1 2012 LACC Clinical Obligations & Grading System Attendance Requirement The Radiology program has a Monday through Friday schedule during the fall, winter, spring and summer semesters. Students are required to be in class and submit all required assignments during the semester. As part of their clinical training students may be assigned weekend and evening shifts to provide a complete clinical education. A student who becomes ill or injured while in the radiology program must submit documentation in the form of a doctors release that states they are able to meet the physical and mental requirements of the radiology program. At the discretion of the program director and clinical instructor the students may be allowed to make-up the missed time or assignments. If any student is absent for more than two weeks the time cannot be made up and the student will be asked to withdraw from the program and reapply the following year. All attendance policies including tardies will be in effect during the clinical training. Any student who reports to their clinical training site after their scheduled start time (including breaks and lunches) will be considered tardy. Two tardies will be equal to one absence. Tardies will result in disciplinary action by the college and the clinical training site which may lead to termination of a student s participation in the radiology program. All students in clinical training will be required to complete weekly timesheets. The timesheets must be submitted to the Program Director by the student every Friday via or fax radtkejg@lacitycollege.edu or fax: and a copy must be kept at the student s clinical site. Failure to provide this information will result in disciplinary action by the college and the clinical training site which may lead to termination of a student s participation in the radiology program. Competency Form The student will be required to complete a set number of clinical competencies established by the ARRT. These competencies must be signed off by a qualified Radiologic Technologist. A qualified Radiologic Technologist has an ARRT & CRT credentials, a minimum of two years full time experience, and must be a staff employee of the clinical site as defined in JRCERT form 1012R(CA). There are three different proficiency levels that students complete prior to requesting a sign off for a competency. Level 1 proficiency- The students must take part in the completion of the procedure. If the supervisor feels that the student did nothing more than stand around, the supervisor should not
2 sign off for that particular procedure. Students should review the procedure manual, help setup the equipment, and assist in the completion of the study. Level 2 proficiency- The student must actively take part in the completion of the procedure. The supervisor may offer advice, add to the patient explanations, and assist with repositioning when necessary, but the setup, the handling of the patient, the initial positioning, the execution of the procedure, the completion of paperwork, and the annotation and distribution of the images must be done by the student. If the supervisor feels that they had to provide more assistance than necessary, and that the study would have been compromised without much of their input, the supervisor should not sign off for that particular procedure. Level 3 proficiency- The student must complete the procedure with observational supervision only. The staff member is NOT to provide assistance to the student. The exception is critically ill patients in which assistance in moving the patient may be necessary. If the supervising staff member feels the need to step to avert a compromised study, the supervisor will do so and the procedure cannot be signed off as complete. When a student has reached the 3 rd proficiency level they may ask a qualified Radiologic Technologist to observe and completion the competency form for that particular examination. No student will be asked to perform at a proficiency level in which they do not feel comfortable with. However, to meet the ARRT and LACC graduation requirements, students will be expected to complete a minimum of 10 mandatory and 5 elective competencies each semester (summer, fall, spring). During the winter semester students must complete a minimum of 5 mandatory and 2 elective competencies. A total of 58 competencies are required to satisfy the Los Angeles City College Clinical Internship guidelines. (35 mandatory and 17 electives, 6 mandatory general patient care activities). Students who do not complete the minimum number of competencies or do not perform them at a 90% competency level will be counseled and reassigned to the same area until the desired competency level is achieved or it is determined by the hospital staff, clinical instructor and program director that the student has failed this aspect of their training and is not capable of working in a hospital environment. The student will be terminated from the program upon mutual agreement of the clinical coordinator and program director. All evaluations will be discussed with the student and signed by the student and the clinical coordinator or clinical instructor.
3 The following criteria will be utilized to correctly complete a competency form: The student must begin and complete the examination from start to finish without any intervention from the clinical instructor. The student must correctly complete all of the guidelines set forth by the competency form under the observation of a qualified Radiologic Technologist (clinical instructor). The competency procedure entails the student: Verifies the correct patient, procedure, and clinical diagnosis/indication Provides clear and complete explanation of procedure to age appropriate patient Obtains patient assessment, preparation, and history. Insures patient privacy and dignity. Knows emergency protocol for Code situations. Insures patient safety and comfort throughout procedure. Dresses appropriately for isolation patients. Knows how to operate bedside controls and remembers to put side rail up after the exam. Provides proper breathing instructions; appropriate for exam and patient language. Removes unnecessary personnel/family members during exposures. Correctly enters patient information. Selects appropriate exam protocol or adjusts as necessary. Adjusts exposure factors and selects proper image receptor. Positions patient efficiently and correctly for the examination. Uses proper immobilization devices. Uses tube angulations properly. Knows storage location of necessary supplies. Examination completed in a timely fashion. Properly archives images to PACS or prints images. Uses proper SID. Uses correct film sizes. Uses proper alignment of x-ray tube to film. (Bucky or grid) Uses proper central ray location. Uses proper orientation of Image Receptor. (Cassette, image intensifier) (Lengthwise, crosswise) Uses personal lead markers on all images.
