Medical Administrative Assistant (Clinical and Administrative Support Team Member) 37.5 hours per week, permanent role South East Toronto Family Health Team South East Toronto Family Health Team (SETFHT) is an Interprofessional Family Health Team (FHT) undergoing exciting development and transformation in ensuring access to quality primary health care to our patients. This academic FHT is affiliated with the University of Toronto and is comprised of a team that includes physicians, medical residents, nurse practitioners, registered nurses, dietitians, social workers, mental health/addictions counsellor, care navigator, physician assistant and chiropodist, all supported by an exceptional administrative and clinical support team. Position Profile This position performs clinical and administrative tasks, on a priority basis, that support the daily operations of the South East Toronto Family Health Team (SETFHT) at our two clinic sites: 840 Coxwell Avenue & 1871 Danforth Avenue. This position provides clerical support to the physicians and Interprofessional Health Care Providers (IHPs) and may also include additional administrative support as determined by the Lead Physician, Executive Director and/or IT/Operation leads of SETFHT. Roles and Responsibilities Incoming Phone Operator Answer all incoming calls. Book appointments. Triage calls. Receive messages and direct to appropriate party. Check-In Desk Greet the patient in a warm and welcoming manner. Follow infection control screening guidelines. Maintain Pick-up box. Process incoming mail. Check and track phone messages very hour. Patient Communication Liaison Responsible for checking and acting on all incoming messages. Address items that are marked ASAP and deal with accordingly. Clinical Assistant 1. Patient Visit Coordination Greet the patient upon arrival, assess and direct to appropriate areas in SETFHT. Constantly monitor the schedule and ensure patients have arrived. Answers phone during peak times and sets up appointments for patients with appropriate physicians. 2. General Office Duties Perform clerical duties (e.g. photocopies, faxes, e-mails, pick up/deliver mail) for the team members of SETFHT. Assist with contacting patients with referrals. Assist with patient communications (e.g. writing or organizing letter mail outs, phone calls etc.) as it pertains to quality improvement strategies.

Ensure general cleanliness of clinic including waiting room and exam rooms. Support operations of clinic by proactively problem solving and following up when items need to be fixed or cleaned up. 3. Clinical Assistance Set up and maintain examination room (e.g., medical equipment, supplies) and ensures its cleanliness. Clean, sterilize and maintain medical instruments/equipment. Assist physician (e.g., PAP smears, suturing, ear syringing, liquid nitrogen, blood pressure, height, weight) as needed. Set up for procedures and cleaning up after procedures. Clean instruments (reference instructions in procedure room). Label and prepare the appropriate requisitions for specimens. Administrative Support, Data Management & Quality Improvement Daily Tasks in order of priority: 1. Down load and process labs (Min 5X per day) Record on daily tracking sheet. 2. Monitor and track outgoing fax terminal Record on daily tracking sheet. 3. Document transfer management into patients charts Record on daily tracking sheet. 4. Other Tasks: Performs clerical duties (e.g. photocopies, faxes, e-mails, pick-ups/delivers mail) for the team members of SETFHT. Process chart transfers, track receipt and payment. Process FHO forms and ensure corresponding billing. Billing support Chasing Health Card numbers Assist with QI items such as preventative screening Assists with patient communications (e.g. organizing letter mail outs, phone calls etc.) as it pertains to quality improvement strategies. Referral Specialist Process referrals that require special assistance. Track outstanding referrals. Keep address book updated with correct referral source information. Skills, knowledge and job requirements: Completion of Medical Secretary Course (medical terminology, pharmacology). Proficiency in computer applications: Windows, Microsoft Office programs, and email. Experience with clinical management system (CMS), preferably Practice Solutions Suite. Exceptional organizational skills. Excellent interpersonal and oral/written communication skills. Strong problem solving skills. Ability to maintain confidentiality and impartiality. Capacity to adapt quickly to a fast paced, dynamic work environment. High degree of accuracy and attention to detail. Ability to work independently and with a team (e.g. ability to work effectively with all members of the health