1 Program Facts Program Facts The Program provides injury/illness-specific programs that offer patients care needed to regain the abilities to perform daily tasks, restore basic life skills, reclaim cognitive and physical functioning and facilitate psycho-social adjustment. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 358 individuals between January 2011 and December CY11 Impairment Group Distribution by Volume Multiple Trauma, 23 Spinal Cord, 42 Debility, 27 Ortho, 60 Amputee, 26 Cardiac, 5 Neuro, 31 Stroke, 71 Other, 13 Other, 7, Pulmonary, 1 Brain, 62 Stroke Brain Neuro Ortho Multiple Trauma Spinal Cord Debility Amputee Cardiac Other Pulmonary UWHC patient vs. national average rehabilitation statistics: Age 53 years 69 years Length of Stay 16 days 13 days Functional Improvement Measure (FIM) 26.6 FIM points 27.9 FIM points Case Mix Index is an indicator of the severity of patients served. The Case Mix Index for CY11 was calculated at 1.28, compared to 1.26 nationally. On follow up three months after discharge, 95.6% percent of patients reported overall high satisfaction. Predicted outcome achievement: 63% of patients met predicted FIM thresholds Home 78% 76% Long Term Care 1% 6% Acute Care 8% 10% Other Rehabilitation 11% 6% Three months following discharge, patients reported additional functional gains averaging 20 FIM points versus the national gain averaging 18.7 FIM points. One year post discharge, 98% of patients discharged during CY2011 live in a community setting.
2 UWHC Inpatient Rehabilitation Program for Adolescents The UWHC Acute Rehabilitation Program offers integrated care to adolescent patients, ages 15 to 18 years. The focus of care is to regain abilities to perform daily tasks, develop life skills, reclaim cognitive and physical function and facilitate psychosocial adjustment. The program features doctors who specialize in pediatric rehabilitation; a certified pediatric nurse practitioner; and an educational counselor who coordinates the return to school or work. Rehabilitation care for adolescent patients occurs on the Acute Inpatient Nursing Unit at the University of Wisconsin Hospital on B4/4. The program offers family centered care and provides opportunity for visits by siblings and peers. All adolescent patients were discharged to their home environments. Services for Adolescents on Inpatient Rehabilitation Age appropriate services to better serve the adolescent population are available through the American Family Children s Hospital Child Life Programs: A hospital school features certified teachers. The rehabilitation schedule incorporates school hours. The Education Counselor facilitates school re-entry. A teen center lounge is a place for the teenage patient and his/her family in the evenings and on weekends. Siblings of adolescent patients can use a sibling services area. Adolescent patients have laptop and Playstation access. Adolescent patients have an opportunity to be part of the Teen Advisory Board..
3 STROKE Stroke Program Outcomes Acute Rehabilitation provides quality services within the continuum of care for stroke patients at UWHC and has certification as a Joint Commission Primary Stroke Center. Inpatients who are recovering from a stroke or other type of brain pathology receive expert treatment for impairments in strength, coordination, cognition, communication, swallowing, perception and/or psychological functioning. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 71 stroke patients, who represented 20% of the total number of patients served by UWHC Acute Rehabilitation. UWHC stroke vs. national average rehabilitation statistics: Age 59 years 69 years Length of Stay 16 days 13days Functional Improvement Measure (FIM) 28 FIM points 26.5 Percent of patients who met FIM Goal 63% Home 72% 70% Three months following discharge, stroke patients treated at UWHC Acute Rehabilitation average additional functional gains of 26.1 FIM points versus 20.2 nationally. High overall satisfaction, 3.86 rating on a 4 point scale was reported.
4 Orthopedics Orthopedic Program Outcomes January 2011through December 2011 Orthopedic patients have who had joint replacement due to a degenerative process or fractures receive treatment to facilitate mobility and enhance independence. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 60 orthopedic patients. They represented 17% of the total number of patients served by UWHC Acute Rehabilitation. UWHC orthopedic vs. national average rehabilitation statistics: Age 53 years 72 years Length of Stay 11 days 11 days Functional Improvement Measure (FIM) 26.9 FIM points 29.5 FIM points Percent of patients who met FIM Goal 78% Home 93% 81% Three months following discharge, orthopedic patients treated at UWHC Acute Rehabilitation average additional functional gains of 15.1 FIM points, comparable to 17.8 nationally. High overall satisfaction, 3.76 rating on a 4 point scale was reported.
