As commercial and government reimbursement models change from fee-for-service to value-based, it is essential to evaluate your strategy to ensure how your organization can achieve effective results. During this session, Caradigm will review key learnings and capabilities required for successful transformation to a value-based care world. Title: Multi Systemic Approach: Prescription Narcotic Misuse and Abuse in Southwestern United States. There has been a huge increase in the quantity of prescription pain relievers sold in United States. Centers for Diseases (US) reports that deaths from prescription pain relievers have reached epidemic proportions (2011). 40 deaths per day with 500,000 Emergency Department visits per year (2009). In the state of Arizona 50% of adults report prescription drug misuse. In 2011, the office of National Drug Control Policy (ONDCP) published the prescription drug abuse prevention plan. In 2013, the Arizona Substance abuse Partnership (ASAP) made this their strategic area of focus. Bringing together Law
2 enforcement, prevention, medical and substance abuse treatment providers along with state pharmacy board and health plans this initiative was launched. Purpose: This presentation will discuss a multi-prong approach developed to combat the epidemic. 1. Reduce acquisition, enrolling in prescription drug monitoring program (PDMP) 2. Educate prescribers and pharmacists about best practices. 3. Enhance prescription drug practice and polices in law enforcement. 4. Increase public awareness about the risk of drug misuse/abuse. 5. Build resilience in local community. 6. Incorporation of Detox and maintenance centers Outcome: Since the program began, these counties have noted a marked decrease in drug seeking patients to the emergency departments and a significant increase in the physician drug monitoring program prescriber sign up. Over one thousand providers have received report cards to prescribing practices. 94 Medical professionals and pharmacists have received training in best practice guidelines. Collection of lbs per month of old narcotics dropped at marked drop boxes. Over 200 law enforcement officers have received specialized training. Public education on misuse and abuse of prescription drugs has reached over an estimated 244, 000 people and detox and maintenance programs continue to help prescription drug abusers taper their use and renew their lives. This initiative provides a great example of various entities teaming up together to address a serious need affecting all communities. Workshop 2 9:35 10:15 Workshop 3 10:20 11:00 1. Development of community partnership with payers and community providers in Az. 4. Increase awareness of different approaches to epidemic. Title: Catalyzing the Shift from FFS to FFV While the term population health is used broadly across the industry, there is no universal understanding of what it actually is. Depending on their network, reimbursement models and level of risk they re assuming, various healthcare organizations may focus on different aspects of population health: improving transitions of care collaborating across care settings engaging patients in a meaningful way. This presentation introduces a new strategy, CareInMotion, to support forward-looking healthcare leaders leaders who are embracing a fluid processof-care approach to improve patient experience and outcomes, replacing the more traditional (and static) point-of-care mindset. Join Martha Thorne, Senior Vice President and General Manager for Population Health Management at Allscripts, to learn how CareInMotion helps healthcare leaders hone in on a select set of population health objectives to achieve their organization s unique goals. Title: Using information to engage physicians and drive results The workshop will discuss the importance of cultural understanding in physician engagement. Workshop 4 11:05 11:45 Keynote Panel 11:50 12:30 Meetings & Networking Title: Leveraging Technology to Change Processes and Implement New Care Delivery Models With the shift to value based care, healthcare providers have to increase panel size and "see" more patients, while lowering the cost of care. We are becoming more focused on preventive care and keeping people out of the hospital. This requires us to use technology and lower cost clinical personnel to touch more people at a reduced cost per interaction. How can we utilize technology - tools such as telehealth, population health management, patient engagement, etc. - to do things differently today, and what infrastructure changes do we need to put in place to use these and other technology tools in the future to enable us to reach the goals of lower cost, better population health and improved access to care. Healthcare organizations have invested significant capital and resources in data scientists, analytical tools and visualization technologies; but in the absence of good Data Governance (DG) and Data Quality (DQ) programs the Analytical, BI and reporting outputs can be inaccurate, misleading and sometimes un-trustworthy. The old adage of garbage in / garbage out still holds true. A well-structured Data Governance and Quality program can increase confidence in analytical results, and increase the speed and frequency of trusted analytical outputs. 1. A basic understanding of Data Governance and Data Quality principles 2. When to engage DG and DQ for transformation initiatives 3. Sharing patient information across providers is problematic today. Data management infrastructures required to support population health management are even more challenging. Join us to share experiences with these two data challenges sharing information about individual patients and data aggregation to support population health. 1. Accurate data capture and representation are necessary prerequisites to data sharing 2. Basic information follows the patient infrastructure exists but is not widely embraced 3. Identify examples of the companies involved in these new offerings, what is included, and how they are being positioned and sold. 2.
