2 Charles Kim, Senior Vice President Academic Health Centers: Trends and Issues Facilitators: Charles Kim and Nick Long Walton Room Charles Kim is a Senior Vice President of Kaufman Hall. Mr. Kim consults on a national basis with clients including regional healthcare systems, academic medical centers, and community hospitals. Mr. Kim s expertise includes preparing financial and capital plans, acting as financial advisor for bond issues, advising on merger, acquisition, and divestiture transactions, and providing training and support to the ENUFF Advisor software suite. Mr. Kim has more than 15 years of healthcare consulting experience and more than 20 years of financial advisory experience. Prior to Kaufman Hall, Mr. Kim was a Manager at Deloitte & Touche, specializing in providing financial advisory services to healthcare organizations. Mr. Kim is a frequent speaker on the topic of strategic financial planning for healthcare organizations. He presents at various industry groups, including local and regional programs for the Healthcare Financial Management Association, and seminars for the American College of Healthcare Executives. Mr. Kim has an M.B.A. in Finance and Accounting and a B.A. in Economics, both from the University of Chicago. Nick Long, Vice President [email protected] Nick Long is a Vice President of Kaufman Hall in the Higher Education division of the Financial Planning practice. Mr. Long has more than 20 years of experience in financial management, information technology, and investment management. This includes more than a decade of higher education experience, consulting with public and private universities and academic medical centers nationwide. Mr. Long s expertise and areas of focus include financial management, cost reduction, information technology implementations, and interim management. He has experience in senior interim management roles and also has worked with international institutions on issues related to financial management. Prior to joining Kaufman Hall, Mr. Long was a Director at Huron Consulting Group, where he worked on engagements focused on cost reduction, technology implementation, and interim management. Prior to this, he held finance roles at Oracle Corporation and Adobe and did management consulting work with Ernst & Young. Mr. Long has presented on topics related to financial compliance at numerous national conferences. Mr. Long has an M.B.A in Finance and Information Technology from University of California, Irvine and a B.A. in Economics from University of California, San Diego.

Allen, Managing Director Building a Post-Acute Network Facilitators: Patrick Allen and Chirag Patel Lakeview Room Patrick Allen is a Managing Director in the Mergers and Acquisitions practice of Kaufman Hall. With more than 20 years of experience, Mr. Allen has significant expertise in mergers, acquisitions, divestitures, joint ventures, valuations, fairness opinions, and related transaction services. Mr. Allen s experience spans the healthcare landscape where he has completed multiple transactions representing not-for-profit and for-profit providers, as well as private equity firms. Mr. Allen is a frequent speaker and author on the healthcare industry, with topics including partnership structuring, evolving healthcare services models, and consolidation trends. Mr. Allen has published numerous articles in industry journals and white papers. He is a regular speaker at industry events and conferences, including The Governance Institute, American College of Healthcare Executives, Healthcare Financial Management Association, and The Society for Healthcare Strategy and Market Development. Prior to joining Kaufman Hall, Mr. Allen was an investment banker in the Healthcare Mergers & Acquisitions practice of CIT Group and previously worked for UBS in their Corporate Finance Group as a Director. Mr. Allen earned his M.B.A. in Finance from Loyola University of Chicago, where he was an Alan C. Greenberg scholar, and his B.S. in Political Science from Santa Clara University. Chirag Patel, Vice President [email protected] Chirag Patel is a Vice President of Kaufman Hall and a member of the firm s Strategy practice. Based in the Skokie office, Mr. Patel focuses on strategic planning for hospitals and health systems nationwide, specifically in the context of service distribution planning and industry reorganization. He also has a breadth of experience in assisting hospitals develop strategic and business plans for new service offerings related to their physician strategy. Prior to joining Kaufman Hall, Mr. Patel was a Consultant at Sg2, where he supported client engagements related to corporate planning, technology acquisition, and business development for both domestic and international clients. Mr. Patel has an M.A. in the Learning Sciences, with a focus on Organizational Cognition, and a B.S. in Biomedical Engineering, with a focus on Biotechnology, from Northwestern University.