care team). Ability to prioritize, manage time effectively and be flexible in a very active work environment. High level of accuracy and attention to detail. Exercises good judgment. Hours of Work The successful candidate must be available to work any of the following shifts: 8 am 4 pm, 9 am 5 pm or 12 pm 8 pm. Our clinics are open 8 am - 8 pm Monday Thursday, 8 am 5 pm Friday, 9 am 12 pm Saturday and 10 am 1 pm Sunday. This position will work out of both clinic sites as required. Compensation is based on experience and will range from $18.00 to $19.50 an hour. Position also includes enrolment in the Healthcare of Ontario Pension Plan and a comprehensive benefits plan. You will perform routine nursing duties, and ensure that the nursing needs of the SETFHT patients and physicians are met. The Registered Nurse s primary role is to work in partnership with SETFHT physicians and providers to perform patient care. According to The Nursing Act, the Nursing Scope of Practice Statement states that nursing is the promotion of health and the assessment of, the provision of, care for, and the treatment of, health conditions by supportive, preventive, therapeutic, palliative and rehabilitative means in order to attain or maintain optimal function. RNs within the South East Toronto Family Health Team will facilitate patient booking, ensure patient continuity and increase job satisfaction. Accountability The Registered Nurse (RN) reports to, and is responsible to, the SETFHT Executive Director. All clinical decision making outside the scope of the Registered Nurse must be done in consultation of the SETFHT Lead Physician and/or physicians of SETFHT. Authority Does not direct the activities of staff or a function without direction of FHT decision makers. The RN must comply with the nursing practice standards used by the College of Nurses of Ontario. Decision Making On a regular and continuous basis, exercises clinical judgement and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of service within the FHT.

Roles and Responsibilities Duties of the registered nurses will be divided into two categories clinical duties and duties related to the health team. RNs will be encouraged to operate within their full scope of practice. They will be encouraged to work cooperatively with all members of the health care team in the promotion of patient health and prevention of disease. Clinical Duties Triage incoming patient calls. Assess patient health care needs, discuss test results and respond to questions pertaining to health promotion and disease prevention. Redirect calls to booking when necessary. For patient appointments, initiate the record of the patient visit, purpose, history and health care needs. Measure and record patient vitals and prepare patient and room for the examination by the physician. Maintain the medical record alert system to recall patients for health promotion, disease prevention follow-up. Maintain the medical record Health Maintenance Plans to ensure patient compliance to timelines for health promotion, disease prevention programs such as pap smears, mammograms, FOB testing, bone density, and diabetes care. Administer and record immunizations and allergy injections. The nurse has a duty to follow the 5 rights of medication administration: right patient, right route, right dose, right time, right medication and associated contraindications. Ensure immunizations are current and educate patients as to the need for and possible side effects of such injections. Administer clinical procedures such as suture removal, skin and wound dressings, TB testing, wart treatment, and ear-syringing. Perform blood pressure follow-ups, assessing response to medications, educating patients and promoting health through blood pressure management. Complete pre-operative and MTO physical forms as far as possible. Maintain accurate, concise and confidential documentation pertaining to patient history and encounter data. Chronic Disease Management Work collaboratively with the health care team in the development, implementation and monitoring of chronic disease management programs around key health issues such as cardiovascular disease, diabetes and obesity. Interprofessional Collaboration Develop and hone collaborative community relationships to facilitate patient access to services upon physician/health care team request. Communicate ideas or concerns to the health care team. Participate in problem solving to enhance patient care. Assist in the training and development of new staff. Practice Environment Ensure adequate stock and appropriate storage of immunizations. Maintain clean, orderly and well-stocked examination rooms. Sterilize equipment for minor surgical procedures.

