The road pavement was compromised and there was advanced wash out of aggregate in the sub-grade. The bottom course of block was broken at the base of the catch basin around the outlet/drain pipe (in the west arc of the structure, draining under the street to a catch basin located on the west curb of Maura Lane) Construction Notes: Elevation Rehabilitation: 1. Replacement block was laid in the second course, on the northwest point of the arc. The block was set in a cementitious masonry bed mixed by RCOC crew. 2. A 15 (+/-) clear opening was left around the outlet pipe (to road side) in the two courses of block. 3. Three block joints were filled using UGT s Crack n Seal, a polyurea elastomer. The Crack n Seal cured in 15 minutes. 4. A 1.0 coat of epoxy grout from UGT was applied to the top surface of the blocks to create a level bearing plane.

2 5. One, 5.5 high expanded polystyrene (EPS) adjustment unit was set in place and pressed into the grout. The wet grout compressed into the female interlocking channel of the EPS unit, creating a ½ nonslip keyway. Note: The 5.5 adjustment unit spanned the 15 opening. 6. An extension was constructed above the 15 opening between the 5.5 adjustment ring and the undisturbed aggregate fill. Made of the epoxy grout and capped with masonry, the extension will retain all new aggregate and prevent it from entering the catch basin structure. 7. The remaining elevation was dry fit and measured in place to ensure correct vertical adjustment and frame location in curb. The ten elevation units where marked and numbered, removed from the hole and bonded in place individually. 8. A continuous ¾ bead of Secure n Seal adhesive was applied to the top surface of each adjustment unit when it was set in place, starting with the 5.5 grade ring 9. Six, 2.0 high, EPS flat restoration units were vertically set in place. Each 2 unit was offset approx. 1.5 to the west of (away from) the back edge of the underlying ring. The offsetting over the six units moved the bearing alignment 9.5 to the west of the original position to ensure the frame was properly located in the curb face. 10. One, 1.0 high EPS grade unit was placed on top of the offset assembly. 11. One, 1.5 high grade unit was placed on top of the 1.0 grade unit. 12. One 1.0 x0.5 angle finish unit was placed on top of the 1.5 grade unit. The angle ran downhill from south to north along the curb line to follow the slope in road. 13. A ¾ bead of Secure n Seal adhesive was applied to the top surface of the finish unit to bond the ironworks frame (East Jordan model 7045) in place. 14. The adjusted height totalled 21 of vertical adjustment with a 9.5 east to west offset adjustment. 15. As constructed, the entire assembly is bonded as a monolithic structure from the top of the block to frame.

3 Curb Reconstruction: 16. RCOC formed and poured a new concrete curb around the rehabilitated catch basin on September 15, Road Paving: 17. RCOC replaced the granular aggregate and repaired the excavated asphalt pavement with hot mix material on September 15, Inspection Schedule: strataworks will co-ordinate a November 2011 and April 2012 inspection with RCOC s Highway Maintenance foreman. A copy of the final inspection report will be provided to RCOC upon completion.

Note: The 10 lateral offset is solid and in place, as constructed Other Notes: 1. A 10 (+/-) diagonal crack has occurred in the concrete, on the southwest of the catch basin frame at the edge of the asphalt. 2. The top surface of the new asphalt pavement is in good condition. 3. The junction seams between the old and new asphalt are tightly abutted and appear well sealed. 4. The overall grade (height) of the new asphalt patch appears to be ¼ to ½ lower than the original asphalt.
