3 THE CALIFORNIA STATE UNIVERSITY ANNUAL REPORT OF EXEMPTIONS FROM THE BIDDING PROCESS DUE TO UNEXPECTED CAUSES January 1, 2013 through December 31, 2013 CAMPUS CONTRACTOR CONTRACT# DATE AMOUNT NATURE OF EMERGENCY Bakersfield Channel Islands Chico Dominguez Hills East Bay Fresno Oilfield Electric Company, Ventura /23/ , Gas Control Technologies /13/ , Remove four power poles and wiring damaged by the spring fire. The fire occurred 5/2/2013 and surrounded the CI campus, parts of Camarillo, Newbury Park, Thousand Oaks, and Malibu all the way to the ocean. The campus and University Glen housing development were evacuated for four days. Replace natural gas main pipe in parking lots G and G9 due to old and rotten pipes causing leakage. Power Systems Testing /31/2013 $ 65, Campuswide power outage due to non-functioning substation circuit breaker in the Plant Operations main power substation. Work included parts and labor to replace with new 2000 amp breaker. Graham Prewett, Inc /12/2013 $ 143, Roof replacement due to leak at Family Food Science Building (#8) Floyd Johnston Construction Company, Inc /22/2013 $ 38, Davis Moreno Construction, Inc /23/2013 $ 10, Electric Motor Shop & Supply /2/2013 $ 6, Agilent Technologies /26/2013 $ 9, Power Systems Testing /21/2013 $ 11, Floyd Johnstone Construction, Inc /19/2013 $ 13, Fullerton Millans Restoration, Inc /5/2013 $ 13, Humboldt Danco Builders Northwest MY 8/22/2013 $ 57, Los Angeles Total Environmental Management, Inc /6/2013 $ 191, Southern California Trane Service /17/2013 $ 126, Leaking hot water pipes near McLane Hall Building (#6) causing ground saturation, no heat in buildings, and chemical waste. Work included material and labor to remove existing corroded material and replace with new pipe and valves. At the Bulldog Ticket Office, a leak in 6" waterline caused flooding. Work included new fire hydrant installed and seven tons asphalt replaced. Variable Frequency Drive (VFD) critical failure on Chiller #1 in the Plant Operations main power substation fan affecting building temperatures campuswide. Work included inspection and installation of new VFD motor and 600V fuses. Existing motor unrepairable. Replace PTV inlet of Agilent mass spectrometer in Science 1 Building (#16) due to time sensitive analysis of environmental samples that cannot be re-collected. Transformer in the chilling system in the Plant Operations yard responsible for cooling 50 percent of campus malfunctioned; needed testing and repair. Underground hot water supply and return piping leaking near McKee Fisk (#9) and McLane Hall (#6) causing ground saturation, HVAC loss of operation, and chemical waste. Work included removal of existing corroded pipe and valves and replace with new 6" and 10" ductile iron hot water pipe and valves with insulated and protective covering. Flood in 12 student dorm rooms and other areas in Pine Residence Hall caused by student tampering with the fire sprinkler head. Work included extracting water, installing air movers, removal of drywall and ceiling tiles, detached base, block and pad furnitures in rooms. Repair damaged wooden truss due to age of the beam, seismic activity, or a combination in Fulkerson Hall in Music Building A (#8). Facilities Management worked with consulting engineer and a local contracting firm to design, fabricate and install parts required to complete the repair plan, which reinforced and properly strengthened the truss beam so that it is structurally and seismically sound. This plan was reviewed and approved by the CSU Seismic Peer Review. Every effort was made to minimize disruptions to the academic program. Chiller failed in South Chiller Plant (#16) and left campus without air conditioning. Due to temperatures, there was a concern for health and wellness. An emergency chiller, transformer and generator were rented to carry the campus during repairs. Chiller failed due to a seized capacity vane controller, multiple condenser leaks and rust-contaminated refrigerant in Chiller Plant (#16) and left campus without air conditioning. Due to temperatures, there was a concern for health and wellness. The chiller was taken apart, repaired and cleaned. 3/26/2014 Page 1

