Making the Connections Transition Services Resource Guide Developed 2005 by OASCES Region 7 (Adapted for Worthington Schools) Making the Connections - A Transition Resource Guide This guide is designed to help parents, family members and students locate resources for future planning. The resources listed will assist students to make connections to services that may be needed during and after the high school years. It s never too early to start the transition planning process. By age 14, and every year thereafter, every student and his/her transition team should be discussing activities that promote movement from high school to post high school activities.

Pre-Employment & Employment Options Range of Employment Services Sheltered-A segregated setting and supervised work environment in which people with developmental disabilities receive pay for completed piece work. High level of personal and employment support provided. Enclave-A supervised community based work setting in which a person with a disability completes production work for the onsite employer through a contract. On the job supervision is medium to high. Supported-An individualized work placement for a person with a disability that offers job coaching as needed. Competitive-A job in the community that provides a competitive wage for industry standard work quality and quantity. Assistance in finding employment is possible but assistance in learning entry level job skills is minimal or not needed.
