2 Logging in Open your Web Browser and enter the Following Web Address: Click on the Link for Remote Access: You will be brought to the Juniper Log in Screen enter your User name and login: 1 JTMMH

3 Once you are logged in Select the link for Physician Portal: 2 JTMMH

4 Home Page From this page you can search by a patient s: You Can Filter by: Encounter number (account number) The Patient s First name Provider: By checking the box you will only see patients with the doctor selected in the drop down The Patient s Last name By Date range: By checking the box and searching by a from and to date. 3 JTMMH

5 Patient Search To Search by a patient s Encounter number type the Encounter number in to the Encounter box. Then Select search. Note: Encounter numbers can be found on the hospital face sheet. On the Top left side of the page in the box marked ECD Number. To Search by a Patients name type the first or last name in to appropriate boxes. Then click Search. Note: you can put a partial spelling if the full name is not known. This will search for names that start with the characters that you have entered. 4 JTMMH

6 Filter: Filters are defaulted in based on your provider group and a 2 week date range if you chose to search without filters you will receive the following pop up reminding you of the HIPPA restrictions you agreed to when requesting a user login. If your search returns a large number of results you can use a Filter to narrow the Results. 5 JTMMH

7 Use a Provider Filter to Return Search Results only for a specific Provider. Check the Box for Provider and then select a provider from the Drop down by clicking the down arrow and then selecting a provider from a list. Then click Search again. This will limit your search results down to only encounters with that provider. 6 JTMMH

8 You can also filter a long list of results by a date range. Make sure the box is check next to Date Range and then select a from date. This should be the earliest date you want to see records for. You can type in the date using a MM/DD/YYYY format or Select the date on the calendar pop up in the From field. Repeat this with the to field. To should be the latest date you want to see records for. Then click search. This will narrow the Search down to the date range selected. As with any search the more information you put in and the more filters you use the narrower your search will become. 7 JTMMH

9 Viewing and Printing a Patient s Record: Once you have located the desired patient select the Encounter number next to the patient s name: A new page will appear The Patient s demographic information will display at the top with a button for Insurance, Contacts, and Results. To expand the information in any section, click on the name of that section to expand it. 8 JTMMH

10 To print any patient information, click on the link that says print in that section. Then follow the prompt to select your printer. And then select Print. 9 JTMMH

11 Under Result s, make sure anything you would like to print is checked and anything you do not want to print is uncheck and then select the print button and follow the same steps as above. 10 JTMMH
