, indicate the version appearing on your official documents. Personal Data/Status in Canada All applicants should complete these sections. Canadian Citizens, Permanent Residents, or those applicants presently working in Canada must indicate their Social Insurance Number. E-mail Please print your e-mail address clearly. If the Graduate Admissions Office needs to contact you this will be the address to which they will direct their correspondence. Address Information In this section, please be as accurate as possible. Expected Enrolment Date Please note at this time Ryerson University admits new students in the Fall term (September) only. Academic History You must list all universities and/or colleges that you have attended, even if a degree has not been awarded. Note: Official transcripts from each university attended must be submitted. State the name of the degree/diploma that you received (or are expected to receive). For example, if you were awarded a Bachelor of Arts degree, you would write B.A. here. State the specific year and month during which you were enrolled in the program. If you have not yet completed your program, state the approximate date of graduation. Specify the language of instruction at the universities you attended. English Proficiency Test Results Applicants whose language of instruction during their undergraduate/graduate studies was a language other than English are required to submit a test of English language proficiency. Some programs have a higher requirement than the minimum required by the School of Graduate Studies so applicants are advised to consult specific program policies on English proficiency. For further details or updates, visit http://www.ryerson.ca/graduate/admissions. Please be aware that where an English Proficiency Test is required, the application will not be assessed for admission without a test result that meets the program minimum. For exemption from this requirement, applicants may be required to provide an official statement from their institution confirming the use of English as the language of instruction and examination. Applicants may demonstrate facility in English using one of the following methods: Test of English as a Foreign Language (TOEFL). For reporting purposes, please use Institution Code 0886-02. International English Language Testing System (IELTS) Michigan English Language Assessment Battery (MELAB) The current test score minimums for the Primary Health Care Nurse Practitioner Certificate are as follows: TOEFL Internet-based: Overall score of 93, plus 22 minimum in Speaking and 22 in Writing sections TOEFL computer-based: Overall score of 237 TOEFL paper-based: Overall score of 580 IELTS: Overall band score of 7.0 MELAB: Total score of 85 Letters of Recommendation Referees should be well acquainted with your education and abilities, and are normally professors and/or your workplace managers or those in a position to speak to the reference requirements. Provide each referee with a Letter of Recommendation form or instruct them to print it from the website. All letters of recommendation, whether they are submitted in a Ryerson University envelope, or in the envelope of the referee s university or institution, must be sealed and signed by the referee across the sealed flap and dated. If not sealed and signed properly when received, they WILL NOT be accepted.
