Clinical Psychologist /Neuropsychologist June 2010-June 2012 Fountain Hills Family Practice, PC Fountain Hills, Arizona Description of Site: Integrated family practice consisting of family medicine physicians, physician assistants, hospitalists, clinical psychologist, and several rotating medical specialties (neurology, urology, rheumatology, etc.) located within a medical campus, which also includes a laboratory, medical imaging, dermatology, rehabilitation therapy clinic, dentistry, cardiology, and orthopedics. Also provided psychological services within nearby skilled nursing, assisted, and independent living facilities. Responsibilities: Provide comprehensive psychological services to a predominately older adult population in the areas of neuropsychology, geropsychology, medical psychology, and behavioral health including comprehensive neuropsychological and dementia evaluations, individual, couple, family, and caregiver psychotherapy, and behavioral health interventions. Provide consultation to family members, caregivers, referring physicians, and other treating providers. Training and supervision of pre-doctoral practicum students. Community outreach including participation in local support groups and National Memory Screening Day. 40 hours per week. Comprehensive services include physical, occupational, speech, and recreational therapies, case management, social work, nursing, and neuropsychological services. Clients are referred by neurologists, physicians, attorneys, workers compensation, and state and government organizations. Responsibilities: Administer, score, and interpret various neuropsychological assessment measures and write neuropsychological evaluations. Develop, implement, and update neurorehabilitation treatment plans. Conduct individual, family, and group psychotherapy, as well as cognitive rehabilitation. Provide consultation to patients, third party payers, referring physicians, and other treating providers. Participate in case conferences as member of interdisciplinary treatment team. Comprehensive services include physical, occupational, and speech therapies, case management, nursing, and neuropsychological services. Clients are referred by neurologists, physicians, attorneys, and state and government organizations. Responsibilities: Administer, score, and interpret various neuropsychological assessment measures and write neuropsychological evaluations for children, adolescents, and adults. Provide feedback and results to clients, caregivers, and referral sources. Provide long and short-term individual psychotherapy, as well as group therapy. Work within an interdisciplinary treatment team. Collaborate on research projects presented at national conferences. 40 hours per week. Diamond, PhD Description of Site: Dr. Ellen Diamond s practice provides psychotherapy and psychological assessment services to children, adolescents, adults, families, and couples within an outpatient private practice setting. Clients suffer from a variety conditions including depression, anxiety, ADHD, Asperger s, and personality disorders. Services are also provided within extended care facilities and rehabilitation centers including brief evaluations and follow-up psychotherapy with medical inpatients. Clients present with various medical conditions including dementia, stroke, paralysis, and cancer. Clients are referred by primary care physicians, area schools, and care facilities. Responsibilities: Provide individual psychotherapy to clients including children, adolescents, and adults. Administer, score, and interpret psychological assessment measures and write psychological evaluations for children, adolescents, and adults. Perform brief neuropsychological evaluations for both elderly and adult populations within short and long-term skilled nursing facilities and rehabilitation centers. Provide individual follow-up psychotherapy for inpatients at extended care facilities. 20 hours per week. Behavioral medicine department provides individual and group psychotherapy to inpatients on the hospital s psychiatric unit, as well as intensive outpatient programs. Psychological services are also provided to medical inpatients in the various departments of the hospitals and extended care facilities including brief mental status and depression evaluations, full neuropsychological evaluations, and brief follow-up psychotherapy. Responsibilities: Administer, score, and interpret assessments and write neuropsychological evaluations for both medical and psychiatric patients within the hospitals and rehabilitation center. Evaluate patients with medical conditions including dementia, delirium, stroke, and brain injury and patients with psychiatric

Co-lead outpatient psychotherapy groups for elderly individuals suffering from anxiety, depression, and comorbid medical conditions. Co-lead inpatient psychotherapy group for inpatients on the psychiatric unit of the hospital. Perform brief psychotherapy with medical inpatients within the hospitals and rehabilitation center. Work in interdisciplinary team including physicians, psychiatrists, nurses, social workers, and physical, occupational, and speech therapists. 20 hours per week.
