2 About the service Quarriers Ventilation Service registered with the Care Inspectorate in June 2014 to support children who require overnight ventilation or tracheotomy support throughout Scotland. The manager is based in Quarriers Chavey Down project (Newton Mearns, Glasgow). Staff from the service stay in the family home overnight and remain with the child being supported. By doing this parents can have an undisturbed nights sleep. The service was supporting two children during this inspection. An additional two children had been funded to receive support. Information about all care services is available on our website at What people told us We had the opportunity to speak with the parents of both children being supported during the inspection. They told us that: "Staff are respectful and brilliant." "My only issue is around staffing levels but I understand that this is due to training and is not Quarriers responsibility." "Cannot think of any fault." "My daughter is very happy with them." "Nothing to complain about." When we spoke to a health professional they were highly complimentary of the service whilst acknowledging training issues. Self assessment The Care Inspectorate received a fully completed self-assessment from the manager. The manager identified what she thought the service did well and gave examples of improvements. The selfassessment identified some areas that the manager believed can be improved and showed how the service intended to do this. The manager told us how the people who used the service can influence its development and improvement. 3 What the service does well The Ventilation Service provided overnight support to children using ventilation or tracheostomy equipment. This is done under the supervision of NHS Ventilation Services and Paediatric Nurses. Before providing support the service carries out an initial assessment which considers: support needs, personal care, diet, medication and ventilation support needs. The latter are linked to NHS assessments and guidance. The ventilation service's practice guidelines were amended from Health Board (Yorkhill) "House Rules" and negotiated with parents and the NHS Ventilation Team. They give clear guidance regarding staff and parents roles and responsibilities, for example staff will not give out medication and that a parent or other responsible adult nominated by the parent must be in the house at all times while support workers are on shift. Children have Quarriers and NHS support plans. The former is linked to SHANNARI indicators whilst the latter outlines the support that is required to be provided to maintain the child's health. This includes details of the equipment to be used and the level of oxygen to be maintained. SHANNARI indicators come from the Getting it right for every child (GIRFEC) approach and say that children should be: safe, healthy, achieving, nurtured, active, respected, responsible and included. Plans contained a range of information such as:- Children and Young People Acute Deterioration Management (CYPADM) forms, personal emergency evacuation plan (PEEP), overnight care plan, NHS Patient Care Plan, home service ventilation agreement and risk assessments. Given the nature of the service support plans are very specific regarding ventilation support and staff practice around this area. Plans are reviewed every six months and more frequently if needed. Records show that staff receive a range of training covering: Tracheostomy care, Tracheostomy Carers Competency Pack, Suction protocols, Child protection and Scottish Vocational Qualifications in Social Care. Staff receive refresher training as required. Much of this training is provided by and to a standard accredited by the NHS. The manager is working with the NHS to look at how training can be provided when the latters trainer is unavailable. We found that managers visit services regularly to monitor the service and discuss children's support needs. What the service could do better The manager should consider using examples of "positive outcomes" when completing the service's selfassessment.
