The application of population analytics is a proven approach by which healthcare providers can strike a balance between keeping their revenue solid with fee-for-service contracts while navigating into value-based contracts, whether they are fee-for-service, pay-for-performance, patient-centered medical home (PCMH), or any other variant on the spectrum. Some health systems are successfully using analytics to influence change in their business; specifically, these organizations are using analytics to help them stratify their patient populations according to risk, measure physician and care management team performance, assess patient and population interventions, and identify financial risks and opportunities. Increasingly, care team members must review large quantities of electronic clinical information from disparate sources to make clinical decisions. This is often an onerous and time consuming process which hampers effective and timely communication among care providers. These factors cause nurses and other members of the care team to multi-task, strive to remember important information, and attempt to communicate regularly to complete their work in an efficient and safe manner. To solve this problem of information overload, a mobile icon interface system has been proposed as a solution for improving this situation and providing the right information at the right time to care teams to help make safe and accurate care decisions. This workshop will provide an overview of the Mayo Clinic nursing's approach to the issue and lessons learned to date.
