1 PHARMACY DEPARTMENT JOB DESCRIPTION POST: PRE-REGISTRATION TRAINEE PHARMACIST BAND: 5 REPORTS AND RESPONSIBLE TO: ACCOUNTABLE TO: RELATIONSHIPS: Principal Pharmacist, Medicines Management (Pre- Registration Tutor) Chief Pharmacist (Pre-Registration Manager) Duties are co-ordinated by Pharmacists and Senior Pharmacists within the area of pharmaceutical activity practised at the time JOB SUMMARY: A one year fixed term contract to meet the requirements of the Royal Pharmaceutical Society of Great Britain (RPSGB) to register as a pharmacist by : a. gaining appropriate training and experience in the provision of pharmaceutical services within each section of the pharmacy department b. demonstrating competence and achievement of performance standards as identified in the RPSGB Training Manual c. undertaking and successfully completing the pre-registration pharmacist examination SECTION 1- MAIN RESPONSIBILITIES: a Clinical Services 1. Assists in the provision of ward and clinical pharmacy services including : Compilation of drug history, where possible, from patients admitted to hospital to ensure continuity of their medication and records relevant information in the pharmacy care record in order to facilitate the discharge process. communication with the patient, their GP and hospital medical staff and reference to the patient s notes in order to obtain accurate details of medication, allergies and other relevant clinical information. checking of the patient s own medication and assessing it s suitability for use where appropriate Monitoring of in-patient and discharge prescriptions for clinical accuracy and cost

2 effective prescribing. Assessing the patient s response to therapy Documenting clinical interventions and activities in line with local policy. Using inter-personal skills to counsel patients to ensure they gain maximum benefit from their medicines, and educating them regarding the use of equipment for the administration of medicines. b Medicines Management 2. Promotes adherence to the hospital formulary, treatment guidelines and protocols. 3. Assists in drug utilisation and expenditure reviews and clinical audit activities. c. Dispensary 4. Assists in the dispensing of prescriptions and supply of pharmaceutical products (including controlled drugs and clinical trial medicines) for inpatients, outpatients, patients being discharged and mental health patients on leave in accordance with local standards, policies and safe systems of work 5. Counsels outpatients to ensure that they gain maximum benefit from their medicines. d Medicines Information 6. Assists with the provision of information and advice on all aspects of pharmaceutical products as requested by medical & nursing staff and other health professionals including other Trust professional staff, GPs, community pharmacists, and residential care and nursing home staff. 7. Assists with the provision of information and advice to patients on all aspects of their medicines 8. Undertakes critical appraisal and evaluation of literature relating to the use of pharmaceutical products. e Extemporaneous Dispensing and Product Assembly 9. To prepare and package extemporaneously or in advance of requirements medicines for internal and external use using suitable techniques. Products are prepared from materials / ingredients in accordance with good manufacturing practice and with COSHH regulations, including the completion of all relevant documentation. This may require use of safe handling equipment including a Fume Filtration Cabinet where appropriate. 10. To be familiar with and use the Pharmacy Computer System in accordance with procedures within different sections of the pharmacy 11. Maintains, replenishes and checks the accuracy of assembly of the resuscitation boxes and emergency trays in accordance with local procedures. f. Ward Medicines Distribution and Pharmacy Top-Up 12. To participate in the maintenance of a cost-effective system of medicine distribution to all wards, departments and community clinics The objective of the service is to ensure that economic stock levels are maintained in appropriate storage conditions and that the products have not time-expired and are suitable for use

