2 Dear colleagues, It is great honour to invite you to Prague for ISAPS Symposium and to Brno for Cadaver Course 2015, which is jointly organized and hosted by the ISAPS Educational Council and Czech Society of Plastic Surgery on September, Scientific committee prepared comprehensive program, which is focused on new trends in aesthetic plastic surgery and which includes all important aspects of aesthetic surgery. World-renowned experts will share their experience during the lectures. For those who are eager to put their hands on and practically improve their skills, we offer a two-day cadaver dissection course, which will be held in Brno at the same place where the 2011 ISAPS Cadaver course was organized ( ISAPS Sept-Dec11.pdf ). We will have the opportunity to try what you have learned during the course and discuss all practical aspects of the surgical procedures with the faculty. Prague is one of the most incredible cities. Prague unique atmosphere and exciting social program will give you the opportunity to enjoy the city during your stay. Brno is the second biggest city in the Czech Republic and has an excellent cadaver lab facility, which meets the highest European standards. It is easily accessible from Prague on highway. We believe that combination of theoretical and practical teaching provides very good opportunity to learn effectively new techniques and to improve the care for our patients. 4 PROGRAM Course will be held on September 28th and 29th 2015 at the Educational Centre for Practical Anatomy ECPA, Department of Anatomy, Faculty of Medicine, Masaryk University, Kamenice 3, Brno, Czech Republic Course follows ISPAS Symposium in Prague on September 27th,2015. Course description Course is specifically designed for young plastic surgeons entering aesthetic surgery practice to improve their confidence in performing aesthetic surgery. For this reason course cover all anatomical areas: body, face and neck. Four trainees per cadaver allowed us to achieve affordable price. Each day program starts with comprehensive oral presentation and continue with practical dissection under interactive supervision of lectors. Course is designed to maximize time for practical dissection. Course objectives Learn aging changes, relevant surgical anatomy and learn different surgical techniques and technical tips how to achieve consistent results and avoid complications. Course offers unique opportunity to try and compare different implants from different manufacturers. Upon receipt of the registration form, a confirmation of registration and payment information will be sent within three working days to the participants. The payer pays the bank charges of their bank, and the beneficiary pays the charges of their bank, if any. Credit Card For Credit Card payments please send an to the Course Secretariat (contact@bos-congress.cz). We will send you the Authorization Form. The form has to be signed by a Credit Card holder and sent to the Course Secretariat by fax ( ), not via for security reasons. The payment will be charged in Czech crowns (CZK) at the exchange rate of the Czech National Bank valid on the date of payment. The confirmation of your payments for taxation purposes will be issued and delivered during the Course. CANCELLATION AND REFUNDS The Course Secretariat must be notified in writing by (contact@bos-congress.cz) or fax ( ) about the cancellation of the registration, accommodation. Cancellations by phone cannot be accepted. The appropriate refunds will be made after the course. The following cancellation conditions will be applied: Registration fee Cadaver Course - before 30 June 2015: full refund of the registration fee less 20 EUR handling fee - from 1 July 2015: no refund
7 Accommodation PRAGUE - before 30 June 2015: full refund of the accommodation - from 1 July 2015 to 15 July 2015: 25% cancellation fee - from 16 July 2015 to 10 August 2015: 60% cancellation fee - from 11 August 2015 to 14 August 2015: 90% cancellation fee - from 15 August 2015: no refund Accommodation Holiday Inn Brno (Cadaver Course) - before 31 August 2015: full refund of the accommodation - from 1 September 2015 to 6 September 2015: 50% cancellation fee - from 7 August 2015: no refund If you arrive later or leave earlier than the originally booked dates the total accommodation amount will be charged and no refunds can be made. Please do not send any correspondence to the hotel directly. VISA REQUIREMENTS Visa & Invitation Letter Information The organising company will start issuing the invitation/confirmation letters from June and the earliest possible date to apply for a visa at the embassy is 3 month in advance. The estimated duration for visas to be issued is 2 weeks. Visitors from non-schengen countries must possess a passport valid for at least a three month stay. To begin with participants are advised to locate their respective diplomatic missions or embassy. As offices aren t available in all countries, it is necessary to either apply at an embassy representing the CZ (listed here) or to travel to an assigned embassy in another country (listed here). It is not possible to obtain a visa on arrival at Prague