1 Kim Byers Kim Byers is the Quality Improvement Manager for the Louisiana Medicare Quality Improvement Organization, eqhealth Solutions. She has a bachelor of science and is a licensed nursing facility administrator. Kim has a background as a Nursing Home Administrator, Certified Resident Activity Director, and a Certified Nursing Assistant. She has a working knowledge of state and federal nursing home regulations, implementing policies, survey process, plan of corrections, admission process, chart reviews, and education in quality. She currently works in the patient safety field in multiple healthcare settings state wide, such as hospital and nursing homes. As the quality improvement manager, she provides education to quality staff to align their quality departments with the CMS goals, such as physical restraint reduction and pressure ulcer improvement. She assists providers in recognizing areas of improvement, setting and meeting goals, implementing best practices and sustaining the improvement efforts. Kim is a member of the Louisiana Oral Health Coalition, LEADER, and Advancing Excellence. She has been an excellent guest speaker for numerous organizations.

5 Myles V. Holyfield Myles V. Holyfield graduated from LSU in 1993 with a Bachelor of Science degree. He has been working the healthcare field since 1991 with the majority of his experience in the long term care industry. He became a licensed Nursing Home Administrator in 1993 and worked as an Administrator until He was hired as a Regional Manager for Central and North Louisiana by a private contract rehabilitation provider in In 1998 Myles moved in to the consulting field. He worked as a Regional Healthcare Consultant in Louisiana, Texas and Arkansas. Although his main focus was the implementation of Medicare PPS, he maintains a variety of knowledge related to regulatory compliance and reimbursement. Myles has since started his own consulting company, Agentive Healthcare Consulting in Agentive provides compliance, operations, reimbursement and therapy management for nursing facilities in the southeast. Myles has provided training for a variety of positions in long term care. He and his staff have expertise in the clinical field, administration and operations as well as billing services. Agentive s specialty is Medicare SNF Compliance and Medicaid Case Mix related education, training and auditing. Agentive Healthcare Consulting provides services to over 30 long term care providers as well as several rehabilitation contractors and claim processing contractors. Most recently, Myles participated in the CMS Train The Trainer MDS 3.0 Conference in Baltimore, MD in April of The Agentive MDS 3.0 training and education is based on the instruction provided by the CMS professionals from the Conference.
6 Gerardo Ortiz Gerardo began his career with CMS in November 2000 in the Division of Medicaid and State Operations. In Medicaid, he worked on Home and Community Based Services (HCBS) Waivers, Medicaid Eligibility and the State Children s Health Insurance Program (SCHIP). In 2005, he moved to the Division of Survey and Certification and worked as the State Representative for both Oklahoma and New Mexico. He was also the Long Term Care (LTC) Initial Certification Lead for Region VI, working to further streamline and improve the LTC Initial Certification process. Gerardo currently serves as the Manager of the Long Term Care Certification and Enforcement Branch and is responsible for Medicare certification and enforcement programs for a five-state region involving over 2000 long-term care providers. The professional staff in his branch interpret, implement and enforce federal regulations in partnership with state health departments to ensure the health and safety of residents in Medicare-certified long term care facilities. Gerardo received his undergraduate degree in Psychology at Texas A&M University, and his Masters of Health Services Management from the University of Mary Hardin-Baylor. 7 Julie Kueker Julie Kueker, MBA, MT (ASCP) is a Quality Improvement Specialist for eqhealth Solutions, the Medicare Quality Improvement Organization. She has worked in healthcare for 25 years, with the last 15 years having the specific focus of quality improvement. She currently works with nursing homes across the state of Louisiana to assess current systems, identify opportunities for improvement, and the design and implementation of improvement strategies for pain management, depression, pressure ulcers and restraint reduction. In addition, she performs educational programs for nursing and medical staff to heighten awareness of accepted practice guidelines and best practices. She has co-authored a CNA educational pressure ulcer prevention program, the Skin Care Fair. This has been spotlighted on the CMS s Medicare Quality Improvement Community website, and utilized by other state Quality Improvement Organizations. She was a guest lecturer at the Quality Improvement Organization s national Tri-Regional Conference on Workforce Retention in St. Petersburg, Florida. She has also been a guest lecturer for a national conference in New York City for the American Society of Clinical Pathologists. The program was entitled, The Elements of a Quality Agenda. Has been a guest lecturer for Louisiana Nursing Home Association many times, and a speaker for the LEADER the Culture Change Coalition for the state of Louisiana. Has successfully obtained two grant projects through the state of Louisiana, and was conference planner for both state-wide projects that included national recognized topic experts as speakers. Her Master of Business Degree and undergraduate Bachelor of Science in Medical Technology degree are both from Louisiana State University.
