2 Performance Review Report Summary of Improvements This year s performance shows improvement in most areas identified for attention in the previous annual Review. The council continues to work with partners, in particular health to deliver the national preventative and independence agendas. Joint strategies to respond to the Adult services Green Paper are being actively considered. Particular improvements have been achieved in the area of conducting assessments; in the production of statements of needs, and in the timeliness of subsequent service deliver including equipment and adaptations. More extra care housing schemes are in place according to programme. Performance on delayed transfers of care improved reflecting the success of local joint arrangements, and a dip in performance in community based intermediate care is explained as temporary by the council and as being caused by the relatively recent introduction of new arrangements. Direct payments have increased from a low base, and there is improved performance in helping adults with a physical disability to live at home. There has been improvement, i.e. performance in practice learning indicators for staff. Summary of Areas for Improvement» With CSCI, continue to explore ways to test the generality of issues identified from a small sample during the Supporting People inspection. DIS 2233 STANDARD 1: National Priorities And Strategic Objectives The council is working corporately and with partners to deliver national priorities and objectives for social care, relevant National Service Frameworks and local strategic objectives to serve the needs of diverse local communities from , have been addressed. The council has a considerable number of working partnerships with other agencies to develop and implement strategies, which will deliver national priorities and objectives. Work on National Service Framework (NSF) milestones is progressing. DIS Early discussions have been held across departments, on how to deliver the Green paper aims. The process to deliver the changes in the management structure as defined in the Children s Act have commenced. Comments from ARM meeting Increases, of which some significant, have been made in performance on completion of assessments. DIS Just started work with the Change Agent, alongside North Yorkshire and Leeds councils, on commissioning for population needs. With CSCI, continue to explore ways to test the generality of issues identified from a small sample during the Supporting People inspection. Supporting People Inspection report Breaks for carers. DIS 2716 STANDARD 2: Cost and efficiency Social services commission and deliver services to clear standards of both quality and cost, by the most effective, economic and efficient means available - 3 -
4 Improved performance on Intensive Home Care as a percentage of intensive home care and residential care. Area for Improvement from Annual Review of Performance DIS PAF B11 Problem drug misusers participating in drug treatment programmes. DIS 2603 & PAF A60 Budgetary constraints present risks to service delivery. Comments from ARM meeting Costs for Intensive social care for adults and older people. DIS PAF B12 Unit cost of home care for adults and older people. DIS 2126-PAF B17 Unit cost of residential and nursing care for older people. DIS PAF B13 STANDARD 3: Effectiveness of service delivery and outcomes Services promote independence, protect from harm, and support people to make the most of their capacity and potential and achieve the best possible outcomes The council have continued to promote independence for younger physically disabled people, and people with mental health needs, the first of which was highlighted for improvement in Performance on the delivery of community equipment services has improved. The number of learning disabled people in work as a percentage of those with learning disabilities known to the council. DIS 2217 The number of adults with learning disabilities helped to live at home. DIS 3323 C51 STANDARD 4: Quality of services for users and carers Services users, their families and other supporters, benefit from convenient and good quality services, which are responsive to individual needs and preferences - 4 -
5 Performance for both acceptable waiting times for assessments for older people (Area for Improvement from Annual Review of Performance ), and acceptable waiting times for care packages for older people has improved. DIS PAF D55 Progress has been made concerning the Single Assessment Process. DIS 2121 Assessments of new clients aged 65 or over. DIS E61 The percentage of clients receiving a fell. DIS PAF D40 Percentage of people receiving a statement of their needs and how they will be met. DIS PAF D39 STANDARD 5: Fair access Social services act fairly and consistently in allocating services and applying charges Necessary work concerning the Race Relations Amendment Act has been completed. DIS , 60 Management attention is required concerning assessments leading to provision of service. DIS PAF E50 The ethnicity of older people receiving an assessment DIS PAF E47 The ethnicity of older people receiving services following an assessment. Area for Improvement from Annual Review of Performance DIS PAF E48 Advocacy should be available to all learning disabled service users. DIS 3502 STANDARD 6: Capacity for improvement The council has corporate arrangements and capacity to achieve consistent, sustainable and effective improvement in social services Transforming York programme is improving management arrangements, including service planning processes and performance monitoring arrangements.
