All plans are monthly; the due date will be same day as Active Date. Long Distance Dialing Plans are Available to SafeTel Phone Customers in the entire State of Oklahoma. The Plans Include; Unlimited long distance which is defined as up to a maximum of 2,000 domestic minutes per month at $12.95 per month 500 1+ Domestic Long Distance Minutes at $9.95 per month 100 1+ Domestic Long Distance Minutes at $4.95 per month Long Distance is monthly program, renewable one month from purchase date or when minutes expire. Customers applying for New Service or Conversion in Month-1 will pay the $ amount of the plan including additional features, LD, or internet service, plus tax for the first month service. 2. Recurring Month-2 and there after, the customer will pay the stated $ amount of the monthly service including additional features, LD and internet service plus tax and fees. Due Date; of the customer s monthly service is the anniversary of the activation date one month later. Billing; a statement/invoice will be mailed to the customers, 20 days before the due date. The Customer must pay their monthly payment in full to avoid late fees and disconnect. 5
Grace Days are a 30 day period; Customer s monthly phone service is due on or before the due date. Customers have 10 days after the due date before the Late Fee is imposed The (11th day) after due date, a $5.00 Late Fee is imposed. The service amount and late fee must be paid by the 30 th day after due date. If the total amount due is not paid by the 30 th day after due date, on the 31 day after due date the customer s service is subject to disconnect. Long Distance Plans should be purchased to coincide with the Phone Service Plans due date. If LD is not purchased to coincide with phone service due date, LD due date will be the anniversary of the LD turn on date. There are NO Grace Days for Long Distance Plans. If the customers that receive the 100 Free LD Minutes decide to purchase another LD Plan, the free minutes will go away because there can be only one LD Plan on the Customer s Account at one time. 6