5 Spinal Cord Injury Spinal Cord Injury Program Outcomes Spinal cord injury patients receive integrated treatment designed to improve independence and foster lifestyle adjustment. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 42 spinal cord injury patients. They represented 12% of the total number of patients served by UWHC Acute Rehabilitation. UWHC spinal cord injury vs. national average rehabilitation statistics: Age 54 years 61 years Length of Stay 22 days 18 days Functional Improvement Measure (FIM) 21.5 FIM points 26.3 Percent of patients who met FIM Goal 47% Home 69% 75% Three months following discharge spinal cord injury patients treated at UWHC Acute Rehabilitation during 2011 have averaged additional functional gains of 15.7 FIM points surpassing average national FIM gain at 15.3 points. High overall satisfaction, 3.78 rating on a 4 point scale was reported.
6 Brain Injury Brain Injury Program Outcomes Brain injury patients receive integrated quality treatment designed to improve independence with mobility, self-care, communication, cognition, leisure and social skills. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 62 brain injury patients. They represented 17% of the total number of patients served by UWHC Acute Rehabilitation. UWHC brain injury vs. national average rehabilitation statistics: Age 48 years 63 years Length of Stay 16 days 14 days Functional Improvement Measure (FIM) 30 FIM points 28.2 FIM points Percent of patients who met FIM Goal 71% Home 69% 73% Long Term Care 3% 8% Three months following discharge, brain injury patients treated at UWHC Acute Rehabilitation average additional functional gains of 22.5 FIM points, comparable to the average 22.6 FIM gain nationally. High overall satisfaction, 4 rating on a 4 point scale was reported.
7 Amputation Patients with Amputations Outcomes Amputation patients receive integrated quality treatment designed to improve independence with mobility and self-care. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 26 patients with lower extremity amputation. They represented 7% of the total number of patients served by UWHC Acute Rehabilitation. UWHC amputee vs. national average rehabilitation statistics: Age 62 years 63 years Length of Stay 12 days 13 days Functional Improvement Measure (FIM) 17.7 FIM points 23.5 FIM points Percent of patients who met FIM Goal 55% Home 69% 74% Three months following discharge, amputee patients treated at UWHC Acute Rehabilitation average additional functional gains of 17.7 FIM points, comparable to the average gain of 14.4 FIM points nationally. 8 Major Multiple Trauma Major Multiple Trauma Outcomes Major multiple trauma patients are those recovering from trauma to multiple body systems, that is a combination of injuries to brain, spinal cord, fractures and or amputations. Traumatic injury conditions require integrated, individualized, quality care designed to improve independence and self care. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 23 patients with multiple traumas. They represented 6% of the total number of patients served by UWHC Acute Rehabilitation. UWHC multiple trauma vs. national average rehabilitation statistics: Age 40 years 49 years Length of Stay 18 days 16 days Functional Improvement Measure (FIM) 36.6 FIM points 32.8 FIM points Percent of patients who met FIM Goal 76% Home 87% 91% Three months following discharge, major multiple trauma patients treated at UWHC Acute Rehabilitation average additional functional gains of 21.9 FIM points, as compared to an average gain of 23 FIM points nationally. High overall satisfaction, 3.82 rating on a 4 point scale was reported.
9 Debility Outcomes for Debility Patients Debility patients are those recovering from complicated medical conditions, such as organ transplants, major surgeries, or infections, and have resulting overall weakness. Debility conditions require integrated, individualized, quality care designed to improve strength, foster independence and regain self care abilities. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 27patients due to debility. They represented 8% of the total number of patients served by UWHC Acute Rehabilitation. UWHC amputee vs. national average rehabilitation statistics: Age 54 years 73 years Length of Stay 16 days 12 days Functional Improvement Measure (FIM) 26.8 FIM points 26.6 FIM points Percent of patients who met FIM Goal 73% Home 81% 75% Three months following discharge, debility patients treated at UWHC Acute Rehabilitation average additional functional gains of 22.2 FIM points, exceeding the average gain of 17.7 FIM points nationally. High overall satisfaction, 3.89 rating on a 4 point scale, was reported.
10 Neurology Update for Neurological Patients Patients with neurological conditions, such as multiple sclerosis, Parkinsonism, and neuropathy, require integrated, individualized, quality care designed to improve strength, foster independence and regain self care abilities. UW Hospital and Clinics (UWHC) Acute Rehabilitation served 31 patients due to neurological conditions. They represented 9% of the total number of patients served by UWHC Acute Rehabilitation. UWHC amputee vs. national average rehabilitation statistics: Age 48 years 69 years Length of Stay 15 days 14 days Functional Improvement Measure (FIM) 21.7 FIM points 28.4 FIM points Percent of patients who met FIM Goal 63% Home 81% 75% Three months following discharge, neurology patients treated at UWHC Acute Rehabilitation average additional functional gains of 18.9 FIM points, exceeding the average gain of 16.7 FIM points nationally.