3 13. To assist with the issue medicines of and provide a stock topping-up service to peripheral hospitals, community clinics, wards and departments in response to an authorised request. 14. To check the accuracy of the assembly of ward orders carried out by pharmacy assistants and other approved personnel 15. To participate in ward based stock audits under the direction of the Dispensary Manager or Chief Pharmacy Technician g. Aseptic Dispensing Service 16. To assemble materials, prepare and dispense products requiring the use of appropriate aseptic procedures, to Good Manufacturing Practice standard, without the direct supervision of a pharmacist, h. Stock Control 17. To ensure that dispensary stock levels are maintained and stocks are replenished from the pharmacy store. 18. Assesses pharmaceutical products returned from clinical areas and returns to stock items which are fit for reuse or disposes of items unfit for use in accordance with COSHH regulations and Trust procedures. Enters details of returned items on pharmacy computer system. 19. Carries out stock checks within the department in accordance with local procedures to ensure computer records accurately reflect stock holding. 20. Monitors cold storage equipment to ensure temperatures are within the defined limits and reports any variation to a supervisor. 21. Identifies and reports stock shortages to procurement colleagues for ordering to ensure that pharmaceutical products are available for patients when required. 22. Identifies and reports stock discrepancies to a supervisor to ensure that pharmaceutical products are available for patients when required. 23. Maintains storage areas in an organised, safe and tidy condition in accordance with Health and Safety regulations. 24. Participates in a system for checking expiry dates of stock to ensure that stock is rotated so that shorter dated stock is used first and avoid unnecessary wastage. Reports out of date stocks to a supervisor and disposes of stock where appropriate recording the issue on the pharmacy computer. i Education & Training 25. Participates in the competency based training programme as required by the Royal Pharmaceutical Society of Great Britain 26. Responsible for self-directed learning to support the collection of a portfolio of evidence to demonstrate achievement of performance standards 27. Participating in quarterly appraisals with the pre-registration tutor and prepare for the preregistration examination 28. Visiting other hospital site in order to gain broad training in aspects of hospital pharmacy practice 29. Participate in a placement of at least two weeks within a community pharmacy setting 30. Undertake projects to assist personal development and that of the pharmaceutical service

4 31. Participate in clinical meetings within the department and prepare and deliver short presentations for other members of staff as part of the CPD programme 32. To attend local and regional study courses and keep abreast of current pharmaceutical practice and knowledge, attending meetings, courses or conferences as necessary, for Continuing Professional Development in accordance with national guidelines from the Royal Pharmaceutical Society. i. General Duties 33. Maintains a high standard of work and promotes the professional image of Pharmacy within and outside the department. 34. Ensures that safe working systems are in operation in accordance with Trust and departmental procedures. 35. Ensures that all legal requirements relating to the prescribing, dispensing and storage of pharmaceutical products in the Trust are met and are in accordance with criteria set out under COSSH regulations. 36. To participate in Saturday, Sunday, Bank Holiday and late night duties as is considered appropriate by the Chief Pharmacist or other senior manager in line with current local policy. This will include duties on Saturdays and Bank Holidays on a rota basis. 37. Undertakes manual handling up to maximum of 15Kg per item regularly each day SECTION 2- KNOWLEDGE AND SKILLS a. Vocational masters degree in pharmacy b. Registered with the Royal Pharmaceutical Society of Gt Britain for pre-registration training c. Good clinical and therapeutics knowledge, practical clinical skills and laws relating to pharmacy practice d. Good organisational, problem solving and decision making skills e. Ability to communicate verbally or in writing, medicines information to patients; carers and healthcare professionals, at a level appropriate to their understanding. f. Counsel patients / carers regarding sensitive issues around medicines and disease g. Able to work on own initiative and as part of a team h. Able to work under pressure. i. Good time management skills j. Able to operate relevant IT systems k. Able to prioritise work to allow treatment to be initiated and patients to be discharged as soon as possible. SECTION 3 - ADDITIONAL REQUIREMENTS: 1. To provide cover for colleagues as directed by your manager. 2. Risk Management (Health & Safety) a. To follow risk management procedures at all times. The risk management procedures for the Trust and the department are kept by the departmental manager. b. To be personally responsible for Risk Management issues in respect of yourself and your colleagues c. To report any potential hazards identified by you to your manager / supervisor, at

5 once. If in doubt you should speak to your manager for guidance d. To ensure that safety equipment provided is always used, and to report any defects to your manager. You must attend Risk Management training as directed by your manager. 3. Infection Prevention and Control a) You are personally responsible for ensuring that you protect yourself, patients, visitors and colleagues from the risks of infection associated with health care activities and the care environment. b) You must adhere to infection prevention and control policies at all times liaising with the infection control team and acting on any instructions given. c) You must attend regular infection prevention and control update training. d) You should at all times promote and demonstrate good practice for the prevention and control of infection. 4. To avoid any behaviour which discriminates against fellow employees, or potential employees on the grounds of their sex, sexual orientation, mental status, religion, creed, colour, nationality, ethnic origin or disability. 5. To safeguard at all times confidentiality of information relating to patients and staff. 6. To refrain from smoking in any areas of Trust premises. 7. To behave in a manner which ensures the security of NHS property and resources. 8. To abide by all relevant Trust policies and procedures.
