1 Title of Immediate Supervisor: Assistant Director of Special Education Intermediate School District 917 Department: Special Education FLSA Status: Exempt Accountable For (Job Titles): Special Education Health Assistant Pay Grade Assignment: Teachers General Summary or Purpose Of Job: Supports a healthy learning environment for all students and staff. Provides a variety of services that promotes wellness, embraces healthy behaviors, encourages self-care, protects the well being of students and staff; provides for skilled health and nursing care and a safe and healthy building. DUTY NO. ESSENTIAL DUTIES: (These duties are a representative sample; position assignments may vary.) FRE- QUENCY 1. Performs and writes nursing assessments for students with health impairments. Provides mental and behavioral health care to students with mental health issues and disabilities. Reviews student mental health history, assesses current mental status, and provides mental health nursing care, as needed. Develops and implements mental and behavioral health plans. Provides referral information and recommendations to parents and guardians. Provides for crisis assessment, referral, and recommendations. 2. Provides and monitors the daily health care of all students in specific building or multiple-site programs. Reviews student health history, immunization records; reports annual immunization status to the Minnesota Department of Health. Assesses current status and provides first aid, routine nursing as medical management, as needed. Develops and implements health care plans. Provides referral information and recommendations, as appropriate, to students and to parents and guardians. Initiates and implements screenings for vision, hearing, height, and weight. Monitors immunization status. 3. Develops and maintains written records of daily care interventions, contacts with parents and guardians, agency and school staff, individual physical and mental health plans, emergency care plans, and special education health assessments. Utilizes this information to bill third-party payers for services provided. Maintains student medical records according to FERPA and HIPAA. Weekly 25% 22% 12.5%

2 4. Functions as a member of the school team with a focus on health and safety, crisis management, and quality improvement. Discusses, researches, plans, develops, presents and evaluates procedures and plans. Writes guidelines for health services, ensuring the delivery of such services within the school policy and nursing standards. Serves on school and District committees. Monitors, records and reports attendance and student abuse and neglect. 5. Develops and presents educational programs to staff and to students on health-related topics. Provides in-services to staff, in such areas as infection control and blood-borne pathogens, diapering and hygiene, personal care assistant services, medication administration and specialized health-care and emergency procedures; collaborates with them regarding student health problems. Teaches and certifies District staff and student in CPR and First Aid. Plans, develops and implements annual training to staff as required by OSHA and other agencies. 6. Monitors and supervises the health services office, as well as coordinating the services provided by the health assistant to the students in various programs. Performs related nursing services in the absence of the health assistant. Writes special education assessments, IHP s, and ECP s, as necessary. Attends student s due process meetings. Manages the health services office. Purchases supplies, equipment and resources. Orders, secures and arranges the delivery of supplies to program sites. Consults with the supervisor concerning budgetary plans. Maintains files and resources. Develops, produces, distributes and maintains forms and resource materials. Routinely cleans and disinfects the office and linens. 7. Provides occupational health services to staff. Monitors and provides health care and resources. Provides referral and recommendations for further assessment or treatment, if needed. Monitors and consults with supervisor concerning safety risks in the building or on the grounds. Monitors and consults with the supervisor concerning safety issues regarding the supervision of student and related procedures. 8. Performs other duties of a comparable level or type. Must be responsible for arriving at multiple worksites in accordance with District policy; this usually means driving between various work sites. 12.5% 10% 10% 8% As required Minimum Qualifications: (necessary qualifications to gain entry into the job not preferred or desirable qualifications) Requires a minimum of a baccalaureate degree in nursing and public health, and a minimum of two years of experience in nursing; or an equivalent combination of education, training and/or experience necessary to successfully perform the essential functions of the work. Prior experience in a pediatric setting or in childhood mental, emotional or behavioral health is preferred for some positions.

3 Certification or Licensing Requirements (prior to job entry): Must pass the Minnesota Board examination to become a registered nurse (RN). Must be licensed by the Minnesota Department of Education as a School Nurse. Must be certified in First Aid. Must be certified as a CPR Instructor. National certification in a related specialty care area is preferred. Knowledge Requirements: Requires knowledge of: Normal growth, development and health of youth. Common childhood illnesses and disorders. Medication management. Common nursing and health procedures, as well as First Aid and CPR. Normal emotional development of children and adolescents, mental health disorders of children, psychotropic medications, mental status examinations, and related basic intervention techniques. General familiarity with school board policies and health and safety, including emergency protocols and data privacy. OSHA standards, Department of Health and Human Services standards, and Department of Education standards, and reportable infectious diseases, as well as Employee Right To Know. Immunization laws. Special education services and the requirements of schools and health services; due process, IEP and ASR services. The nursing process, as in plan, develop, implement and evaluate. Familiarity with third-party billing practices for supervision, nursing and PCA. Skill Requirements: Skilled in: Basic health care and nursing procedures. Specialized health care procedures, including G-tubes, feeding pumps, Hoyer lifters, standers, wheelchairs, changing and positioning tables, venting, nebulizations, bronchial drainage, trach tube management, IV therapy, and blood glucose testing. Vision and hearing screening and referral procedures. Writing IHP s, ECP s (emergency care plans), reports, assessments, and communications. CPT (non-violent crisis intervention); behavior interventions and assessments. Preparing and presenting in-service education and educational materials to students and staff; using manikins for CPR training. In evaluating and developing evaluation tools. In working autonomously, assessing, prioritizing, and delegating. Use of MS-Word for writing health plans and procedures. Working with an interdisciplinary team; ability to work with multiple disciplines. Ability to be flexible regarding changing work demands. Planning and writing plans.

General Environmental Conditions: % Continuously Work is performed under normal office or classroom conditions, but there are some minimal environmental risks or disagreeable conditions associated with the work. The smelling physical requirement relates to alcohol or drug testing, as well as assessing personal hygiene issues. Occasionally, work may be performed in areas where airborne particles of mold and mildew are present; or, where there may be exposure to toxic or caustic chemicals such as bleach, disinfectants, nebulizing solutions, and some medications; or, where there may be some exposure to hazardous weather conditions when traveling between school sites. General Physical Conditions: Work can be generally characterized as: Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects. Occasionally, children need to be lifted.
