330 Guildford Road Your own personal space The house has five bedrooms over two floors. Each is bright and airy, has lots of storage, and we can easily organise for special equipment if necessary, including profiling beds, specialist chairs and hoists. 11
330 Guildford Road Person-centred support We understand how important it is to treat each of our residents as an individual and to get to know, understand and respect their experiences, interests and dreams. We take time getting to know each person and their family. By doing so we can create a support plan that s uniquely and sensitively tailored to them and supports them to lead the life they want. Promoting positive risk taking Our home manager and team always conduct a comprehensive risk assessment before supporting an individual with any new activity, investigating the pros and cons and talking to the resident and their circle of support to understand what an acceptable risk might be. Focused on independence Our approach is based on a nationally recognised care and support planning model that emphasises the importance of promoting independence. We have built up a comprehensive knowledge of the local community and formed many lasting relationships with local groups and centres, offering a greater level of community inclusion for the people we support. Every activity as part of an individual s dynamic weekly programme is chosen with them to meet their own personal goals. Our person-centred approach enables us to identify and develop opportunities, with paid work being the ultimate goal. Over the years we ve developed strong relationships with local employers and colleges. As a result, the links are in place to enable the people we support to attend life skills centres, work place and day centres, as well as local colleges and education centres, so they can develop the vital skills they ll need to move into paid employment. 19
330 Guildford Road Keeping healthy Each individual is registered with a GP and supported to access regular health checks at local surgeries. Pictorial health picture banks enable individuals to be involved in making decisions about their health. Keeping active is a big part of everyday life at Guildford Road. The Sussex Road Recreation Park is close by and cycling, swimming and horse riding are all easily accessible in the local community. We also offer access to a range of specialist services within the home and local area including chiropody, music therapy, optical support and aromatherapy. 20

Support to build up and maintain the skills required for daily living is always on hand. Where appropriate and with the correct level of support, individuals are encouraged to help around the house with domestic tasks, cooking, cleaning and spending time in the garden, such as watering the plants. 23

330 Guildford Road Comprehensive transition planning Change makes everyone anxious so that s why we involve each resident and their circle of support in all decisions, before, during and after their stay with us. Trying us out We want people to feel comfortable about living at 330 Guildford Road, so we offer prospective residents the chance to come along and try our facilities before they commit to joining us. This might be for a few hours to begin with and even a weekend stay if they want to fully experience what we have to offer. Getting ready to move in Before an individual joins us, we ll conduct a comprehensive assessment with them and their circle of support (if appropriate). This helps us to identify the individual s capabilities and develop any necessary behaviour support strategies. Case managers and support workers from previous placements also take part in these assessments if it s appropriate. This assessment helps us to create a development programme for the new resident, and this is regularly evaluated to check it s still relevant and effective. Twelve weeks after each individual joins us we produce a full report about their development and discuss it with the them and their support circle. 26
Forming friendships We re very careful when we introduce new people into the house. We want everyone to get along, and good friendships to form, so it s important that we take all personalities into account. 27

Over time Care UK has established an effective communication strategy that ensures that listening always translates into positive action for the people using our services. Our Listen to Me groups, living together meetings and other facilitated forums are part of this strategy. Funding our service Our current fee structure is based on a full assessment of an individual s needs in consultation with their care manager. We aim to reduce these fees over time as an individual progresses, gaining a greater degree of independence or needing a reduced level of support. Our ideal outcome is to help an individual into supported living. Residency is therefore often sold around shorter stays (typically up to 18 months) with person-centred support plans based around three key phases. Phase 1 Individual assessment, creating a personalised programme of support and meaningful routine Phase 2 Embedding the routine and developing the individual s programme of support to work towards identified outcomes Phase 3 Fine tuning the daily living skills and confidence required to move towards supported or even independent living 29
