Family Practice Physician (Full time): Northern Sussex County/West Orange/Somerville. Seeking Family Practice Physician to provide direct clinical care to patients for the entire life cycle (i.e. pediatrics, adults and geriatrics). The Physician is a key member of the health center's clinical team with the purpose of providing evidence-based, state of the art and high quality medical care to Zufall Health Center patients. Qualifications include a minimum of 1 year of experience in a community health setting. Bilingual Spanish/English a plus. Possession of a MD/DO degree and active NJ license required. Board certified or board eligible in Family Practice. Valid DEA and CDS registrations. CPR/ACLS certifications. **Competitive salary and benefits along with malpractice insurance. Plus position is eligible for NHSC & NJ Primary Care Loan Repayment program.** Family Nurse Practitioner (Full time): Northern Sussex County and Hackettstown. Seeking Family Nurse Practitioner in our Sussex County location to diagnose, treat and manage health problems for children and adults of all ages. The Nurse Practitioner is a key member of the health center's clinical team with the purpose of providing evidence-based, state of the art and high quality medical care to Zufall Health Center patients. Qualifications include experience in women s health services. Knowledge of conversational Spanish strongly preferred. Able to relate to culturally diverse patients and staff, and to work efficiently and with other team members. Minimum of 1 year of experience in a community health setting preferred. Must have at least 1 year of experience in ambulatory care caring for children and adults.

2 Licensed Social Worker (Part time): Somerville and Morristown. Seeking Licensed Social Worker (LSW) to provide behavioral health services at the Zufall Health Center. The LSW provides direct services to patients, families and groups, provides individual and family counseling and engages in crisis intervention. Identifies opportunities for developing integrated Behavioral Health Services in the primary care setting and serves as a resource to the provider staff. Bilingual English/Spanish required. Master Degree in Social Work. Valid NJ license as an LSW. Preferred two years post graduate experience in Clinical/Ambulatory care setting. Behavioral Health Case Manager (Part time): Dover. Seeking Case Manager to assists patients and their families in accessing appropriate clinical and social services. The Case Manager is a key member of the Care Team and focuses on needs and barriers (such as financial, cultural, language, insurance) that prevent patients from achieving healthy outcomes. Travel is expected to be local with required attendance at some community meetings. Work hours are regular business hours but may include some evenings or weekends as needed. Bilingual English/Spanish required. Bachelor s Degree with appropriate health and/or social services related experience and a minimum of two years experience in a clinical setting. Certified Medical Assistants (Full time): Somerville (1) and Morristown. Seeking Bilingual Certified Medical Assistants, fluent in English and Spanish. Knowledge of ecw to assist providers with patient visit, inventory and administrative duties. Education requirements include Medical Assistant Certification/ Medical Assistant Registration from an accredited school for Medical Assistants. Current BLS Certification, Valid EKG Certification, Valid Phlebotomy Certification required. Must be able to independently travel to multiple Zufall sites, if needed. Patient Representatives (Full time & Part time): Somerville/Flemington/Hackettstown/W. Orange. Seeking Patient Representatives, to provide assistance with patient intake and a variety of clerical functions including professionally administering all incoming calls, ensuring phone calls are re-directed accordingly and greeting guests in a professional, friendly, hospitable manner. Bilingual English/Spanish required. Candidate should have excellent phone etiquette and verbal communication skills, Excellent reading, writing and spelling skills are required, must be punctual, have the ability to multitask and work with minimum supervision. Must be computer literate, with knowledge of eclinicalworks (ecw) preferred, but will train. Experience with medical or dental insurance a plus. Schedule will vary and can include evenings and weekends. Must be able to independently travel to multiple sites if needed. * Hackettstown/W. Orange/Somerville/Flemington (4 FT) and Somerville (1 PT) Dentist (Full time): Dover/Morristown. Seeking a Dentist for an expanding Community Health Center/Primary Care practice. The Dentist is a member of the health center s clinical team with the purpose of providing evidenced-based and equality dental care to patients under the general direction of the Chief of Dental. Must have possession of a current license to practice dentistry in New Jersey, valid DEA, CDS and BLS certification. Dental degree and residency years of experience working in health centers. Excellent verbal and written communication skills. Bilingual in English/Spanish a plus.

