POSITION DESCRIPTION Position Title: Community Services Manager- Purpose of Position: To provide leadership within Tallangatta Health Service (THS) in conjunction with the Executive, to achieve the organisations vision, through the planning, delivery and evaluation of a quality Community Health Service (CHS). The Community Services Manager (CMS) is accountable for the efficient and effective management of all human and material resources within CHS. Program: Community Services Program Role Summary: 1. Demonstrated effective Human Resource Management skills and knowledge pertinent to CHS. 4. Demonstrated commitment to ensure a high level of customer service. 7. Effectively communicates and promotes good interpersonal relationships both within and outside the organisation. 8. Ensure documentation and administrative responsibilities conform to legal and legislative requirements and THS policy. 9. Demonstrated knowledge of and a commitment to Quality Improvement, Risk Management and the accreditation process. Qualifications/Experience: Mandatory o Registered Nurse / Allied Health Professional with current registration with the Australian Health Practitioner Regulation Agency o Post-registration qualification in management, community health or significant experience in management, administration or associated discipline, or working toward the same. o Conceptualisation of the wider health service sector environment and the opportunities that could arise from this. o Knowledge of current statutory requirements, including relevant Awards, OH&S etc. o Financial acumen and the capacity to enhance the budget and services. o Demonstrated highly developed interpersonal, analytical, writing, research and business management skills o Demonstrated ability to affect a culture of continual improvement resulting in positive, best practice outcomes. o Satisfactory criminal history check and issue of an ongoing police certificate. Desirable o Membership of relevant professional organisation. o Computer literacy with word-processing and data base programs, e.g. HMS, Microsoft office. o Ability to analyse and utilise clinical data in service development and quality improvement initiatives. o Community Development experience. Award Classification: Nurse Unit Manager (NUM) / Management and Administrative Officer Grade 5 (HS5) Performance Review and Development:
A performance review will occur three months from commencement of employment and then annually or earlier if required. Review criteria will be based on the KPI s as outlined. Organisational Environment / Reporting Relationships: The CSM is accountable through to the Director of Nursing (DON). The CSM must assume responsibility and be accountable for her/his own actions whilst promoting professional standards of practice and conduct. Supervisory Responsibilities: This position provides leadership to: Allied Health Assistants Community Nurse(s) Allied Health Services (in-house & / or contracted) o Counsellor o Occupational Therapist o Physiotherapist o Podiatrist o Dietitian District Nursing HACC Services Health Promotion Intake Coordinator Practice Nurse(s) Diabetic Educator Others as determined Primary Responsibilities: KPI ~ 1 Strategic Imperatives: To effectively coordinate and efficiently manage a range of primary care initiatives and direct client services, consistent with the Mission Statement of THS, THS Strategic Plan and the current Community Health Plan. Advise on strategic and service delivery issues consistent with the organisational strategic plan, the relevant Governmental strategic plans and evolving service / clinical trends. Advice and converse with the Executive by providing input into the strategic direction of THS and specific direction and guidance relating to the CHS. Develop and implement business objectives for the CHS which are consistent with the strategic direction of THS. Provide strategic leadership and developmental direction to ensure an optimal level of service quality, particularly within the services in the CHS. Contribute to the future development of THS through monitoring community, business and operational needs and make appropriate recommendations for service delivery and development. Ensure that sufficient resources are available to meet the competing demands and different funding streams, e.g. Community Health, Health Promotion, HACC. Ensures effective planning, development, co-ordination, monitoring, evaluation and supervision of CHS. Ensure productivity targets are understood and met by the CHS. Actively and strategically grow the CHS to further meet the needs of the organisation and the community. KPI ~ 2 Leadership and Management: A thorough understanding of the principles underpinning primary health care and health promotion.
Responsible for operational and financial performance of services. Provide opportunities for staff in CHS to develop innovative and sustainable service models in their areas of expertise. Demonstrates a sound knowledge of professional Standards of Practice and relevant legislation. Identifies and promotes strategies that drive best practice in all areas of CHS. Promotes an environment that empowers and motivates staff to achieve organisational and CHS. Co-ordinates the development of salary and non-salary budgets, takes responsibility for the efficient management of expenditure of the CHS and ensures that strategies are developed and implemented to manage budget variances. Promotes the professional development of all staff in CHS. Identifies gaps in CHS resourcing and/or service provision, and participate in appropriate partnerships making innovative and relevant recommendations on how to address such areas of concern. Actively promotes the organisation s Mission, Vision and Values to all staff at THS. KPI ~ 3 Human Resources Management: Ensures recruitment strategies are developed and aligned to THS achieving its strategic imperatives Ensures training and development strategy and plans is aligned to strategic imperatives of health service. Develops a human resource planning strategy appropriate to the needs of the services of the CHS. Ensures staff appraisals are completed annually. Ensures staff are appropriately credentialed. Promotes the monitoring of workload and productivity in all services of CHS. Participate in staff recruitment, including preparation of position descriptions, interviewing of potential staff, staff orientation and preceptoring, staff development including the development of individual Key Performance Indicators. KPI ~ 4 Clinical / Technical Skills and Abilities Promote the co-ordination of comprehensive nursing and allied health assessment processes that ensure patient/client/resident needs and expectations are identified and met. Provide opportunities for staff to develop innovative and sustainable service models in their areas of expertise. Promotes the continuous improvement of co-ordinated care plans and continuing management plans, by the health care team, that address the needs and expectations of patients/clients/residents. Ensures effective planning, development, co-ordination, monitoring, evaluation and supervision of CHS. Promotes and incorporates education and health promotion / illness prevention in patient/client/resident care plans and service delivery. Manage and promote to all staff the efficient and cost effective use of all resources. Influence Best Practice in all aspect of care utilising principles of Evidence Based Practice and Action Research. Promote accountability in practice by all staff within Community Service Division. Predict, control, research, evaluate and make recommendations for purchase of new equipment or consumables making the appropriate budget adjustments or submissions to relevant funding bodies. Demonstrate knowledge of THS policies and procedures that have legal implications for practice, and promote staff compliance. Protect the rights of individuals and groups, and assist informed decision making. Promote a physical and psychosocial environment that enhances safety, security and optimal health for clients/patients, visitors and staff.
