1 Baltimore Tennis Patrons Spring 2013 Registration Form Pee Wee, Junior, Adult and Family Tennis Classes, Practices and Play Clubs Class Dates: April 22 May 19 Class Rain Make-Up Dates: May Standard loaner racquets and QuickStart reduced size racquets available for all classes Pedometers available for Junior Cardio class PROGRAM DESCRIPTIONS: QuickStart (QS) for Pee Wees and Juniors: Finally a court size, scoring system and equipment that fits! With the QuickStart Tennis Play format, the court size, the racquet size, the balls, the net height and even the scoring system have all been adjusted to match kids age 4-8. It s tailor made! BTP provides QuickStart reduced size loaner racquets for all practices. BTP coaches are all USTA QuickStart specialists. Practice sessions include a warm-up activity, a new skill, a team game, a cool down and a review. QuickStart (QS) and Cardio: Two blockbuster ways to introduce your child to tennis are Cardio & QuickStart! Each practice starts with heart rocking fun featuring an action-based group warm-up, cardio and cool down followed by QuickStart. QuickStart Tennis is a play format to help kids learn and play the games effortlessly with reduced size equipment and user-friendly scoring and games Beginner Tennis: Learn to Play Fast! Featuring the Games Based Approach to learning the basics in play situations from the get-go. Instant fun, learning, exercise and success! Advanced Beginner Tennis: Improve Fast! Featuring the Games Based Approach to applying the fundamentals in play situations. Instant fun, improvement and exercise. Beginner/Adv. Beginner Tennis: Learn and Improve Fast! Family classes-adults and children take their lesson at the same time but on different courts. Adults are grouped together and kids are grouped together on separate courts. 4 How to Sign Up MAIL IT IN Complete the Registration Form in this and send it to the address noted on the bottom of the registration form. Please provide all information on the form to ensure prompt processing. FAX YOUR REGISTRATION You may fax your completed Registration form to the Baltimore Tennis Patrons office at , using VISA, Master Card or Discover. Please provide all information on the form to ensure prompt processing. *Registrations of all kinds (mail, , in-person and/or fax) will be processed in the order that they are received. IT IN Fully completed registration forms, including your credit card number and expiration date as payment, can be ed to Submitting a registration form without payment will not secure a spot in the class. REFUND POLICY Requests for full class refunds must be received by April 12, Class credits may be given for withdrawal requests made after that date at the discretion of the Baltimore Tennis Patrons office. Credit must be requested within 30 days after the last scheduled class. No class refunds will be issued after two classes. To withdraw from a class, you must contact tennis@tennispatrons.org. REGISTRATION We process registrations according to the order in which we receive them. Confirmation notices are not mailed to program registrants. **Registration deadline is April 12, CONNECT WITH US Join BTP list: Twitter: Facebook Fan Page: Tennis-Patrons/ Website: CANCELLATION POLICY Baltimore Tennis Patrons reserves the right to cancel any course at their discretion. In case of cancellation, a full refund will be given. INCLEMENT WEATHER POLICY Because of the large geographical area covered by these programs, cancellation of classes or practices due to inclement weather is at the sole discretion of the Head Coach of each class. Head coaches are required to report to all classes regardless of weather conditions or forecasts and determine whether or not courts are suitable for play. We encourage all participants to attend each class, especially if the weather is questionable. Participants do not receive phone calls for cancellations. Credits may be issued upon request for classes rained out beyond 20% of the total classes scheduled without make-up opportunity. FINANCIAL ASSISTANCE Baltimore Tennis Patrons can provide a limited number of full and partial scholarships based on need. Please call for more information. LOANER EQUIPMENT AVAILABLE Head coaches will have loaner racquets available for all programs and pedometers available for cardio for kids classes. STILL HAVE QUESTIONS? Contact the Baltimore Tennis Patrons office at or tennis@tennispatrons.org. OOPS!!! Your class, practice or workshop may be cancelled due to low enrollment!! Sometimes great programs with excellent instructors are cancelled when too many people put off registering. We need a minimum number of participants before a class will run. Don t wait register today!!

Authorization for Use of Photographic/Video Likeness: By/for Participant, it is agreed that BTP may take and utilize photographic/video images of the Participant for the purposes of promoting and publicizing BTP programs and/or events. In case of a preference not to allow such images to be so taken and utilized, call to register such request.