3 General Outreach & CEED Outreach Representative (Full time): Somerville. Seeking an Outreach Representative to conduct outreach and health education within the target community, particularly among residents of public housing, the homeless and farm workers with the goal of engaging new patients into care at Zufall Health Center in Somerville and identifying eligible candidates for the Cancer Education and Early Detection Program. The candidate must be flexible for evening and weekend events. Extensive knowledge of outreach strategies/ skills, public health issues and resources. Experience with motivating and supervising staff/ volunteers. Self-directed individual with public speaking experience. Strong verbal and written communication skills in English and Spanish. Must provide own transportation to meetings and events. Possesses flexibility, cultural sensitivity, and tactfulness. Maintains HIPAA compliance in all regulated transactions and complies with all regulatory standards. Maintain valid driver s license, automobile registration and insurance. Support Zufall Health Center s Mission. Fax Administrator (Full time): Hackettstown. Seeking an Administrator to manage all clinical documents faxed to the health center, as well as answer telephone and direct calls. The FA handles requests for medical records and scans paper medical charts into the electronic medical records. Bilingual English and Spanish required. High School diploma required; college credit or additional education in medical field or insurance and billing courses preferred, but not necessary. Must be able to independently travel to other Zufall sites as needed. HIV Linkage to Care Coordinator (Full time): Dover. Seeking a Care Coordinator to serve as the linkage to care point person for people newly diagnosed with HIV, patients living with HIV/AIDS that have been lost to care and those at high risk of falling out of care. This position will serve as a complement to the Zufall Health Center Ryan White HIV/AIDS primary care program. Bachelors or Masters Level Degree in Public Health, Psych, Social Work, or related. Minimum of three years of clinical experience in ambulatory care setting. Knowledge of Microsoft Office applications including Word and Excel, and use of electronic medical records. Minimum two years of experience in related field with patient contact. PCMH Care Coordinator (Full time): Hackettstown and West Orange. Seeking a Care Coordinator to work in collaboration with the patient, family, physician/nurse practitioner and office staff within the team structure to ensure that patient-centered, cost effective, high quality care is provided. Patients under the care of the Coordinator will be supported by the development of a multi-disciplinary care plan, self-management goals and referrals to both internal and external resources. Nursing Assistant or Medical Assistant training or other health care related field required. Licenses and Certifications as per training and education. 5 or more years of experience in a clinical setting requiring frequent contact with providers, patients and family member. Prior experience in an ambulatory care or outpatient clinic setting and case management experience preferred. Knowledge of the basic concepts and principles of managed care required.

4 Site Manager (Full time): Dover. Seeking a Site Manager to oversee and operate the Zufall Health Centers located in Dover. Responsibilities include all matters pertaining to clinical support functions, patient customer service, front office operations, facilities and management of support staff including front desk and clinical support staff. The Site Manager will be the administrative supervisor of the PCMH Coordinators and Dental Assistant Coordinator in regards to daily operations, scheduling and patient flow and will ensure that all performance reviews and staff management issues are addressed in a timely fashion. Minimum of three years of supervisory/management experience. BS or BA required, Master s preferred. Clinical background strongly preferred. Strong general computing skills, including internet, excel, word and practice management software experience. EHR experience a plus, especially ECW. Strong, selfdirected written and oral communications skills. Knowledge of federal programs and community health centers. Homeless Outreach Representative (Full time): Dover. Seeking an Outreach Representative that will identify Homeless and At-risk of Becoming Homeless individuals and families who are in need of preventive and primary health care services, including care for acute, chronic and communicable diseases, dental services, prenatal care, family planning, WIC and other nutrition services and pediatric care and immunizations. Possesses basic knowledge of target population s ethnic and medical needs. Possesses flexibility, cultural sensitivity, and tactfulness. Maintains HIPAA compliance in all regulated transactions and complies with all regulatory standards. Maintain valid driver s license, automobile registration and insurance. Support Zufall Health Center s Mission. Community Health Worker (Full time): Dover. Seeking a bi-lingual Community Health Worker (CHW) to serve in a program for the purpose of improving pregnancy outcomes. The CHW is a paraprofessional (a paid professional worker who does not need to have formal training) and trusted member of the community. CHWs will conduct neighborhood/community outreach in Morris County to identify, educate and link women of reproductive age who are not currently being served within the healthcare system. The CHW will provide community outreach and referrals, coordinating links to health care and human services, social and logistical supports with the goal of improving overall health in women, and thereby improving pregnancy outcomes county-wide. Familiarity and comfort with a healthcare environment a plus. Bilingual Spanish/English. Must have valid driver s license and reliable transportation. Dental Assistant (Full time): Flemington. Seeking Bilingual (English/Spanish) Dental Assistant responsible for assisting the dentist(s) and dental hygienist(s) in the direct provision of primary care dental services to patients of the center. The Dental Assistant is also responsible for sterilization, preparation and inventory control of dental instruments and supplies. Requires a comprehensive knowledge of dental assisting techniques and procedures for all phases of general dentistry. Knowledge of modern dental material, their storage, handling and applications is required. Fluent in both English and Spanish. High school diploma or GED certificate, certified Dental Assistant, current CPR (BLS) required. Registered Dental Assistants preferred.

5 RN-Behavioral Health: Parsippany. The Registered Nurse for the Primary Care Integration Program is a licensed professional that utilizes specialized knowledge, judgment and nursing skills in the triage and care of patients and maintains standards of the professional nursing practice in the ambulatory care setting. The RN works in collaboration with the patient, family, physician/nurse practitioner and office staff at both Zufall and Newbridge to ensure that primary care services are provided to patients at the Newbridge site. The incumbent plays an integral role in the identification of potential patients in being a liaison with the staff at both service organizations, in triaging and in the care, coordination and management of patients. This is a part-time professional position encompassing all nursing aspects of primary care services. Clinical skills in ambulatory care, knowledge of triage protocols, great communication, interpersonal relationships. Current valid NJ Licensed RN from a Nursing Education program. Current BLS certification. Minimum of 1 year experience in a clinical outpatient setting. Bilingual Spanish/English preferred.