Participate in the maintenance and update of THS emergency plans, policy and procedures applicable to the Community Service Division, providing staff education to maximise organisational effectiveness in a crisis situation. Effectively market, promote and advocate for the program within THS and the broader community. Participate or delegate a representative as required in committee / working groups as designated or required by the Director of Nursing / Chief Executive Officer, implementing policies or procedures, preparing necessary reports or recommendations. Work collaboratively with relevant local services to ensure integrated, comprehensive service provision. Facilitate collaboration between Tallangatta Health Service and other health networks as required regarding clinical guidelines, client management, secondary consultation and adoption of integrated and interfacing approaches. KPI ~ 5 Personal and Professional Development: Presents as a role model in all areas of management and leadership within the organisation and CHS. Maintain and enhance current knowledge and skill base that facilitates the best possible client care. To act within professional and ethical standards outlined by Australian Health Practitioner Regulation Agency (AHPRA), THS Code of Conduct and abide within the Health Practitioner Regulation National Law (Victoria) Act 2009. Maintain an active professional development program by: o Attendance and participation in relevant courses, workshops, lectures, tele/video conferences, evidenced by provision of an attendance record of in-service education sessions. o Sharing of knowledge and skills so gained with other staff via formal and informal training. o Provide in-services appropriate to staff sharing knowledge and skills, learning and o experience. Attend mandatory education on an annual basis including: Fire and Evacuation Manual Handling Cardiopulmonary Resuscitation Infection Control Exhibit flexibility, creditability, commitment, enthusiasm and caring to staff, clients and the organisation. KPI ~6 Customer Service: Drives customer service excellence across the Community Services Department and THS. At all times promote good interpersonal relationships both within and outside the organisation, recognising internal and external customers. Provide education and act as a resource to: Other staff, students, clients and carers. Community groups and other agencies as appropriate and required. Comply with and promote to all staff the THS Code of Conduct. Ensure that patient rights of human dignity, confidentiality, privacy and informed consent are maintained. KPI ~ 7 Teamwork and Communication: Promotes an environment that empowers and motivates staff to achieve organisational and CHS objectives. Make informed decisions confidently and solve problems fairly and effectively using staff input.
Promotes timely communication of information regarding continuing management to health care professionals, community services and carers. Liaise with other disciplines to achieve common goals, promoting good interpersonal relationships and recognition of skills. Maintain a cooperative working relationship with all staff. Communicate effectively and accurately and document relevant information according to THS policies and procedures. KPI ~ 8 Documentation and Administration: Ensure that statistical data is appropriately documented and that it meets the reporting requirements of the state and federal agencies. Ensure that all clients related documentation is completed by staff in a concise and timely manner meeting all legislative and professional guidelines. Document patient information in a clinically effective manner that conforms to legal requirements and THS policy. Participate in the ongoing development, revision and implementation of policies and procedures within Community Health. Maintain confidentiality of information in compliance with privacy legislation. Contribute to and maintain where required data collection for statistical purposes and collate where required Department/Program data for presentation to appropriate bodies. KPI ~ 9 Continuous Quality Improvements and Risk Management: Provides leadership in the development of strategies and initiative for improving performance and service delivery across CHS. Demonstrates a commitment to quality and promotes evaluation/reflection and continuous improvement in all areas. Promotes the development and implementation of organisational policies and standards, which ensure compliance with professional standards and relevant legislation. Participate in and promote the value of research that contributes to development of evidence based nursing practice and improved standards of care. Adheres to and promotes to all staff THS Infection Control policy and procedures, promotes to all staff Department of Health Guidelines and Australian Standards relating to infection control. Adheres to and promotes to all staff THS Occupational Health and Safety (OH&S) policies and procedures and the OH&S Act (Vic) as amended 2005 and related regulations, this includes ensuring minimal lift principles are adhered to and maintained by self and staff. Maintain regular checking of the environment and equipment to ensure safe, effective and efficient practice. Promotes the maintenance of a safe, secure and clean environment. Facilitates the identification of risks to staff, patients/clients/residents or visitors and ensures appropriate corrective action is implemented. Maintain an awareness and understanding of THS Workplace Bullying policy and ensuring behaviour is free of harassment, discrimination or victimization towards any other person. Maintain and update knowledge of THS emergency plans, policy and procedures to maximise effectiveness in a crisis situation.