Qualifications Completion of a Bachelor of Science in Nursing with, ideally, two or three years of clinical experience. Must possess a license with the College Nurses of Ontario as a Registered Nurse. Knowledge of and working experience with computer systems and applications. Honed patient assessment and triage skills. Excellent verbal and written communication skills. Ability to work co-operatively as member of the FHT. Compensation is based on experience and will range from $31.00 - $33.00 per hour and also includes enrolment into SETFHT s benefits plan and HOOPP (Healthcare of Ontario Pension Plan). Clinical Pharmacist Position Profile As a key member of the South East Toronto Family Health Team (SETFHT), and an integral part of an Interprofessional health care team, you will deliver pharmaceutical care in accordance with SETFHT policies and professional practice standards. You will perform routine pharmaceutical duties, and ensure that the pharmaceutical needs of the SETFHT patients are met. Your primary role is to work in partnership with SETFHT physicians, learners, and other healthcare providers to perform patient medication interviews and assessments. You will also be involved in a pharmacist managed oral anticoagulation program and will assist to develop strategies for drug-related problems and provide objective drug information and drug therapy educational opportunities. You will deliver services to a variety of patient populations including high-risk groups suffering from chronic diseases and/or complex multi-system diseases. You will also provide health education to our patients with a focus on health promotion, disease prevention, and chronic disease management. As SETFHT is an academic institution affiliated with the Department of Family & Community Medicine at the University of Toronto, you will be involved in the education of medical learners which includes clinical teaching and seminars. Qualifications Minimum of Bachelor s Degree in Pharmacy is required. Registered in Part A of the Ontario College of Pharmacists is required. Community or Hospital Pharmacy residency or equivalent clinical experience in either setting is required. Additional training or credentialing in pharmacotherapy (e.g. Certified Geriatric Pharmacist, Certified Diabetes Educator, Certified Asthma educator, etc.) is considered an asset. JOB DESCRIPTION Social Worker (1.0 FTE) Position Profile The South East Toronto Family Health Team (SETFHT) is a primary health care clinic undergoing exciting transformation in providing Interprofessional health care services to our patients. We are currently seeking a full-time Social Worker at our 840 Coxwell Avenue site to join our team who will work collaboratively with the physicians and Interprofessional Health care Providers of our team by providing care for clinic patients and accepting referrals from, as well as consulting with the physicians as necessary. Provision of primary care will be within the scope of the Social Worker (SW) and she/he will play a key role in developing programs with the clinic to enhance patient care. Accountability The Social Worker reports to, and is responsible to, the SETFHT Executive Director. All clinical decision making outside the scope of the Social Worker must be done in consultation of the SETFHT Lead Physician and/or physicians of SETFHT and/or medical directives. Authority Does not direct the activities of staff or a function without direction of FHT decision makers. Must comply with the standards of practice guidelines of the Ontario College of Social Workers and Social Service Workers. Decision Making On a regular and continuous basis, exercises clinical judgment and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of service within the FHT. Roles and Responsibilities Is responsible for making clinical decisions, exercising judgement to ensure best possible care is provided within available resources; Provides counseling services, with particular regard to individual and family issues and decisions that impact the health of the patient and family. Works within the primary care model of brief therapy, ensuring seamless transition to community resources as appropriate; Is knowledgeable of the community and the resources available, especially in the areas of crisis services, Mental Health, and Long Term Care; Assists the referring physician/health care providers with referrals to secondary and tertiary health care centers; Knowledge and skill in group work including cognitive behaviour therapy, Stanford self-management program and Mind over Mood; Enthusiastic, passionate and self-driven who will participate in data management and measurement of quality improvement indicators of patient outcomes; Communicates results of measurements that are easy to use and understand, looking for patterns that could assist with improvement; Acts as an advocate for patients and their families in matters affecting health; Provides house calls and community outreach visits when appropriate; Liaises with physicians and other team members as required;

Teaches family medicine residents and other health care professional students on the role of the social worker and psycho-social issues facing patients and families; Effectively uses the Social Work standards of practice and the therapy process in assessing, planning, implementing, and evaluating the care of the patient; Maintains own continuing education to enhance professional knowledge and growth; Participates and collaborates with other Family Health Team members and health care providers in provision of patient care; Acts as a professional and knowledgeable role model or mentor to colleagues and students; and Maintains confidentiality in the work place and within the community. Qualifications Education & Specific Job Requirements Masters Degree preferred; Registered with the Ontario College of Social Workers and Social Service Workers; Previous related patient care experience, specifically in a primary care setting preferred; Minimum of 3 years experience; Experience in solution focused brief therapy, cognitive behaviour therapy, motivational interviewing and mindfulness; Ability to operate with the highest level of confidentiality and discretion; Strong conflict resolution skills; Exceptional oral, written and presentation skills; Strong interpersonal skills; Group facilitations skills; In depth knowledge of chronic illness care, including both clinical and supportive care; In depth knowledge of the local health program delivery and community based services; Able to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population; Ability to work effectively with all members of the health care team; and Ability to prioritize, manage time effectively and be flexible in a very active work environment.
