1 PRE-REGISTRATION NURSING SKILLS PASSPORT Adult Nursing Student Name: Student Number: Cohort: Students must complete the pre-registration nursing skills passport as this is a mandatory course requirement. The pre-registration nursing skills passport provides evidence to inform the assessment of the NMC Essential Skills Clusters and Standards of Proficiency in the Placement Assessment Document. Students are responsible for ensuring that the pre-registration nursing skills passport is completed on an ongoing basis throughout the programme. 1
2 Guidance Notes: Assessment of skills using the pre-registration nursing skills passport The pre-registration nursing skills passport provides one form of evidence that mentors should use when making judgments about the student s achievement of the NMC Essentials Skills Clusters and Standards of Proficiency in the Placement Assessment Document. It is an assessment for learning tool and as such it is the responsibility of the student to ensure that it is completed throughout the full period of training. It can subsequently be used, by the student, towards continual development of their personal and professional portfolio. The supplementary skills passport contains: Skills are those that are identified as being central to the role of an adult nurse and, in this context, are supplementary to the Essential Skills identified in the Placement Assessment Document. These skills are likely to be available in most adult placements. Additional skills are those that are important to the role of an adult nurse. However, these skills may not be available in all placements, although students may encounter them at some point. Students should attempt to achieve additional skills if these are available and it is appropriate for students to undertake these in light of their knowledge, experience, service user s wishes and Trust policies and procedures. Mentors should be aware that they remain accountable for delegating tasks to the student and must: 1. Be able to justify the decision to allow the student to carry out the practice 2. Provide an appropriate level of supervision whilst the student carries out a procedure. 2
3 Assessing the student s skills: The student and mentor are required to plan how the student will achieve each skill by identifying which of the skills listed in the passport are available to the student during the placement. They should do this on an ongoing basis and by reviewing the student s progress in the initial, intermediate and final interviews. Students are required to attempt all skills that Trust policies and procedures allow and their mentor deems appropriate (i.e. students cannot opt out of being assessed). The student s performance should be assessed only for the skills that are available and that the mentor deems appropriate. Mentors should use a team approach to assess the student by gathering the views of their colleagues about the student s performance. The mentor should consider two elements when assessing the student: 1. Observation of the student s performance 2. Checking the student s knowledge by asking questions and / or reviewing one further piece of evidence provided by the student e.g. a short reflective account which the student may have completed in their Placement Assessment Document. N.B. A student s level of proficiency may fluctuate between placements for a variety of reasons. Mentors should use their professional judgment to determine whether the student has demonstrated sufficient evidence of skills achievement to warrant a pass in the related NMC proficiencies. 3
4 Achieving the skills in a safe and effective manner requires the student to: 1. Adhere to appropriate policies, procedures and legislation 2. Prepare and maintain a safe environment 3. Use and discard or dispose of equipment and aids appropriately and safely 4. Seek assistance where this is required 5. Emotionally and physically support the client / client / service user. Mentors should assess the student and enter that the student has achieved by signing in the appropriate box. Fraudulent entry of assessment outcomes or signatures by students in any document is a serious disciplinary matter that will be investigated by the University. This document should be completed in ink and must be available in the clinical setting at all times. All parts of this document requiring a signature should be signed by the appropriate person. It is the student s responsibility to provide this document when required. 4

Responds appropriately to any adverse effects 2d. Safely disposes of blood / blood products and equipment 5

Assists with toileting needs, ensuring privacy and dignity 3c. Assists in assessing the need for patients to have an indwelling catheter 3d. Identifies and prepares the equipment for patients requiring urinary catheterisation 3e. Adheres to policy and procedures for insertion of a urinary catheter 3f. Manages the care of an indwelling urinary catheter effectively 3g. Assists in assessing patients with bowel problems (constipation / diarrhoea) 3h. Assists in planning the care of patients with bowel problems (constipation / diarrhoea) 3i. Assists in implementing care for patients with bowel problems (constipation / diarrhoea) 6

Assists in evaluating care for patients with bowel problems (constipation / diarrhoea) 3k. Assists in providing health education, including dietary advice, for patients with bowel problems (constipation / diarrhoea) 3l. Attends to the hygiene needs of patients after toileting. 4. Moving and Handling 4a. Ensures the safety of patients, staff, self and others including co-handlers 4b. Assists in completing a comprehensive assessment of the individual patient s moving and handling needs9b. Assists in completing on the spot assessment using appropriate assessment frameworks (e.g. LITE: task, organization, individual capability, load, environment, equipment or TILE) before commencing the manoeuvre 7

Works with others to plans and implement moving and handling manoeuvres 4f. Communicates effectively with the patient and all involved. 4g. Co-ordinates co-handlers 4h. Selects and uses appropriate aids and equipment as per individual patient assessments. 4i. Contributes to the risk assessments of tasks in a specific workplace to prevent injury, obtaining appropriate assistance where necessary 4j. Reports incidents verbally and assists in completing all relevant documentation (e.g. self and patient untoward incidents). 8

Assesses oxygenation / circulatory condition and needs using observation skills 5c. Uses pulse oximetry and is aware of its limitations 5d. Selects and assembles appropriate equipment for oxygen administration and airway management ensuring correct dose as prescribed 5e. Assesses effectiveness of oxygen therapy and airway management, taking appropriate action whilst maintaining safety 5f. Takes appropriate action to ensure safe administration of oxygen / medicines. 5g. Records and reports appropriately all relevant data 5h. Demonstrates understanding of issues and acknowledges limitations when caring for patients with respiratory problems / receiving oxygen therapy. 9

Assesses the pain of those who are unable to verbally express pain / have difficulties communicating. 6c. Positions the patient appropriately 6d. Implement pain relieving interventions: Pharmacological Non-pharmacological Alternative therapies 6e. Evaluate, act upon and document a patient s response to interventions. 6f. Assist in referral to and liaison with other members of the multidisciplinary team as appropriate. 10
