Patient Portal and Secure email: Our office staff will ask you if you want to communicate with us by secure email, and view your patient summaries by secure electronic Patient Portal. If you agree, they will ask you for your email and enter it into your Patient Information in our electronic record system and send you an invitation to join the portal. You will receive an email that looks like this on a PC: It says On behalf of South Bay Allergy and Asthma (at the red arrow). click or double click on message to open. You can also go to the patient portal by clicking this link: https://sballergyportal.gloclouds.com. (no www.) Opening the email on a PC gives you this window, Patient Portal Instructions, again, click on Click on this link Page 1 of 8
You can also get to this screen by going to https://sballergyportal.gloclouds.com. Another Welcome screen comes up. If you have been registered, first and last name will be filled in already. You would have to enter Date of Birth (DOB) and ZIP, and click Continue. The next window (below) will state you can choose your email and user name and password. We advise that you use a user name you can remember like firstlast name = minniemouse (no letters allowed between first and last name) or, you may use your email address as your user name. Then pick a password (make sure you use one you can remember), such as your email password or other commonly used password. Next box will ask for security questions and for your answers, (down arrow for choices). Page 2 of 8
Make sure you read Privacy Information and Terms of Service which can be reviewed anytime by clicking on the following links: Click here to read our privacy information and Click here to read complete terms of service, then click the two boxes after each Click here to acknowledge: receipt of Privacy Policy Information and Terms of Service. Then click Create account box (at bottom). You should see box, stating Profile updated successfully. Then click OK. You are then in the Patient profile screen to make necessary changes if needed. Then click Home. Clicking Home above should bring you to the patient portal, or you can type https://sballergyportal.gloclouds.com and sign in. Sign in box looks like this: Page 3 of 8

If the Favorites Bar ( Sidebar ) of the Mac is obscuring the left black sidebar, click on the little icon ( Show sidebar ) in the upper left part of screen (red arrow) and the black sidebar of the portal will appear. To return to the home screen from any screen, (PC or Mac) click on the Home tag or text (green arrows). To log out, scroll down from the right user name (in this case Mickey Mouse ) and click Logout (blue arrow) The rectangles ( Boxes ) on the top need explaining (see below): Health information - This allows you to look at a summary of your office visit, not the progress note itself. Click on Health Information box then click on View my health information. or tab at left A screen with a Box with a View My Health Information with flashing Magnifying glass (red line) will come up, click on that box (below): Page 5 of 8
A summary of your health care will come up. Appointments Box ( Read only ) allows patient to view upcoming appointments if they have one. It is NOT to make an appointment. It looks the same as Appointments Tab on the left. Messages Box or My Messages tab are not to send messages, only to review messages. Refill Request box is for requesting refills of prescription medications prescribed by your SBallergy, Inc. doctor. In general patients should be seen once a year in order to get a prescription refilled, and within a month for narcotics. Page 6 of 8
Please look at the tabs on the left sidebar dropdown menu, particularly the Send Message tab. The Send Message tab on left sidebar dropdown menu is to ask questions, request prescription refills, or request an appointment. The four actions are Ask a Provider, Ask a Staff, Refill Request, and Appointment Request. Ask a Staff tab is the same as Ask a Staff box. A message box will come up and you can ask your question: Page 7 of 8
1. Ask a Provider goes to a Medical Assistant (MA), receptionist, or nurse (LVN or RN) for screening before the question is sent to a provider. 2. Ask a Staff tab or Ask a Staff Box goes to a Medical Assistant (MA), receptionist, or nurse (LVN or RN) for screening and may be routed to a provider if appropriate. Ask a Staff or Ask a Provider functions are for established patients only for non-emergency situations. Ask a Provider messages all go to office staff first, NOT directly to provider. Please allow 2 business days for a reply. If you have a more urgent problem, call us at one of the telephone numbers in our letterhead on page 1. Remember, as in the Terms and Service agreement: User agrees not to send any messages requiring urgent attention. Delays can occur based upon volume, availability of staff and the complexity of your condition. If you need a response in less than two (2) business days please contact your clinician's office directly by telephone. Our regular business hours are Monday through Friday 9 a.m. to 5 p.m. For all urgent medical matters, contact your clinician's office or dial 911 if emergency attention is needed; 3. or Refill Request is for requesting refills of prescription medications prescribed by your South Bay Allergy and Asthma Group, Inc. doctor. The AppointmentRequest screen looks same as Ask a Staff and all other Send Messages tabs. The secure messaging (email) services that we offer are for established patients of doctors of South Bay Allergy and Asthma Group, Inc. Other persons who are not established patients in the practice may not use our secure messaging services for advice or to establish a doctor patient relationship with our doctors. This can only be done in a face-to-face visit in one of our offices with one of our physicians. When you are done, go to the user name in the upper right hand corner (in this case, Mickey Mouse ) pull down the dropdown menu and Log out.
