3 CMHA Services CMHA is committed to providing quality service to residents who are seeking assistance. Our offices in the Stokes Mall are open from 8 a.m. to 5 p.m., Monday though Friday. 4 CMHA Services Computer Lab/Resource Room Suites 2560 & a.m. to 4:30 p.m. The computer lab at the Social Services Mall provides Internet capability and access to computer related training to residents with an emphasis on pre vocational skills. Pre vocational skills provide the foundation needed to be successful in educational and employment related endeavors. The computer lab assists residents in obtaining economic selfsufficiency through the provision of computer training and assists residents in increasing their job search skills. Skills such as resume writing and Internet job search are provided to adults and to older youth. Through the computer lab, younger youth are provided with after school programming to reduce opportunities to participate in atrisk behaviors and deliver programs that will not only provide students with basic computer skills, but also stimulate and expose them to careers in computer technology and assist them in transitioning to employment and post secondary education. 4

5 Beech Brook Suite 2260 (216) Debra Brown, Supervisor Beech Brook provides a full range of treatment, education and support to help troubled or at risk children, youth and their families. Whether the need is for residential treatment, foster care or adoption, outpatient therapy, early childhood programs, or school based mental health services, Beech Brook will be here, helping children and strengthening families. Beech Brook s services include: Day treatment Foster care Mentoring Family preservation Outpatient counseling Community based programs Parent support and education Residential and intensive treatment Adoption for children with special needs School based mental health services Stop by during our Drop-In hours, every Monday through Friday from 9:30 a.m. to 4:30 p.m. We also have Monday evening hours from 6:30 p.m. to 8:30 p.m. 5

6 Bellefaire JCB Suite 2261 (216) (intake dept.) Bellefaire JCB is an innovative organization that provides exceptional care, education, and advocacy to enhance the emotional, physical and intellectual well being of children, young adults and families in the Jewish and general communities. 6 Bellefaire JCB provides the following services to CMHA residents: In Home Services for Parents and Children Together (PACT): Using family therapy and individual counseling interventions, case management and wraparound services, this program preserves the family by teaching new behaviors in the home. Child and Family Counseling: Individual and family counseling services are available for those experiencing a wide range of difficulties, including depression, anger management, and adoption issues, Help is also available in dealing with trauma, loss, and family violence.

7 Brother Hubbard s Cupboard Suite 1529 (216) Brother Walt Robb, Director Brother Hubbard s Cupboard is a weekly food distribution service run by the Mission of Catholic Charities and the Capuchin Franciscans, dedicated to providing food to those in need. Since its inception in 2004, the pantry has grown tremendously and has distributed more than 500,000 pounds of food annually for the last four years. BHC has over 4,500 families registered and feeds over 850 families every month. BHC also has a home delivery service to the elderly, which takes place on the first, second and third Tuesday of each month. Brother Hubbard s Cupboard is open every Friday from 8:30 a.m. to 11:30 a.m., with the exception of Christmas, Thanksgiving, New Year s and Good Friday. An ID is required, as is proof of residency if claiming dependency. 7

8 Care Alliance Main 2nd Floor (216) Francis Afram-Gyening, CEO Care Alliance Health Center is the leading provider of primary health care and comprehensive dental services to individuals and families who are experiencing homelessness, living in public housing, and uninsured or underinsured in Cleveland. Care Alliance has both a medical clinic and a dental clinic on site, as well as a podiatrist, Dr. Dwight E. Scott, D.P.M. Care Alliance provides services such as: Primary Health Care for all ages Comprehensive Dental Care Podiatric and foot care services for ages 6 months and up Short Term Substance Abuse & Mental Health Counseling Confidential HIV Testing, Treatment and Counseling Medical Case Management Services Health Education Programming Women s Health Services Cancer Screenings Diabetes Clinic Medical Clinic Hours: Monday - Friday: 9 a.m p.m. and 1 p.m. - 5 p.m. 8 Dr. Scott (Podiatrist) Hours: Monday, Tuesday: 9 a.m. - 5 p.m. Wednesday: 10 a.m. - 2 p.m. Friday: 12 p.m. - 5 p.m. Dental Clinic Hours Monday, Tuesday, Thursday: 8:30 a.m. - 4:30 p.m. Wednesday 8:30 a.m. - 6 p.m. Friday 8:30 a.m. - 3 p.m.

9 Cleveland Municipal Courts Adult Probation Department Suite 2820 (216) Susan Little, Probation Supervisor The mission of the Cleveland Municipal Court Probation Department is to provide protection to the community and enhance the quality of life for those on probation by professionally enforcing the orders of the Court and holding offenders accountable for violations of the law and conditions of probation. The department s priority is to ensure that adult probationers adhere to the orders of the court. The department also offers a resource to probationers seeking assistance. The Cleveland Municipal Court Probation Department is open on Mondays through Fridays from 8:30 a.m. to 4 p.m. The department also stays open until 6 p.m. on every first and third Wednesday of the month. 9

