Attach a list of supporting documents sent with the Questionnaire. Attach the list just after the last page of this self-filling Questionnaire. 3. All the annexures and supporting documents attached with the Questionnaire should be duly signed by the Dean or Registrar or an equivalent authority. The Questionnaire will not be processed in the case of non-submission of relevant supporting documents. 4. Make sure that all information is filled in the space provided in the Questionnaire. 5. If information is not available or the question is not applicable for the B-School on any particular area, please write NA clearly in the space provided. 6.
