2 From the Corporate Director Thank you for your interest in Winchester Rehabilitation Center and Valley Health Rehabilitation Services. At Winchester Rehabilitation Center and at each of our sites of service, we focus not only on patient satisfaction, but on the outcomes of the services we provide. These outcomes may include the functional improvement of our patients, satisfaction with our services, discharge setting, improvement in pain levels, the length of treatment, and cost of treatment. We utilize national outcomes measures to quantify the effectiveness of our services. It is our pleasure to share our 2011 Annual Outcomes Report of Winchester Rehabilitation Center and Valley Health Rehabilitation Services with you. In this publication, you can review the outcomes of our inpatient and outpatient services at Winchester Rehabilitation Center from January 1, 2011, to December 31, The functional improvement of our patients is the common outcome measure we use throughout our rehabilitation services and programs. The effectiveness of our interventions is best documented by measuring improvement in functional activities that are important to the patients we serve. Rehabilitation is achieved through the collaborative efforts of our experienced and dedicated team working with the patients and families to improve function, relieve pain and improve performance. You can compare our outcomes with other hospitals and outpatient programs offering rehabilitation services across the country as we have benchmarked our outcomes. We identify best practices and make every effort to incorporate them into our services. As you will see, we compare favorably in almost all areas to regional and national benchmarks. We hope you will take a moment to review our annual report to see how we compare and perform. We continually strive to improve and enhance the rehabilitation services we offer so that each patient receives the best experience every time, every place. 3 Winchester Rehabilitation Center Annual Report 2011 Throughout 2011, Winchester Rehabilitation Center (WRC) participated in a number of activities to promote acute and outpatient rehabilitation services, patient education and outcomes. Winchester Rehabilitation Center staff participated in the 8th annual Rehab Expo for all Valley Health Rehabilitation Services employees on October 26, We recruited a new medical director, Dom Vinh, MD. Dr. Vinh is board certified in internal medicine, physical medicine and rehabilitation, and spinal cord injury. Prior to coming to Winchester Rehabilitation Center, he practiced at the Rehabilitation Institute of Chicago. Winchester Rehabilitation Center s acute inpatient rehabilitation program transitioned to a new model for medical management of each patient through use of hospitalists in addition to physiatrists directing each patient s hospital course and rehabilitation. Winchester Rehabilitation Center and Valley Health Rehabilitation Services transitioned eight of our nine outpatient clinics to electronic medical record documentation. Additionally, outpatient services have continued to increase outcomes reporting through FOTO. Winchester Rehabilitation Center had two therapists participate in the Valley Health Leadership Mentoring Program in This is an organizational initiative to promote growth and development of staff as part of succession planning. A physical therapist and an occupational therapist from Winchester Rehabilitation Center traveled to Haiti in 2011 as part of a mission trip to assist with the transition of a child back into her community following the earthquake. Winchester Rehabilitation Center continued to participate in the following community disability organizations: Access Independence, American Red Cross, and Virginia Council of Rehabilitation Administrators. 3

4 Winchester Rehabilitation Center and Winchester Medical Center staff continue to coordinate the Northern Shenandoah Valley Chapter of the FREE Foundation (Foundation for Rehabilitation Equipment and Endowment), which distributes equipment to individuals with a medical need. WRC staff participated in many community outreach events including: Arthritis Walk, Community Safety Fair, Health Explorers 2011, Come Explore Nursing 2011, and Diabetic Retinopathy Screenings. Valley Health has acquired a mobile Health Coach vehicle, and Valley Health Rehabilitation Services brought diabetic retinopathy screenings to members of our rural communities using this vehicle. Winchester Rehabilitation Center and Valley Health Rehabilitation Services staff supported a fellow occupational therapist and troops stationed in Afghanistan throughout WRC continues to pursue its mission of promoting healthy lifestyles for our community by participating in community health fairs and providing mentorship programs for careers in healthcare for area high school and college students. Mission Statement The Mission of Winchester Rehabilitation Center is to provide medical rehabilitation services that improve the functional abilities of our patients and promote healthy lifestyles for our community. 4

13 Service Locations and Programs Winchester Rehabilitation Center Inpatient hospital, 30 beds, CARF & Joint Commission accredited Winchester Rehabilitation Center has received full three-year CARF accreditation (Commission on Accreditation of Rehabilitation Facilities) since 1996 in the following programs: Comprehensive Integrated Inpatient Rehabilitation Outpatient Medical Rehabilitation Winchester Rehabilitation Center is also accredited by The Joint Commission as part of Winchester Medical Center. Everyone was friendly and on the ball Thank you all Outpatient Rehabilitation Services CARF & Joint Commission accredited 333 West Cork Street, Suite 142 Winchester, VA Outpatient Rehabilitation Services offers comprehensive multidisciplinary services on an outpatient basis. This cost-effective alternative to inpatient hospital care provides a range of treatment from a half a day of therapy to individual therapy 2-5 days a week based on the patient s individual needs. We utilize a team approach to rehabilitation and meeting the individual needs of each patient. 14 Valley Health Rehabilitation Services Services provided by Valley Health Rehabilitation Services are available at multiple convenient locations throughout the Shenandoah Valley, allowing individualized patient treatment along the continuum of rehabilitation services from inpatient care to outpatient therapy. The following outpatient services and programs are available throughout Valley Health in Luray, Winchester, Front Royal, Woodstock, Strasburg, and Mount Jackson. In addition, we have locations in Romney and Berkeley Springs, West Virginia. Because of the entire staff I was able to complete my therapy weeks/months ahead of time. 15 Community and Professional Outreach Support Groups: Amputee The Differently Abled Club Low Vision Stroke Brain Injury Screening for Diabetic Retinopathy A partnership between WRC, Valley Health Diabetes Management and Retina Associates, provided free quarterly retinal screenings for diabetics living in the Shenandoah Valley. The highly qualified and experienced rehabilitation staff members give lectures and presentations at professional conferences and training seminars throughout the year. In addition, staff are often asked to be guest lecturers at Shenandoah University Schools of Occupational and Physical Therapy to share their expertise on rehabilitation topics. For our community organizations, local churches and business groups, the rehabilitation staff provides free informational talks on various topics related to the rehabilitation process and health-related areas. The entire staff at WRC and the facility are exceptional. Would definitely recommend WRC to anyone in need of rehab services. Patient Quote 15
