1 FOR IMMEDIATE RELEASE October 9, 2013 Contact: Denise San Antonio Zeman President and CEO Saint Luke s Foundation (216) dzeman@saintlukesfoundation.org SAINT LUKE S FOUNDATION EXCEEDS $100 MILLION IN CUMULATIVE GRANT PAYMENTS WITH SECOND CYCLE 2013 GRANTS AWARDS Grants Intended to Promote Health and Well-Being in Greater Cleveland CLEVELAND The board of trustees of Saint Luke s Foundation has approved its second cycle grants for 2013, awarding a total of $2,948,000 to 16 organizations that advance the Foundation s strategic priorities focusing on Healthy People, Strong Communities and Resilient Families. With these second cycle 2013 grants, the Foundation has now exceeded $100 million in cumulative grant payments since it was established in We are extremely proud to cross this significant grantmaking milestone, said Denise San Antonio Zeman, Saint Luke s Foundation President and CEO. Achieving this milestone was made possible through the vision, commitment and leadership of our board, the dedication of our talented staff, and the transformative work performed by our grantees during our first 15 years of grantmaking. Our mission to improve and transform health and well-being throughout Greater Cleveland is stronger than ever, and we look forward to making an even greater impact in 2014 and beyond. This grant cycle is the second of the Foundation s three annual grant cycles. The full list of second cycle grants is noted below, by program strategy area:

2 PAGE TWO HEALTHY PEOPLE Beech Brook: $75,000 over 15 months for Strategy Counts, an initiative that seeks to establish an integrated care environment addressing the physical and behavioral health needs of system-involved high risk children and youth. Established in 1852, Beech Brook provides mental health counseling, residential treatment and community-based services to support the well-being of children and their families. Cuyahoga Health Access Partnership (CHAP): $75,000 to support a dedicated health navigator program that seeks to build capacity in the community health center system, assist providers to prepare for the transition to Medicaid expansion, ensure that qualifying individuals are enrolled in appropriate coverage and develop solutions such as access to dental care, prescription assistance and behavioral health. Diabetes Partnership of Cleveland: $50,000 for operating support for a diabetes education pilot program on healthy living within two federally qualified health centers: Care Alliance and The Free Clinic. Forces4Quality Northeast Ohio (dba Better Health Greater Cleveland): $750,000 over three years for operating support and capacity building support for financial management. Better Health Greater Cleveland is an independent nonprofit organization dedicated to improving the health of people in Northeast Ohio living with chronic disease. Universal Health Care Action Network of Ohio (UHCAN Ohio): $85,000 to support citizen advocacy initiatives around the State of Ohio s healthcare exchange navigation system. UHCAN Ohio is a statewide organization engaged in policy work to achieve high-quality, accessible, affordable health care for all Ohioans. STRONG COMMUNITIES EDWINS Restaurant and Leadership Institute: $65,000 to launch the EDWINS Restaurant and Leadership Institute. EDWINS, an acronym for education wins, is a nonprofit

3 PAGE THREE entity with aspirations of developing a new generation of leaders in the culinary industry by training returning ex-offenders for long-term, successful careers in fine dining restaurants. Having piloted the culinary arts training program in an Ohio prison and the Northeast Ohio Pre-release Center in Cleveland, EDWINS is working to launch the second phase of its restaurant and leadership institute - a restaurant located in Shaker Square that will allow participants the opportunity to train and use the skills they learn to operate a fine dining restaurant. Friends of Breakthrough Schools: $400,000 to support the charter management organization s new school development and expansion initiative. Specifically, this grant will be used to complete renovations in currently uninhabited wings of a former Cleveland Metropolitan School building located at 93rd and Union, as well as continuing to update classrooms, technology and security enhancements in the building. PolicyBridge: $100,000 to support its policy, advocacy and capacity agenda. PolicyBridge is a nonprofit organization that has provided public policy research, analysis and evaluation as a public policy "think tank" since its creation in Senior Transportation Connection of Cuyahoga County: $32,000 for capital support related to a vehicle purchase. The Senior Transportation Connection of Cuyahoga County was established in 2005 to create a coordinated transportation system for older and disabled adults needing transportation services. This transportation network has developed an efficient and cost-effective way to provide services to seniors needing access to medical and other appointments throughout the County. RESILIENT FAMILIES Centers for Families and Children (CFC): $150,000 for operating support. CFC provides an array of human services throughout Cleveland and its inner-ring suburbs, with core services in early learning, behavioral health, youth and family services, and workforce development. The Centers has a long reach, serving more than 20,000 people each year at 17 locations.

4 PAGE FOUR Cleveland Housing Network, Inc.: $500,000 over three years to support Resident Success & Housing Stability, an initiative designed to stabilize housing and help families increase their financial assets. Cleveland Housing Network (CHN) is a 32 year-old organization formed to address lack of quality, affordable housing for low-income families in Cleveland. Ohio IAF (Greater Cleveland Congregations (GCC)): $40,000 for operating support and development of an evaluation plan. GCC is a coalition of 40 religious congregations and community organizations working together to make the Greater Cleveland region a more just and prosperous place. The Foundation s grant will be used to support GCC s work in the education arena, specifically its Parent Organizing Initiative, which seeks to increase parental engagement and establish a strong parent voice district wide that can advocate for changes in the education system to better meet the needs of children and families. Partnership For A Safer Cleveland: $131,000 to support the STANCE Police Assisted Referrals (PAR) program, which creates a new point of entry for families to access needed services and resources, including crisis intervention services and the Children Who Witness Violence program. In recent years, the program has expanded to include access to anger management, parenting supports, and employability skills services. The partnership was established in order to develop and promote effective violence prevention efforts in Greater Cleveland through collaborations with public and private partners, from justice system leaders and law enforcement, to schools, businesses and community groups. In addition to grants targeting the Foundation s specific priorities, the board awarded the following grants under its Strengthening the Nonprofit/Philanthropic Sector grantmaking category: Cleveland Leadership Center (CLC): $20,000 to support tuition scholarships for nonprofit leaders to attend CLC programming as part of its Strengthening and Building Nonprofit Leadership initiative. CLC is a locally based organization dedicated to building a continuum of civic leaders committed to the community's excellence by serving as a catalyst for civic engagement.

5 PAGE FIVE The Center for Community Solutions (CCS): $450,000 over three years for general operating support in order to strengthen its capacity to provide critical decision support materials to the community. During the term of this grant, CCS proposes to produce a series of reports on key human service systems including aging, developmental disabilities, TANF, child and family welfare, public health and mental health. Additionally, the Foundation awarded a $25,000 grant to Neighborhood Progress, Inc. which will be applied to the Friends of Saint Luke s Capital Campaign for the naming opportunity for the Outdoor Garden at Saint Luke s Pointe. The garden will be named in memory of Francis H. Beam, Jr., Founding Chair of the Saint Luke's Foundation board of trustees. During 2011 and 2012, the Foundation reinvented its grantmaking approach in order to provide larger and longer-duration grants that make measurable impacts while also tying grant decisions directly to mission-driven criteria and outcomes in our three strategic program areas, San Antonio Zeman added. These second cycle 2013 grants are reflective of that approach, as well as our commitment to improving health and well-being throughout the communities that our grantees serve.
