Correspondence must be through your HVCC webmail account. No Exception. All three parts must be completed and completed correctly. Incomplete and incorrectly completed forms will be returned. PART I Please answer the questions regarding your academic and future endeavors. The answers you provide will become part of your physical record. If a question is not applicable, please indicate so by putting N/A. Your answers to the questions may result in follow up questions. PART II Complete the academic planning sheet.