4 Kenneth Kaufman, Chair C-Suite Leadership Issues Facilitators: Ken Kaufman and Rob Fromberg Lakeview Room Kenneth Kaufman is Chair of Kaufman Hall a management consulting firm that provides advisory services and software to hospitals and health systems nationwide. Since 1976, Mr. Kaufman has provided healthcare organizations with expert counsel and guidance in areas including strategy, finance, financial and capital planning, and mergers, acquisitions, and partnerships. Clients include organizations of all types and sizes community hospitals and health systems, academic medical centers, and regional or national health systems. Recognized as a leading authority and committed to industry education, Mr. Kaufman has given more than 400 presentations at meetings such as those organized by the American College of Healthcare Executives (ACHE), American Hospital Association, Healthcare Financial Management Association, The Governance Institute (TGI), and others. Mr. Kaufman has authored or coauthored seven books, most recently authoring Fast and Furious: Observations on Healthcare s Transformation; Focus on Finance, published by TGI; and Best Practice Financial Management, 3 rd Edition, published by ACHE. In addition, he s often quoted and his articles regularly appear in major healthcare publications. Mr. Kaufman has an M.B.A. with a concentration in Hospital Administration from the University of Chicago Graduate School of Business. Robert Fromberg, Senior Vice President [email protected] Robert Fromberg is a Senior Vice President in the Thought Leadership group at Kaufman Hall. Working with the firm s executives, industry groups, and other external audiences, his focus is program development and management, and the strategy for communicating thought leadership. Before joining Kaufman Hall, Mr. Fromberg was Vice President and Editor-in-Chief at the Healthcare Financial Management Association (HFMA). In this role, he was responsible for editorial direction, content, and production of hfm magazine, developing this publication into one with a leading position in the healthcare industry. He also oversaw all facets of communications, including magazine/newsletter publishing, web/digital strategy, custom publishing, conference general-session production, media relations, speechwriting, speech coaching, social media, online communities, and video for the 40,000+ member healthcare association. Prior to HFMA, Mr. Fromberg held a variety of marketing, communications, and writing roles in healthcare-related organizations. He also spent 17 years as a Lecturer at Northwestern University School of Continuing Studies. Mr. Fromberg holds an M.F.A. from Warren Wilson College and a B.A. in English from Bradley University.

5 Catholic Healthcare: Trends and Issues Facilitators: David M. Cyganowski and Kate Guelich State Room David M. Cyganowski, Managing Director David Cyganowski, who joined Kaufman Hall as a Managing Director in September 2011, is recognized as one of the nation s leading healthcare professionals. With 30 years of senior experience in healthcare, he has extensive and specific expertise in strategy, mergers, affiliations, and corporate capital finance. Mr. Cyganowski directs the firm s New York office, using that location as a base to advise hospitals, health systems, and other healthcare organizations nationwide. He is known for his candor, objectivity, and creativity in advising Boards of Directors, Chief Executive Officers, and their leadership teams as they develop, evaluate, and execute New Era growth strategies. Mr. Cyganowski formerly was the Managing Director and Co-Head of the Health Care Group at Citigroup Global Markets, with responsibility for working with not-for-profit hospitals, healthcare systems, and managed care organizations across the country, and directing Wall Street s largest healthcare practice exclusively devoted to the not-for-profit sector. During his tenure, Citi ranked first in senior managed healthcare financings during 14 out of 16 years, an unprecedented record of success for a healthcare investment banking practice. Prior to joining Citi in 1995, he managed the healthcare practice at Credit Suisse First Boston. During his 30 years of investment banking experience, Mr. Cyganowski served as senior banker for mergers, affiliations, and partnerships with an aggregate deal value of $15 billion and more than $35 billion of debt financings. He led the Citi team that represented eight not-for-profit healthcare systems in their successful acquisition of 21 hospitals from Columbia/HCA for $1.2 billion, the largest-ever acquisition of investor-owned hospital assets by not-for-profit systems. Mr. Cyganowski was also one of the original architects of the Annual Non-Profit Health Care Investor Conference, jointly sponsored by the American Hospital Association, Healthcare Financial Management Association, and Citigroup. This is the only such conference exclusively devoted to not-for-profit healthcare. Mr. Cyganowski is a frequent speaker at healthcare conferences and board retreats. He is a member of the Board of Directors of Western Connecticut Healthcare Network, and is its current Finance Committee Chair. A native of Chicago, Mr. Cyganowski received an M.B.A. from Northwestern University s Kellogg Graduate School of Management. Kate Guelich, Managing Director [email protected] Kate Guelich is a Managing Director of Kaufman Hall and a member of the Strategic Financial and Capital Planning and Financial Advisory practices. With more than 15 years of healthcare consulting experience, her responsibilities focus on financial and capital planning for a range of healthcare clients, including large, multi-state healthcare systems, academic medical centers, and community providers. Her areas of expertise include strategic financial planning, mergers and acquisitions, and financial advisory services.