4 Overall Clinical Grading System for 1 st and 2 nd year students RT 160 Students (1 st year students from February to June) RT 160 Clinical Performance: RT 160 Competency Evaluation Forms Attendance Assignments, Homework, Surveys 70% of grade 25% of grade 5% of grade Overall, the list of mandatory RT 160 Competency Evaluation Forms include Reception Area (Deadline: May 24, 2013) Radiology Equipment (Deadline: May 24, 2013) Image Receptor Processing (Deadline: May 24, 2013) Patient Care and Management (2 Copies) (Deadline: May 24, 2013) Personal Assessment Evaluation (2 Copies) (Deadline: May 24, 2013) RT 160 Competency Evaluation Form Guidelines The Reception Area Competency, Radiology Equipment Competency and Image Receptor Processing Competency evaluations may be completed by the person in charge of the area of rotation or by a qualified Radiologic Technologist. Students must perform their assignments with a 90% competency level. Those who do not will be counseled and reassigned to the area until the desired competency level is achieved or it is determined by the hospital staff, clinical instructor and program director that the student has failed this aspect of their training and is not capable of working in a hospital environment. The 160 Competency Evaluation Forms must be sent to the program director at the end of each
5 Patient Care and Management 5 th week and 10 th week Competency Form Students in RT 160 will have a competency evaluation on Patient Care and Management during their 5 th (Deadline: March 8, 2013) and 10 th (Deadline: April 19, 2013) weeks of the semester. Students must maintain a 90% competency level. The forms must be completed by a qualified Radiologic Technologist. Students who do not achieve a 90% competency level will be counseled and reassigned to the area until the desired competency level is achieved or it is determined by the hospital staff, clinical instructor and program director that the student has failed this aspect of their training and is not capable of working in a hospital environment. The 160 Competency Evaluation Forms must be sent to the program director at the end of each Personal Assessment Evaluation 5 th week and 10 th week Competency Form Students in RT 160 will have a Personal Assessment Evaluation completed during the 5 th (Deadline: March 8, 2013) and 10 th week (Deadline: April 19, 2013) of the course. Students must achieve 90% on all Personal Assessment Evaluations. The forms must be completed by a qualified Radiologic Technologist. Students who do not achieve 90% competency level will be counseled and reassigned to the area until the desired competency level is achieved or it is determined by the hospital staff, clinical instructor and program director that the student has failed this aspect of their training and is not capable of working in a hospital environment. The 160 Competency Evaluation Forms must be sent to the program director at the end of each
6 RT 160 Assignments, Homework, Surveys Students will be required to complete all assignments specifically, homework and program evaluation surveys. RT 180 Students (Second year students from June to June) RT 180 Clinical Performance Attendance Examinations Assignments, Homework, Surveys 65% of grade 5% of grade 25% of grade 5% of grade By the end of the clinical training RT 180 students must demonstrate competence in the areas listed below. Six mandatory general patient care activities. Thirty-one mandatory imaging procedures. Fifteen elective imaging procedures to be selected from a list of 35 procedures. One elective imaging procedure from the head section. Two elective imaging procedures from the fluoroscopy studies section, one of which must be either an Upper GI or a Barium Enema. To demonstrate satisfactory progression in program students must complete a minimum of 10 mandatory and 5 elective competencies each semester (summer, fall, spring). During the winter semester students must complete a minimum of 5 mandatory and 2 elective competencies. 7 The 180 Competency Evaluation Forms must be sent to the program director at the end of each Comprehensive Progress Evaluations: 3 rd Month, 6 th Month, 9 th Month, 12 th Month Students in RT 180 will have Comprehensive Progress Evaluation during their 3 rd (Deadline: September 27, 2013), 6 th (Deadline: December 13, 2013), 9 th (Deadline: March 28, 2013) & 12 th (Deadline: June 6, 2013) Months. Students must maintain a 90% competency level. The forms must be completed by a qualified Radiologic Technologist. Students who do not achieve 90% competency level will be counseled and reassigned to the area until the desired competency level is achieved or it is determined by the hospital staff, clinical instructor and program director that the student has failed this aspect of their training and is not capable of working in a hospital environment. The 180 Competency Evaluation Forms must be sent to the program director at the end of each RT 180 Didactic Requirements while in Clinical Training: There will be scheduled weekly review quizzes that students must pass with a cumulative score of 75%. RT 180 Assignments, Homework, Surveys Students will be required to complete all assignments specifically, homework and program evaluation surveys.