airport. It is necessary that the applicant applies in person at the embassy. This is due to a new Visa Information System, which requires the collection of fingerprints. The list of countries that are a part of this new system is here. Invitation letters are issued upon request of the applicant. Most embassies accepts invitations issued by the organising company, nevertheless some may demand an official invitation letter issued by the alien police of the Czech Republic. In such case, additional fees apply and the estimated time of issuance is prolonged. It is therefore suggested to begin the visa handling as soon as possible, as the total processing time may be up to 5 weeks. Conditions to issue the invitation letter are as follows: paid registration fee. There are additional documents requested in order to apply for a visas at the embassy, these are listed here. Reimbursement requests caused by visa denial or expatriation of the delegate will be handled on an individual basis. For issuing the visa invitation letter please contact the Course Secretariat. GENERAL INFORMATION Information Corinthia Hotel Prague Only a short distance away from the city centre of Prague, the Corinthia Hotel is a landmark in itself, standing atop one of Prague s seven hills and offering outstanding views of the capital city. The hotel is situated at the metro stop Vyšehrad (line C), just two stops or a short drive from the historical city centre as within a three-minute walk of its oldest castle the 10th century Vysehrad. Airport Information Václav Havel Prague Airport is a modern and spacious international airport located on the northwest edge of Prague, around 17 kilometres north of the city. The journey from the airport to the city centre takes around minutes by taxi or up to one hour by public transport.. Getting from the airport is possible by public bus, shuttle or taxi service. Public transport bus service links are available to underground stations on two different lines. The approximate time from the airport to the nearest metro (Nádraží Veleslavín, line A) is around 25 minutes.
8 Prague Transport Public Transport: Prague has a good public transport system, which includes 3 metro lines, trams and buses. Prague s metro system is quite new and efficient. During peak hours trains run every 1 or 2 minutes and at least every 10 minutes off peak. Tickets can be purchased in yellow machines at the public transportation stops or in newsstands. Information on Pragues' transportation system can be found on the official website: Taxi: Most taxis in Prague do not accept credit cards! Although it is easy enough in the centre to hail a taxi on the street, it is recommended that you book one in advance. The official fare is approximate (starting fee is 40 CZK plus CZK per kilometre, plus 4-6 CZK per minute of waiting in traffic), so please check the price before you get into the car. AAA taxi City Taxi: Profi Taxi Useful Telephone Numbers Fire 150 Ambulance 155 Prague Police 156 Police 158 General Emergency for Europe 112 Currency and Banking: Czech koruna (CZK) is the official currency in the Czech Republic. Foreign currency exchange is available at Prague International Airport and at most hotels, banks and exchange offices throughout the city. International credit cards are accepted for payment in hotels, restaurants and shops. Paying by cash in Euro is also possible in some restaurants and shops. Electricity: The Czech Republic uses a 230 volt 50 Hz system, sockets are the standard European type and two-prong round pin plugs, with a hole for a male grounding pin. To use electric appliances from your country you may need a special voltage converter with an adapter plug. Telephone: International Dialing for Czech Republic: Dial Telephone number. Weather: Prague is a city with a mild continental climate. The average temperature in September varies around 18 C. WiFi is available throughout the Corinthia Hotel Prague free of charge. Refreshments: Morning and afternoon coffee break, lunches, dinner 28 September 2015 Smoking Policy: For the comfort and health of all participants, smoking is not permitted at any time. Liability & Insurance: The Course Secretariat and organizers cannot accept responsibility whatsoever for injury or damage involving persons and property during the meeting. Participants are advised to make their own arrangements with respect to health and travel insurance. Force Majeure: Please note that in the case of force majeure the event organizer is relieved of all obligations. Force majeure includes any circumstance beyond the control of the event organizer, and that impedes permanent or temporary compliance, such as war, revolts, strikes, shut-out of workers, transportation difficulties, fi re or severe disturbance affecting the event organisation or its suppliers. Time Zone: The Czech Republic is on Central European Time Greenwich Mean Time (GMT) plus1 hour. From April to October is summer time, i.e. GMT + 2 hours. Tipping: Service is usually included in the bill in bars and restaurants, but tips are welcome. If you consider the service good enough to warrant a tip, we suggest about ten percent.