6 Prior to joining Kaufman Hall, Ms. Guelich was a Senior Consultant with Cap Gemini Ernst & Young s Healthcare Strategy and Transformation group. Ms. Guelich has provided a wide-array of presentations on strategic financial planning at professional industry meetings on a national, regional and local basis. Ms. Guelich has an M.B.A., with a concentration in Finance, from the University of Chicago Graduate School of Business. She received a B.S. in Business Administration from Miami University in Oxford, Ohio. Ms. Guelich is also a CPA in Illinois.

Wareham, Managing Director and Chief Executive Officer [email protected] Terri Wareham, Managing Director and Chief Executive Officer, as well as a founding partner of Kaufman Hall, has provided management consulting services to hospitals and health systems of all types and sizes nationwide for more than three decades. Recognized as a leading authority on healthcare financing, Ms. Wareham has an extensive background in the development and execution of strategic capital plans for freestanding providers and multihospital systems. Her areas of expertise and focus include: capital and business planning; financial advisory services, such as the structuring of financing vehicles and the formulation of master indenture financings; and mergers, acquisitions, divestitures, joint ventures, and affiliations. Benefitting from Ms. Wareham s guidance, Kaufman Hall has been the leading financial advisor to not-for-profit healthcare organizations, as determined by Thomson Reuters based on direct representation of healthcare debt issuances, for nearly a decade. She received a B.S. from the University of Notre Dame Ryan S. Gish, Managing Director [email protected] Ryan Gish is a Managing Director of Kaufman Hall and a member of the firm s Strategy practice. Mr. Gish works with executive leadership teams and boards of all types of hospitals and health systems nationwide. The strategic advisory focus is on helping organizations address the most pressing industry challenges through defining and implementing resilient strategies for the changing healthcare landscape. The result for clients is a platform for their ongoing strategic and financial success. Mr. Gish has authored numerous articles published in healthcare professional journals, including hfm magazine, BoardRoom Press, and Trustee. Additionally, Mr. Gish was a contributing author for Health Care Strategy for Uncertain Times, published by AHA Press/Jossey Bass. Olin School of Business at Washington University in St. Louis and a B.S., cum laude, also from Washington University. Mr. Gish currently is serving a three-year term on the SHSMD Board of Directors.

9 Robert Turner, Senior Vice President Financing Issues and Opportunities Facilitators: Robert Turner and Ryan Freel Bellevue Room Robert Turner is a Senior Vice President of Kaufman Hall and a member of the firm s Financial Advisory practice. Based in Dallas, Mr. Turner consults with healthcare clients nationwide, focusing on issues related to capital structure strategy, and the analysis and implementation of debt transactions. He specializes in public and private tax-exempt financings, plan of finance development, and debt restructurings. Mr. Turner also consults with the firm s clients on FHA 242 debt financings. Prior to joining Kaufman Hall, Mr. Turner was a Vice President with Goldman, Sachs & Co. in the Not-for-Profit Healthcare Group, where he specialized in tax-exempt healthcare finance. Mr. Turner is a regular speaker on capital market issues and financial planning topics for organizations including the Healthcare Financial Management Association. Mr. Turner holds an M.B.A. from Southern Methodist University, with a concentration in Finance, and a B.A. from Washington & Lee University. Ryan Freel, Senior Vice President [email protected] Ryan Freel is a Senior Vice President of Kaufman Hall and a member of the firm s Financial Advisory practice. Mr. Freel works with a wide range of healthcare organizations, including academic medical centers, multi-state health systems, children s hospitals, and community hospitals. His areas of expertise include taxable and tax exempt financings in the public and private markets, derivative transactions, mergers and acquisitions, and enterprise risk management. Prior to joining Kaufman Hall, Mr. Freel was a Vice President in the Healthcare Investment Banking Group at Goldman Sachs. In that role, he led a variety of taxable and tax-exempt security offerings and strategic advisory assignments for some of the nation s largest healthcare organizations. Prior to Goldman Sachs, Mr. Freel was a Director in the Healthcare Finance Group at Citigroup, where he developed capital structure and strategic solutions for health systems of all sizes. His previous positions include serving as a Vice President at UBS Investment Bank and as an accountant at Ernst & Young. Mr. Freel has an M.B.A. from the University of Chicago Booth School of Business and a B.S. in Accounting from Miami University in Oxford, Ohio.

