1 SAMPLE Equity Data Profile and LEA Equity Action Plan The following LEA Equity Data Profile and corresponding LEA Equity Action Plan have been provided as SAMPLES in order to help LEAs in developing their unique LEA Equity Action Plan tailored to the LEAs equity needs. This sample only identifies and plans in response to one Equity Data Profile variable and corresponding Equity Intervention; however, LEAs are REQUIRED to select TWO Equity Data Profile variables and TWO corresponding Equity Interventions for focus each year. An Equity Gap Intervention Plan should be completed for each of the two Equity Data Profile variables and Equity Interventions. Please contact your assigned Title II, Part A Specialist with questions. REMINDER: FY17 Equity Action Plans are due July 31 and should be uploaded as an attachment to the CLIP in the GaDOE Portal.
