2 ON GOING ACTIVITIES DINNER & MOVIE Dates: Saturdays, January 2 February 6 & Sunday, March 6 Time: 3:00 7:30p.m. Cost: $22/per date We will meet at Logan Center at 3:00p.m. We will view a matinee show at a local cinema, and go for dinner at an area restaurant. The time of our movie will determine if we eat before or after the movie. Cost will include: your movie ticket, dinner, and transportation. Please bring extra money if you wish to have snacks at the movies. A beverage at the movies will cost $4 - $6, candy is $3 - $4, and popcorn is $4 - $6. Arrange to be picked up from Logan Center at 7:30p.m. PRE-REGISTRATION REQUIRED LIMITED TO 18 PERSONS ATTENTION PLEASE READ CALENDAR DETAILS CAREFULLY! Logan Adult Recreation offers activities in the community and at the Logan Center building for individual s ages 27 and above. You must register to attend any of our activities. Please note, we also offer a Young Adult Recreation Program for ages 18 through 26. Please call Dana Molenda or Alicia Hood at Logan Center if you have any questions about our Recreation Groups or Activities. Please note: Activity details for our trips will inform you what to expect during our activity. Activity details will include what costs will be covered, what you may need to bring with you, and when and/or where to be dropped off & picked up that day. Please keep a copy of our calendar for your records and share activity details with all caregivers/staff. FRIDAY CHIPPEWA LANES Logan Bowling will continue in January at Chippewa Lanes in South Bend. Please see the calendar for specific dates. Please read the information below. Cost will be $3.00 per week to bowl. Cost will include your shoe rental & bowling from 4:30 6:00p.m. CAREGIVERS and/or STAFF NEED TO STAY with you during this event. This is a Loosely Supervised Event. ALL BOWLERS MUST BE ON A LANE BEFORE 5:00P.M. LIMITED TO 80 BOWLERS 20 OPEN LANES 4 PER LANE RELEASE FORMS REQUIRED FOR ALL BOWLERS! Larger Group activities are loosely supervised, and a caregiver will need to attend with you. Activity details will explain what activities are loosely supervised. Recreation events have 1 staff person per 8 individuals attending. When needed, some participants may be asked to have staff stay with them at smaller events to insure they receive enough support during community outings. One-on-one support cannot be provided at all times during events. Remember to always dress according to the weather including your jacket/coat, hat, gloves, scarf, and sunscreen if needed. In the event of inclement weather, activities may be canceled or changed. If an activity is canceled or changed, you will be called at the number indicated on your registration form at least one hour before the activity is to begin. You must be registered to get a call! You may also check the local news channels for closings. If you have questions, call or text Dana Molenda at

RETURN YOUR REGISTRATION FORM TO LOGAN CENTER OR REGISTER logancenter.org LOGAN RECREATION LOGAN CENTER REGISTERING FOR ACTIVITIES: Provide your name & phone number at top of Registration Form. List any special needs you may have for transportation or help during our events. Check the box next to the activity dates you wish to attend. Payment is not required at time of registration. If you send payment, make checks payable to LOGAN Recreation. If an activity is already full & you have sent a payment, your check will be returned with a list of activities you may attend. Logan Dances require you to PRE-REGISTER every 3 months when new calendars are received. Tickets will be sent only if payment is included with your Registration Form. If you are pre-registered, you may pay at the door and receipts will be provided. Dances are offered to anyone 18 years and above served by Logan Recreation. Annual Participation Forms are required to attend Any Activity. Please update forms as needed and keep information current. Please mail, or bring in, your Registration Form to be on our list for the dances. You may also register online for dances on the Logan logancenter.org If you include $3.00 for your ticket with your registration form, you will be sent a ticket and receipt in the mail. If you are preregistered, you may pay at the door. Receipts will be provided. SATURDAYS: January 23 February 20 & March 19 Time: 6:00 9:00p.m. Cost: $3.00 / ticket Doors will open at 6:00p.m. and dancing/music will begin at 6:30p.m. Rides are not provided to or from the dances. Recreation Rules are to be followed at dances! Do not bring outside music to the dance; our DJ will provide the music. You must have an annual participation form on file in order to attend. Arrange to be pick up at 9:00p.m. DANCES ARE LOOSELY SUPERVISED EVENTS - OVER 100 PERSONS ATTEND EACH DANCE! STAFF AND/OR CAREGIVERS ARE REQUIRED TO ATTEND THE DANCE & SHOULD MAINTAIN A LINE OF SITE WITH THEIR PARTICIPANTS AT ALL TIMES, INCLUDING OUTSIDE WHILE THEY ARE SMOKING! A beverage and snack will be provided for all dance participants. Additional pops will be available for 50 each All Behavioral, Dining, and/or Risk Plans (i.e. falls, choking, seizures) should be included. Classes will be held in the Fine Arts Room at Logan Center. A pizza party will be held on the last day of class! For those of you at Logan Industries, a ride will be provided from LI to Logan Center at 4:00p.m. each Thursday. For those of you at Logan Industries during the day on Tuesdays, a ride will be provided from Logan Industries to Logan Center at 4:00pm. We will participate in an instructor led exercise class together. Exercises will focus on dancing and keeping our bodies moving. Please dress comfortable in movable clothing and wear shoes for fitness. Cost will cover bottled water during the classes and the price of our instructor. Please arrange to be picked up at Logan Center at 6:00p.m. each Tuesday. Dates: Sundays, January 3 February 21 & March 20 Time: 1:00 5:30p.m. Cost: $25 We will meet at Logan Center at 1:00p.m. We will go to lunch before going to an area Bowling Alley for a day of bowling. Cost will include your lunch, bowling, shoe rental if needed, transportation, and supervision. Please bring extra money with you for beverages at the bowling alley. You may bring your own shoes & bowling ball if you have them, but they are not required! We will travel to an area fast food restaurant for dinner before going to the Compton Family Ice Arena on Notre Dame Campus to see the Notre Dame Hockey team play. Cost will include your dinner, ticket to the game, transportation, and supervision. Please bring additional money with you for beverages, snacks, or souvenirs at the game. There is a small Notre Dame Gift Shop inside the Compton Ice Arena and concession areas. Please wear your coat, gloves, and hat to the activity; the Ice Arena can be chilly during the games. Arrange to be picked up from Logan Center at 9:30pm. We will participate in a Canvas and Cookies party! We will follow the Art teacher s direction to complete a painted picture on Canvas. Everyone will have their own paints and Canvas to use. We will also enjoy Cookies and Pop as we view each others creations and wait for our paintings to dry! Cost will include your Canvas, painting supplies, refreshments, transportation and supervision. Please dress to get dirty while painting! Arrange to be picked up at 4:30p.m. REGISTRATION REQUIRED LIMITED TO 20 PERSONS MOVIE LOGAN CENTER Date: Saturday, January 9 & Sundays, January 24 & March 13 Time: 3:00 5:30pm Cost: $12 / per date We will meet at Logan Center at 3:00p.m. We will watch a newer Movie on the big screen at Logan Center. Cost will include your lunch, ticket to the show, transportation, and supervision.