10 Cleveland Treatment Center s Project S.A.F.E. Suite 2121 (216) William Richards & Jerome Littlejohn, Project Managers Project S.A.F.E. (Staying AIDS Free through Education) provides education and prevention in the field of HIV/AIDS. Through Project S.A.F.E., residents receive HIV testing, pre and post HIV/AIDS counseling, and referral services to drug treatment programs, STD clinics and community based organizations. Outreach teams provide health education and risk reduction interventions to hardcore substance abusers and their sexual partners, heterosexuals and at risk youth in the Greater Cleveland area. The educational services that Project S.A.F.E. offers focus on: methods of transmission, self esteem, abstinence, risk reduction, public awareness, and negotiating skills Project S.A.F.E. also conducts street and community outreach, preand post HIV testing counseling, referrals, condoms, and group counseling services. Project S.A.F.E. is open every Monday through Friday from 8:30 a.m. to 5 p.m. 10

11 DJ s Boutique 1st Floor (216) Commissioner Doris V. Jones DJ s Boutique is managed by CMHA s very own Commissioner Doris V. Jones, the Vice Chairperson of the CMHA Board of Commissioners. The boutique offers free new or gently used clothing, bedding, household items, shoes and even toiletries to both the general public and CMHA residents. 11

12 Progressive Action Council 2nd Floor (216) Lillian Davis, President The Progressive Action Council (PAC) is one of CMHA s most important and committed partners. The PAC is recognized by CMHA as the Authority wide residents organization. The residents of CMHA elect delegates to PAC, who in turn, serve as their resource for information, as well as an advocate when residents want to voice opinions regarding their homes at CMHA. PAC also oversees elections for resident leaders of the Local Advisory Councils (LAC) at each CMHA property. All residents are considered part of their development s LAC, and vote to elect their own officers. 12

13 Salvation Army Railton House Transitional Housing for Men 7th Floor (216) Rob Lax, Program Manager The Salvation Army s Railton House provides transitional housing, case management and aftercare services for homeless men who require additional support on the way to permanent housing. Program components are comprised of: assessment and intake, individual and group counseling, peer support and intervention, recreation and socialization. Overnight program monitors are trained and available to, among other things, provide late night crisis intervention. Railton House also provides residents with direct assistance in obtaining permanent housing. Residents who effectively complete the transitional housing program at Railton House typically have earned a GED, completed college or vocational training, regained and/or maintained their sobriety, secured gainful employment, re established family relationships and acquired permanent housing. 13

14 Y-Haven 4th, 6th and 8th Floors (216) Chip Joseph, Director Y Haven II 6 th Floor (mailbox 703) This is a drug rehabilitation program with the primary focus on substance abuse treatment and drug rehab. Y Haven II specializes in Residential long term treatment (more than 30 days) Long Term Substance Abuse Treatment and Long Term Drug Rehab. Y Haven III 4 th Floor (mailbox 703) For homeless men 18 years old and older, Y Haven assists with gaining self sufficiency and permanent housing. Therefore, the program provides transitional housing, treatment for substance abuse and mental illness, educational training and vocational assistance to homeless men. Y Haven Open Door 8 th Floor (mailbox 703) The YMCA s Y Haven Open Door program is a partnership with Center for Families and Children and Cuyahoga County s Office of Re entry. This collaboration offers formerly incarcerated individuals comprehensive case management, transitional housing, permanent housing placement with CMHA and wrap around services in a safe housing unit. Y Haven Open Door s program goal is for the participant to gain the necessary skills that will aid in ameliorating antisocial attitudes and behavior. 14

15 Youthbuild Room The Youthbuild program is an economic self sufficiency program that provides job training and educational activities to at risk youth who, as part of their training, help construct or rehabilitate housing for low income or homeless individuals and families in their respective communities. Youthbuild assists residents age 18 to 24 who have not completed high school. The five key focuses of Youthbuild are education, occupational skills training, community service learning, leadership development, and transition into employment or post secondary education. The CMHA Youthbuild program provides integration of classroom academics and occupational skills training in partnership with Cuyahoga Community College. Through this partnership a combination of classroom lecture and hands on practice with the basics of electrical wiring, carpentry, plumbing, demolition, and abatement principles is provided. 15

16 Boys & Girls Club 2561 East 59th Street Cleveland, OH (216) The King Kennedy location of the Boys & Girls Club of Cleveland (BGCC) is just a short walk away from the Carl B. Stokes Mall. BGCC exists to provide children who are most in need with a safe place to grow, learn and realize their full potential, as they will become tomorrow s parents, workforce and leaders. With a wide range of programs, it supports healthy lifestyles, academic success, character building and leadership development. The Boys and Girls Club of Cleveland is open after school Monday through Friday. Please call for additional information on hours. 16

17 Golden Age Center 2501 E. 59th Street Cleveland, OH 4104 (216) The King Kennedy Golden Age Center is a supportive housing center located on the second floor of the King Kennedy North High Rise building. On site services include professionally facilitated programs covering a variety of subjects. Programs are designed to educate, empower, entertain and assist in preserving independence of GAC seniors. GAC specializes in areas of recreation and socialization, nutrition, and other supportive services for seniors, including: Programs for seniors who reside in and around the center that provide a daily hot and nutritionally balanced meal Access to individual counseling on areas such as independent living, consumer knowledge, finances, housing, and benefits Counseling is available between 8 a.m. and 4:30 p.m. Regular home cooked breakfasts and monthly lunch bunch outings for GAC participants Trips to other GAC s for bingo, exercise, and other activities 17