8 Wilma Marler Wilma Marler, MSA, QMHP, is a Quality Improvement Specialist with the Nursing Home Team of eqhealth Solutions (eqhs), joining the Patient Safety team in October She works closely with nursing homes in Southwest Louisiana as well as providing support in achieving the goals of nation-wide efforts including The Center for Medicare and Medicaid Services (CMS) Nursing Home Quality Improvement Project (NHQI), The Advancing Excellence in America s Nursing Homes Campaign and the Five Star rating system through CMS/DHH. As the Quality Improvement Organization for Louisiana, LHCR works closely through the team approach to achieve quality care in Louisiana s nursing homes and to facilitate culture change She has worked in the long-term care industry since 1988 in various roles, beginning with Adult Day Services as the Executive Director of Friendship House, adding skills as a Qualified Mental Health Professional (QMHP) in the Geri-psyche services provided through nursing homes where her experiences included topic areas such as depression and stress, as a court appointed curator for the elderly and disabled, both inpatient and partial day programs, administrator of an assisted living facility, and lastly as an elder victims of crime advocate. She has served as a board member for the Vernon Parish Elder Crimes Committee and formerly a participating member of the Region VI Senior Coordinating Council. Her Masters of Science in Administration from Central Michigan University is with a concentration in health services with a Bachelor of Arts in Sociology from Louisiana College.
9 Dr. Susan Nelson Dr. Susan Nelson is chairwoman of the LaPOST Coalition, a network of Louisiana health care professionals raising awareness of the LaPOST document. Board certified in internal medicine, geriatrics and hospice and palliative medicine, she serves as medical director of St. Joseph Hospice in Baton Rouge. Dr. Nelson is a fellow of the American Academy of Hospice and Palliative Medicine and the American College of Physicians, as well as an advocate for senior health. She works with the Louisiana State Medical Society Council on Public Health and the Louisiana Department of Health and Hospitals. A University of Houston graduate, Dr. Nelson earned a medical degree from the University of Texas Medical School at Houston, where she also completed her residency and served as chief resident. While at UTMSH she was an assistant professor in the department of internal medicine. 11 Dana Territo Dana Territo is currently the Director of Services at Alzheimer s Services/Charlie s Place in Baton Rouge, LA and also serves on the Board of Directors for the National Adult Day Services Association. A long-time advocate of those suffering from Alzheimer s disease, Dana has worked with individuals with Alzheimer s and dementia for over 20 years. In 1991, she developed the activity programming for the first specialized Alzheimer s unit in a long-term care setting in Baton Rouge and later was honored for two consecutive years by the Governor s Office of Elderly Affairs for having one of the Top 5 programs in the State of Louisiana to benefit the elderly. As Director of Charlie s Place, the Alzheimer s Services Adult Day Respite Center, Dana oversees the day-to-day programming and organization of the Center, a center which received the 2008 Day Center of the Year by the National Adult Day Services Association and the 2011 Excellence in Dementia Care Program of Distinction by the Alzheimer s Foundation of America. Dana also directs the daily programs and services at Alzheimer s Services, which include answering calls on the organization s Helpline system, overseeing educational programs such as Lunch-n-Learn and the annual Educational Conference, November Awareness activities and also social activities for caregivers. Working with two professors from Louisiana State University in Baton Rouge, Dana developed curriculum for the new state required Dementia Care Training for nursing facilities and adult residential communities. The curriculum was certified and approved in January, 2010, [the first and only in Louisiana at the time], and the first class was launched in February, Dana has given a variety of training and presentations on Alzheimer s, including Behavior Management, Communication Strategies, Therapeutic Activities, Brain Health, and Spirituality and Dementia. She joined the LEADER Board of Directors in early 2011 and is currently taking the lead on the CMS Partnership to improve Dementia Care initiative.