10 Fostering Physician Leaders in Your Organization Facilitators: Matthew Lambert, M.D., and Plamen Todorow State Room Matthew J. Lambert III, M.D., Senior Vice President Matthew J. Lambert III, M.D., Senior Vice President of Kaufman Hall, has more than 40 years of healthcare experience working as a physician, healthcare executive, and board member. He is a member of Kaufman Hall s Strategy practice, and is focused on assisting hospitals and health systems with integrated strategic and financial planning, service line planning/distribution across systems, and medical staff planning/physician alignment strategy. Prior to Kaufman Hall, Dr. Lambert held senior executive positions at several Chicago area hospitals. He also has provided consultation to hospitals and health systems in the areas of physician relations and continuity of care. Earlier in his career, he was a faculty member in the Department of Surgery at the University of Virginia School of Medicine in Charlottesville. Dr. Lambert served seven years on the board of directors for a multihospital system, including as Vice Chair. He also has served on both the American Hospital Association s Leadership Development Council and Committee on Governance. Dr. Lambert was a regent for the American College of Healthcare Executives (ACHE), is a frequent speaker at ACHE annual meetings, and developed and leads its annual Physician Executive Boot Camp for physicians new to management. Dr. Lambert s book, Leading a Patient-Safe Organization, was published by ACHE s publishing arm, the Health Administration Press, in Dr. Lambert received an M.D. from St. Louis University School of Medicine, an M.B.A. from the College of William and Mary in Virginia, and a B.S. from the University of Notre Dame. He completed a residency in General Surgery at the University of Michigan Medical Center. Dr. Lambert is serving a three-year term on the Board of Trustees of the Catholic Health Association of the United States. Plamen Todorow, Senior Associate [email protected] Plamen Todorow is a Senior Associate in Kaufman Hall s Strategy practice with responsibilities focused on physician advisory services, population health management, and market strategy. Prior to Kaufman Hall, Mr. Todorow was a Senior Consultant at FTI Consulting. He has extensive experience in turnaround and change management, financial modeling, data analytics, practice assessments, outpatient workflow, valuation, and capital optimization. Mr. Todorow held previous positions as a Senior Consultant at Navigant Consulting and as a Delivery Consultant at Cerner. At Navigant, he served as an advisor to hospital and health system executives across the country on operational strategy and physician-hospital alignment. The scope of client engagements included guiding assessments, productivity enhancements, operational redesign, and compensation analysis for academic medical centers. Other areas of focus included developing workforce efficiency benchmarks, and revenue cycle and patient access tools. Mr. Todorow holds a B.S. in Finance from Indiana University s Kelley School of Business, and is pursuing an M.S. in Predictive Analytics from Northwestern University.