The track surface restoration mitigated the deficiencies identified by EH&S. Emergency repair of a break in the heating hot water line on East Campus Drive that serves the Microbiology Building (#41). Restore power lost to High Voltage Feeder 3 at the Electrical Substation (#191). Electricity is required to maintain legally-required fire/life safety systems, lighting, refrigeration, fume hoods, exhaust fans & ventilation. Refrigeration is required for teaching labs and perishables. Exterior lighting is required for public safety. Emergency required immediate remedial measures to protect property and/or the public's health, safety and welfare. Emergency replacement of failed compressor on chiller at Montecito Hall Building (#21). Service to restore air conditioning on 7/15/2013 in the builidng (approximately 220 occupants). Due to the excessive heat (temperatures in the 90s), this was a life safety concern. SBCMP /5/2013 $ 11, CE Wilson Corporation /7/2013 $ 20, CE Wilson Corporation /7/2013 $ 16, CE Wilson Corporation /14/2013 $ 12, Wade Company /29/2013 $ 12, Belfor USA Group, Inc* 1/30/2013 $ 100, Water leak in food service area of University Union Building (#47) due to broken pipe. The service included both remediation of the area and the repair of the pipe and rebuilding the area. Replace high wall split air conditioning system located in Telecom Room of the University Police Building (#4B). Replacement necessary to keep equipment from overheating and to keep 911 Telecom system in operation for campus police department and two offsite college police departments. Locate, excavate and repair broken water line outside the Performing Arts Building (#20). The accumulation of water was creating a safety hazard. Repair fire sprinkler dry standpipe system in Parking Structure 1 (#55) due to west side not holding pressure. Excavate and replace leaking 6" underground dry standpipe. Repair Adams Humanites (#58) water main break due to abandoned building domestic water shut-off valve and paved over five feet below surface. Chilled water main leak in roadway at Scripps Terraces. Excavation of street north of Exercise and Nutritional Sciences Annex (#20) for repairs on chilled water valves. Imperial Valley Campus roof repair due to foot palm tree blown over and crashed into the west side of the Administration Building. Replaced broken fire hose cabinet coupling that caused the flooding, de-watered walls, floors and ceilings and replaced water damaged materials in Creative Arts Building (#7). Replaced Computer Room Air Conditioning Unit in the Health Center Building (#27) that failed to protect student medical record servers. American Air Conditioning, Plumbing & Heating /27/2013 $ 8, Belfor USA Group, Inc* /30/2013 $ 100, Rekey University Park North/Housing (#100) due to theft of master keys Belfor USA Group, Inc* /23/2013 $ 100, Campus electrical outage in Centennial Village A (#23A), Centennial Village B (#23B), Centennial Village C (#23C), Student Services Building (#23D), Corporation Yard (#25), Central Plant (#26), Waste Management (#26A), Residence Dining Center (#29), Mary Ward Hall (#91), and Mary Park Hall (#92). Students returned and to avoid blockage issues, main sewer lines at various locations were cleaned and video assessed. Miksis Services /27/2013 $ 6, Redwood Empire Disposal /12/2013 $ 35, Garbage and recycling services due to break down of campus truck. Leed Mechanical /16/2013 $ 24, ThyssenKrupp Elevator PO /23/2013 $ 31, Costco Fire Protection PO /7/2013 $ 21, RF MacDonald PO /11/2013 $ 58, Costco Fire Protection PO /3/2013 $ 46, Thyssen Krupp Elevator PO /11/2013 $ 106, Remove and install heat exchanger in Carson Hall (#8) and Nichols Hall (#9) to correct temperatures of 60 degrees or less. Elevator valves were failing and needed to be replaced in atrium elevators of Mary Stuart Rogers Gateway Building (#27). Original fire pump in Mary Stuart Rogers Gateway Building (#27) failed, necessitating emergency replacement to maintain fire protection in the building. Replace 30-year-old pool boiler east of the Fieldhouse, Pool Facility (#40) due to failure and inability to repair. Heated pool required for classes. Campuswide fire sprinkler repairs needed to correct code deficiencies in critical life-safety sytems as ordered by State Fire Marshal. Demergasso Bava Hall (#21) elevator went down due to oil leaks from jack. Cylinder had to be replaced immediately as it is the only way to provide accessibility to 3rd floor of south wing.
