2 2 Novant Health Rehabilitation Center 2013 report card Novant Health Rehabilitation Center is a dedicated inpatient rehabilitation facility located on the campus of Novant Health Forsyth Medical Center. Our team of rehabilitation experts is dedicated to providing a full range of rehabilitation therapies in a safe and easy to access facility. Using intensive therapies and skilled nursing under physician guidance, we help our patients recover, regain strength and return to independence quickly. Decisions related to admission, transfer, and discharges are based upon the patient s needs and the ability of Novant Health Rehabilitation Center to meet such needs. To be admitted as an inpatient, a patient must: Be at least 18 years old and able to breathe without a ventilator Require daily physician medical management and 24 hour nursing care and physician availability Be able to participate in 3 non-consecutive hours of therapy per day We have excellent outpatient clinics available for the following: Audiology testing and hearing aids for infants, children and adults Lymphedema Driving rehabilitation Wheelchair and seating Low vision The adult inpatient medical rehabilitation and stroke specialty programs at Novant Health Rehabilitation Center received their renewed three-year accreditation from the Commission on Accreditation of Rehabilitation Facilities (CARF) in NovantHealth.org
3 3 Novant Health Rehabilitation Center 2013 report card Clinical achievements Discharged 746 patients 96% of all patients showed improvements in Functional Independence Measure (FIM) scores from admission to discharge Surpassed the 2013 national average for percent of patients discharged to the community by 3% for all patients as measured by erehabdata on 01/07/ Surpassed the national average for stroke patients by 11% - Surpassed the national average for general rehab for medical patients by 11% More than 4 out of 5 surveyed discharged patients would recommend the facility to family and friends on a National Research Corporation (NRC) Picker survey.