Singh, Managing Director Governance Issues during Healthcare s Transformation Facilitators: Anu Singh and Ellen Riley Lakeview Room Anu Singh, a Managing Director of Kaufman Hall, provides financial advisory services to healthcare organizations and companies nationwide. Mr. Singh leads the evaluation, structuring, negotiation, and execution of mergers, acquisitions, partnerships, joint ventures, and other forms of transactions. He also leads projects involving the assessment of strategic options, growth strategies and business unit/segment viability. His clients include national and large multi-regional health systems, academic medical centers, community hospitals, large physician practices and groups, health insurers, nonacute care providers, and capital providers to health care entities. Mr. Singh has advised on or evaluated more than 100 healthcare transaction-related engagements. Prior to joining Kaufman Hall, Mr. Singh was a Director at Huron Consulting Group, within its Corporate Advisory Services practice. His previous experience includes the execution of turnaround and restructuring plans, and advising on mergers, acquisitions, and other transactions. Mr. Singh also has raised capital for a variety of clients, ranging from underperforming or distressed companies to well-capitalized, investment-grade companies across multiple industries. He has extensive overseas work experience, particularly on behalf of aid agencies, Central Banks, and other parties involved in major financial markets. Mr. Singh has presented at various industry and professional events and seminars, including those sponsored by The Governance Institute, Healthcare Financial Management Association, Volunteer Hospital Association, Becker s Hospital Review, National Federation of Municipal Analysts, and the Turnaround Management Association. He also has authored articles and been quoted in a number of industry publications. Riley, Senior Vice President [email protected] Ellen Riley is a Senior Vice President of Kaufman Hall, working from the firm s Los Angeles office since With more than 25 years of experience in the healthcare industry, her areas of expertise include financial and capital planning, development and implementation of capital allocation processes, strategic options assessments, strategic cost management and financial advisory services in support of debt transactions and business valuations. Ms. Riley has worked with a wide range of healthcare organizations, including health systems, academic medical centers, specialty hospitals, district hospitals, community hospitals, and children s hospitals. Ms. Riley is a regular speaker on healthcare finance topics at Healthcare Financial Management Association s regional and local chapter programs, National Association of Children s Hospitals and Related Institutions, California Hospital Association, National Council of Health Facilities Finance

Additionally, Ms. Riley is a member of the Health Advisory Board of the USC Sol Price School of Public Policy, where she also is an Adjunct Associate Professor for the Health Administration Program. Prior to her tenure at Ernst & Young, Ms. Riley was a Project Analyst in the Corporate Acquisitions and Development Department at National Medical Enterprises, Inc., where her responsibilities focused on financial analyses and due diligence related to hospital acquisition and development projects. Ms. Riley has an M.B.A. from the University of Southern California, with a concentration in Finance and Marketing, and a B.A. magna cum laude, from the University of California at San Diego.

13 James J. Pizzo, Managing Director Improving Physician Practice Performance Facilitators: James J. Pizzo and Todd Fitz Lakeview Room Jim Pizzo is a Managing Director of Kaufman Hall, responsible for directing the firm s Physician Advisory practice, an integral part of the Strategy practice. With more than 30 years of healthcare consulting experience, Mr. Pizzo has held leadership positions as a Partner with Accenture, Cap Gemini Ernst & Young, Ernst & Young LLP, and Ernst & Whinney. At each of the firms, he led the Physician Services and Finance and Planning practices. Mr. Pizzo first assumed the role of Partner and Practice Leader in At Accenture, Mr. Pizzo also was responsible for the Great Lakes Region, working with diverse clients, including freestanding hospitals, academic medical centers, physician and ambulatory providers, more than 20 of the largest health systems, and other healthcare organizations. His areas of focus included financial, strategic, and operations planning, and physician operations and ventures. As hospitals and health systems face the challenge of developing and sustaining physician relationships in a rapidly changing environment, Mr. Pizzo brings solutions to help organizations align with their physicians. The Physician Advisory practice includes: physician growth planning; hospital-physician integration strategy development and implementation; physician work-effort allocation; physician performance optimization; physician-related mergers, acquisitions, and joint ventures; and comprehensive planning for academic physician enterprises and physician affiliates. Todd W. Fitz, Senior Vice President [email protected] Todd Fitz is a Senior Vice President in Kaufman Hall s Strategy practice. His responsibilities include strategic planning assistance to hospitals and health systems nationwide, focusing on network development, physician integration and governance, performance improvement, and academic medical centers strategy. With more than 20 years of healthcare experience, Mr. Fitz has applied his expertise in finance, strategy, and operations to a variety of settings, including community hospitals, physician groups, AMCs, insurance organizations, government agencies, and medical device companies. His work has included major strategy and operations projects with leading healthcare organizations both in the U.S. and internationally.

Speaking engagements have included presentations to industry associations, medical staff, organizational boards, community groups, and annual conferences. Prior to joining Kaufman Hall, Mr. Fitz led the Healthcare Consulting practice at HOK an integrated planning and design firm.
