1 EXAMPLE of LEARNING GOALS Make sure you keep the OT practice process in mind in this section. STAGE 2 SUPERVISED PRACTICE AUDIT PLAN ABC REHAB Name: A Occupational therapy process skills, case management skills & interventions What do you want to achieve? How are you going to achieve it in this workplace? What will you do in your workplace in order to meet your objectives? Make sure your goals are measurable, and be clear on how the aims will be measured and benchmarked. Acquire knowledge of assessments through reading relevant literature. Learn from observing experienced practitioners using assessments. 1. To be competent using formal mental state (MSE) assessment instrument and the services Acute Care Assessment. (ACA). Identify and read relevant literature. Liaise with occupational therapist on Acute Care team to observe assessments. Observation of acute care team member in situ performing assessments. Practice undertaking both Formal mental state assessment appearing in medical charts. Review successful performance of assessments in Attendance record/ CPD for professional development program. 2 assessments under direct observation from OT supervisor. Attendance at professional development program on undertaking formal MSE. 2. To be competent in administering the Domestic & Community Skills Assessment (DACSA) and completing the written report form Acquire knowledge of assessment and its application in this setting. Feel confident in completing assessment with client. Use standard format for report writing. Understand the relevance of this assessment with the OTPP and be able to articulate its use to non OT professions. Acquire copy of assessment. Arrange to be present while assessment is administered by a competent occupational therapist. Independently complete assessment with client under direct observation of occupational therapist. Research evidence base for ADL assessments in this environment. Use supervision session for DACSA when client is referred for assessment. Seek feedback in supervision for administration of assessment and written report. Run education session for non OT staff on the evidence base and application of the ADL assessment. 3 3. To be competent in the use of the service outcome measures scale (HONOS and Lifeskills profile measure). Literature review for the HONOS scale. Use guidance notes for use of the scale. Attend mandatory training in Lifeskills profile measure. Literature search for background on the scale. Identify suitable clients from caseload. Include rehabilitation intervention within client care plan and proceed with intervention. Read the OT competencies in mental health documentation. Use occupational therapy psychiatric rehabilitation perspectives to formulate an intervention for a client. Research evidence based mental health OT interventions and how this applies at ABC rehab. Ensure OT supervisor observes at least three sessions of OT intervention in the first three weeks of supervised practice. Evaluate intervention in Feedback and guidance to be provided by supervisor. Discuss progress specifically in relation to the OT Competencies in mental health document with supervisor in one supervision session. 6. To be competent in using the principles of evidence-based practice as applied to the occupational therapy practice process in this setting To be aware of the best evidence for a range of interventions offered by therapist and the service. Identify with supervisor a number of interventions to research evidence on. Practitioner to conduct library searches on these interventions.
