4 1 SOMC Development Foundation Corporate Partnership Program July 1, June 30, 2017 Our Mission: To make a difference by identifying our community s health and wellness needs while bridging the gap between those needs and the cost of the solutions. SOMC Corporate Partnership Program SOMC is committed to making a positive difference in the lives of our patients, employees and the communities we serve. Our corporate partnership program was created to help raise awareness about SOMC community events that were established to promote our community s health and wellness needs. Our vision for this program is to establish strong philanthropic partnerships between Southern Ohio Medical Center and our valued corporate partners. We invite you to participate in any or all of our community fundraising events. SOMC Corporate Partner Fair The Corporate Partner Fair will be held on September 15, 2016 from 9am to 2pm at the SOMC Friends Center. All partners that sponsor an amount of $2,000 or more will receive a free booth at the fair. The fair will be advertised to our community as well as SOMC employees. We invite each partner to set up a booth, display products, and provide information about their company to those in attendance. Details about the fair will be disclosed at a later date. Partner Recognition Your organization will be recognized at each of the events sponsored. In addition to individual event recognition, all partners who sponsor an amount of $2,000 or more will receive a booth at the Corporate Partner Fair. They will also have the ability to supply marketing materials in the SOMC gift bags (for applicable events) given to event participants. Community Wellness Champion Those partners who choose to sponsor every event will be named to SOMC s Community Wellness Champions. Community Wellness Champions will receive the same recognition as regular partners as well as quarterly advertisements in the local newspaper, a corporate link on the SOMC Development Foundation s website and a copy of the monthly SOMC Community Health & Wellness Newsletter. In the inaugural year of membership, a free consultation from SOMC Community Health & Wellness will be offered. In subsequent years, a bi-annual visit from Community Health & Wellness will be provided. During this visit, the SOMC Community Health & Wellness team will provide education on a healthy subject. The presentation can be tailored to meet the needs and interests of the partner. In addition, these partners will receive further recognition at every SOMC event acknowledging each partner as a Community Wellness Champion.

5 2 Benefits of Event The proceeds of The Summer Soirée will benefit the SOMC Hospice Expansion Project. A Capital Campaign is underway to enhance the services that Hospice offers to our community. Event Enjoy an evening of classic summer fun with the SOMC Summer Soirée. Come enjoy live music from Bad Habits, a dinner of lobster tail and filet mignon and the natural beauty of an outdoor summer evening. Guests will have many other opportunities for fun, including the chance to challenge one another in a singoff competition. 7 4 Benefits of Event A donation to the Hospice Caritas Fund allows us to make a positive difference in the lives of hospice patients and their families. Your gift ensures that local patients and families coping with a life-limiting illness will receive comfort, support and care in their time of need when the hope for a cure is no longer possible. Event The Memorial Bike Ride is an annual event that was created in memory of motorcycle enthusiast and hospice patient Ricky Bryan. This is a wonderful event that enables the community to come together to support a great cause. Shirts go on sale every year to provide additional support to this event. 9 6 Benefits of Event The Community Health & Wellness Fund is designed to offer support to projects that will positively affect the health & wellness of those living and working in our community. Our goal is to provide a community where families can live a life of health and wellness. Event The SOMC Tri for your Life will begin with a 200 meter swim in the SOMC LIFE Center pool and then progress to a 10 mile bike course on Route 23 with a challenging short loop through the beautiful Feurt Hill neighborhood. The 5k run takes place through the streets of historic Portsmouth on a flat course. 13 10 Benefits of Event The Cancer Compassion Fund assists local cancer patients with transportation, medication, wigs, etc. Your generous support helps a patient and their family during a very difficult time. Event Paint It Pink is a unique 5k event named by Spry Living as one of America s best breast cancer 5k s. Community members celebrate a weekend-long event of painting the community Pink to help raise awareness about breast cancer. The entire community shows support for the festivities by wearing their Paint It Pink t-shirts the day prior to the 5k while decorating their workplace, schools and homes. On October 18th, participants of the 5k enjoy a flat race course through a scenic residential neighborhood. Our goal is to provide a community where families can live a life of health and wellness. Event Help stomp out tobacco use at the Raven Rock Ash Dash. The race is a course with 20 obstacles. It is designed to test your limits in a fun, healthy and exciting adventure. The Raven Rock Ash Dash is a family friendly event and everyone is welcome. 17 14 Benefits of Event Donations to the area of greatest need are undesignated and applied where the funds will have the greatest impact on Southern Ohio Medical Center and our community. Undesignated funds are the backbone of the SOMC Development Foundation, allowing us to allocate the funds when and where they will have the greatest impact, responding to challenges and new opportunities as they arise. Event Ball - November 18th Train Show - November 26th and 27th Polar Express Pajama Party - November 29th Each year, we kick off the start of the holiday season with our Winter Wonderland Ball and Events. The Ball is a black tie, formal event featuring a four-course meal and entertainment. The Polar Express Pajama Party features food, several children s activities, a visit from Santa and a viewing of the holiday classic Polar Express. The Train Show features local train displays along with several booths of trains and accessories. 19 16 Benefits of Event Your support of the Heart & Vascular Fund enables SOMC to offer heart patients a second chance at life through the most advanced technology available. Gifts will also enable SOMC to offer support to those patients who experience financial difficulty in their time of need. Event In honor of National Heart Month, a ladies night out event will be offered in the Month of February. The event will be tailored toward pampering ladies so that that they can take better care of themselves, enabling them to continue to be the heart of the family.
